Recurrent UTI (Urinary Tract Infection) Lecture

  • What other pain in the body do we not examine? I argue that the pelvic exam is extremely underutilized in the evaluation of recurrent urinary tract infection.
    How do we actually define a UTI?
    How to intelligently interpret the UA and only use this as a screening test. The Urine Culture is necessary to confirm the infection and that the presenting symptom is actually due to a bacteria.
    What is the recommendation for an acute UTI by the Infectious Disease Society?
    What do you do when people have multiple antibiotic allergies?
    Behavioral strategies to reduce UTIs.
    How we can change our gut microbe and change our UTI risk.
    How your vaginal health affects your UTI risk.
    How your microbiome affects your UTI risk.
    How the post menopausal state increases the risk for UTI.
    Therapeutic interventions for UTI: suppressive antibiotics and the risks associated with antibiotics.
    Does cranberry work? How about Vitamin C? Any prescription preventative measures? UTI Options.
    When is a cystoscopy a good idea in the workup of a UTI?
    Antibiotic stewardship
    Look at the “owie” - Other things that mask as UTI
    How Dr. Casperson works up a patient with recurrent UTI

    I suffered from UTIs for 37 years had at least 7 a year. I was on antibiotics all those years. She discussed Methenamine/Hiprex & Vitamin C this changed my life. I researched further. The Methenamine and the high dose of Vitamin C/probiotics made the difference. I had seen countless specialists over the years and they never mentioned this. I asked my most recent urologist about it and she was willing to write it. It's the answer to solve UTIs along with some of the other obvious things. I give the Methenamine/Hiprex the full credit.

    I had 10 UTIs in the last 1.5 years, probably caused by chronic constipation. E.coli culture showed a couple of times, and I got BV (also E.coli) and yeast infections when I wasn’t on antibiotics. After developing a very meticulous washing routine after sex, I saw my symptoms change over the course of the year and got less painful. I was able to manage it with D mannose, probiotics, and now I believe I need to lay off the alcohol as well as reduce flour intake to feel even better.
    The pooping on a consistent basis is a great reminder! I also drink lemon water and urine alkalinizers from Uqora. I avoid antibiotics like the plague, just because the pain isn’t so great anymore.

    If you have to you may use it as a maintenance dose as well. I also drank lots of water to flush out the bacteria. It is definitely worth a try.

    What an amazing urologist. And I found simple things to try, because my microbiome wishes me to avoid antibiotics! So, lots of water, vitamin C, perhaps vaginal estrogen, and then I'll see how things go. Thank you so much.
    Update: my infection eventually got worse and I was becoming alarmed. I eventually asked my doctor if I could try nitrofurantoin, an older antibiotic that works primarily in the bladder, and has minimal effects on the gut microbiome. And it worked!! Such a relief. And my gut seemed unaffected. Apparently it's prescribed less often because you have to take it twice a day for up to a week. But it's well worth it for me

      @Olivia-bq9rk 2 ปีที่แล้ว +2

      Does estrogen creams really work for that? I’m 15 and I’ve had non stop utis for this entire year. I’m running out of options to stop it from happening again 😭

    • @judithenglish
      @judithenglish 2 ปีที่แล้ว +5

      @@Olivia-bq9rk Hi. I think the estrogen is probably more important for those of us whose bodies no longer produce much. Topical estrogen stops the shrinking of the inner vaginal lips in post-menopausal women, since those lips help protect the urethra from bacteria getting into the bladder. At 15 the causes of your UTIs could be very different. I remember the pain of that and I feel for you!!! One thing that might apply is that I'm working hard to avoid antibiotics as much as I can, particularly Cipro if possible. But I understand trying anything that might work. I hope you find a solution.

    • @Olivia-bq9rk
      @Olivia-bq9rk 2 ปีที่แล้ว +2

      @@judithenglish I just did some research and the topical estrogen helps with the health of the urethra tissue, I’ll talk to my doctor about it and go from there. • @judithenglish
      @judithenglish 2 ปีที่แล้ว +3

      @@Olivia-bq9rkHi, Olivia. I'm using d-mannose every day. I can't really tell if it's helping, but there doesn't seem to be any downside, so why not. I looked up uva ursi, and it seems interesting. But I'm scared off by possible side effects to the liver. So I'm sticking to the d-mannose and making sure I drink 1.5 litres of water every day. I'm also taking 1000mg of vitamin C every day So far this seems to be working. I did manage to avoid antibiotics, but it took about 2 months for the symptoms to go away, and I still don't know if it was smart not to take them

    I want to be empowered with knowledge! I love when doctors give me knowledge! I have reoccurring UTIs and I enjoyed this video. I don't want to take antibiotics because I work so hard on having a healthy gut!
    My questions are: 1. Is the bladder supposed to be an acidic organ or alkaline? 2. Is having UTI issues during ovulation give me a clue that my estrogen levels are bouncing too high? So do I see an urologist or my gyno? 3. How often is medication the issue to occurring UTIs? (I have RRMS and take Vumerity). 4. Is low bile function possibly linked as well? And I ask because you did talk about poop a bit. Low bile can cause a fattier, greasy stool.

    Excellent presentation for a 64 year old female retired nurse with chronic infections every 3 weeks (basically as the bacteria build up from antibiotics). I do all of the proactive strategies but I have a triple pelvic floor prolaps with urethral opening stretched wide and internal tissue exposed. I weigh 112lbs and always do jegal exercises. I get about 3 hours relief from sodium citrate but you can only use it for 2 days at a time. WHAT if anything can I do until covid allows for prolapse surgery? I would love for you to have made it more informative on the menopausal woman. Perhaps a second video? I use estrodial pil l in the vagina twice a week. No use. 😰 The vulval pain and bladder ache is miserable. The freezing and sweating is miserable. The relief after a 7 day course of antibiotics is sooooo lovely. Thanks

      If you have not had your surgery yet, you can always be fitted with a pessary. A pessary is a small medical grade silicon device that is inserted into the vagina to support the prolapse. It's a nice interim option while you wait for surgery. Also the "vaginal pill" you speak of is useless. You are quite right. It was a low dose tab to begin with and the company went lower dosing knowing full well it wouldn't treat moderate atrophy. Basically it was a company move to profit more from women. I would ask your doctor for Premarin Cream or Estragyn Cream.

      @@judithcimino1772 I had surgery in 2008 to have my bladder put in a sling. I also have recurrent UTIs and interstitial cystitis. Recently my bladder feels like it's all the way back down again and I'm terrified of having to go have surgery again to have it put back up! Did you feel like your bladder was pushing way down when you were walking or even standing? Is that why you had to have 3 surgeries? I'm just wondering if I can deal with all this pressure pulling down on me the rest of my life or if I have to go have it pulled back up.

    • @moosemtnprem
      @moosemtnprem ปีที่แล้ว +1

      DaisyBlue, I did not feel like my bladder was pushed down. I had 3 sling surgeries because I had one GYN that didn't know what in the heck she was doing and that caused a cycle of needing 2 more surgeries. Basically, I view my history of repetitive surgeries as bad doctoring and I still have problems in spite of 3 sling surgeries. The reason why I continued to have UTI" S was due to the wrong antibiotic the doctors put me on. Every month or less I wound back in urgent care and they kept prescribing me Keflex. Finally, the last time I had a UTI I ask the NP at urgent care to please do not put m on Keflex. I want to try Cipro since I have taken the Keflex several times and I still have this infection. Keflex is not killing it! The NP did what I asked and put me on Cipro and it worked. I haven't had a UTI since! now I use my vaginal cream once or twice a week and I am good. Also, my primary told me that if I continue to have UTI she can put me on a low dose of antibiotic every month which takes care of problems too. But at this point, I am doing fine! Good Luck Daisy!
