Log Roll Maneuver (Horizontal Canal BPPV Treatment)

  • Dr. Danielle Tate, PT takes the time to show you how to perform a Log Roll Maneuver (aka BBQ roll, aka Lampert Roll) at home to treat Benign Paroxysmal Positional Vertigo (or displaced crystals in the inner ear that causes positional dizziness) specifically for the Horizontal Semicircular Canal. Additionally, she shows you how to determine which side (left or right) is the side with the displaced crystals.
    NOTE: It is recommended that if you have vertigo, that you first see a healthcare professional or vestibular specialist to determine the right course of treatment for you. This maneuver is primarily used to treat a specific form of positional vertigo. There are many other causes of vertigo, as well as many different types of treatment.

    Thank you so much for this. My husband has had the most common type of vertigo for about 4 years, and last summer he thought he was going through another "episode", but the eply maneuver was no longer having any effect. For the past year we've spent thousands of dollars trying to find out why this was so different. Several ENT visits, gp, even a cardiologist, thinking it could be cardiovascular... We have been crushed by this for a year without answers. We just stumbled onto this video this morning, after he fell out of bed while rolling over... I wish I could send you all the money we spent at the doctor's because for the first time in a year, he actually got out of bed normally without the horrible spinning. Thank you so much for creating this video. I don't even have the words for what happened this morning...

      Hi Yvette, i am glad that your husband is feeling better now this is music to my ears as i am going through the same thing for about 2 years now and it is affecting my life completely. If i my ask how many times have your husband done this maneuver and if he is feeling completely better now. Thanks

      @@arkane5170 I really hope that the maneuver works for you. Husband does it as needed, he's only done it a handful of times in the last month or so, if that. From my understanding, the calcium crystals in the ear canals make the fluid in the ear stop moving, or rush-moving because of a blockage, and that's what causes the unsettling sensation. Our ENT had only recommended the Eply maneuver for the LONGEST time, we didn't realize there was another possibility. But the result was instant, and if those calcium crystals can get to a corner, or to one end of the ear tube (please forgive my lack of medical vocabulary) it can stay in that position for days or weeks, so he doesn't have that movement of fluid that causes dizziness. Of course husband doesn't always move gently in life, when vertigo kicks back in, this maneuver has seemingly corrected it for longer periods of time than the Eply maneuver did. He is not entirely "better" yet. I believe he has other health concerns that haven't really been determined yet, but the sensation of spinning has been the most disruptive to his work life. (He used to just tip over and fall when he had vertigo and he couldn't drive) My husband is also hypertensive and has high cholesterol, which is another conversation. But I think it's worth mentioning because his GP has been stuck on this idea that his dizzy spells were due to blood pressure, and only blood pressure, and the doc is completely unmotivated to consider other ailments because he's so focused on his weight and BP. That has been one of the most frustrating problems - when a doctor is unmotivated to run other tests because doc keeps insisting it's diet-related. It might be, but hubby has now lost 50 lbs and still had these dizzy spells. When we finally were able to get a referral to an ENT, the Eply maneuver was the only option he offered. When we found this video, we tried this instead and BAM! ... Something else happened. It's sadly funny how years have gone by, thousands of dollars to doctors denying a problem... and then a TH-cam video seemingly has an answer.

    I have been having Vertigo for a month, did eply maneuver , it helped partially and but my head felt heavy if I nod my head or looking up. Went to ENT, and tested my ears, prescribed antibiotics and Meclizene ( motion sickness pills).) Waste of time and money. Then I kept searching other types of Vertigo exercises and came across with this video. Apparently , there are 2 to 3 possibility the detached crystals can get into vertical or horizontal canal in our inner ears. If the crystals fall to vertical canal , the Eply manuever is the one you need , but if the crystals fall to the horizontal canal, this log roll / barbecue maneuver will fix it in less than 15 minutes . Was not working the first try , and I repeated again and follow the steps on the video. Felt much better and after 1 hr , my vertigo 95% gone. Perhaps another remaining tiny calcium crystals still in the canal. I took my time to share this experience because I care about other people . Thank you Dr Danielle for your time and love.     Thank you for posting this video. I've had severe vertigo for almost 3 weeks. Non of the other maneuvers worked for me like the eply, etc. BUT yours did ... thank you very much! I'd give you a big Minnesota hug and kiss if I could. This was definitely a game changer. BTW, your model also helped me visualize what was going on in my head ... awesome to see.

    Thank you!!! We'll see if it takes but I'm pretty sure this is the maneuver that I needed! My pt did the right and left side maneuvers with me a week ago but didn't do this one!!!!

    This made me so ill, but I get it had to be done. Luckily my pt is still working through it. , what I would like to know is how did they get there in the first place.

    Thank You, for information on log roll maneuver, I had no idea this was a treatment. I have only heard of the eply, maneuver, for vertigo. This is going to help me. The more information the better. Again, Thank You so much, God Bless. Also the model you show helps to understand the positioning of canals. Other videos, I have seen doesn't show the canals very good.

    My wife had this back in Cincinnati. We moved to AZ and it's like 7 or eight years ago. A very highly trained balance therapist determined that it was her horizontal canal. She did this role (3 times in a row) and had her sleep sitting up for one night and it was gone. If the Epley doesn't work, we will do this move again.

    Thanks for the videos, very helpful and clear. I did the Log Roll on the worst side and it cleared up the problem. Can I do it on the other side or do I need to wait 24 hours?

      You only treat one side,the strongest side with vertigo

    I have been experiencing minor headache in head movements after treated from bppv horizontal canal. I don't have any idea if this is common or I have to do more maneuver to fix this. Please help me in this.

    Confused about the different names for the canals?

    How come I've seen this done a different way. I saw where for example if it's the left side you lay on the left side then roll onto your elbows with head down then onto the right side then on your back then sit up. I'm confused or is that another way to do it?

    Hello, question: Can the log roll be done on one side, then the other? Or would doing them back to back cancel the procedure out (like with the Epley Maneuver)? If so, how long should I wait before doing the log roll on the 2nd side? I have dizziness on both sides, plus looking up/down, plus general wooziness when upright as mentioned in the video. And unfortunately the earliest appointment I can get with any ENT is 6 weeks from now…. Thank you!

    If you have symptoms from rolling over in both directions, should you try the maneuver for one ear one day and then in the other direction the next day?

      I’m not a PT, but I was told to do one side for a 7-10 days, then take two days off and then I can do the other side for 7-10 days.
      I am sure you will hear varies opinions, but that is the opinion I received from my PT

      I need to know this too! I did the test, determined left ear. About an hour later went to do the roll, and then it was my right !!! Ahhhh

    hi can this type of vertigo happen while sitting. I was sitting on a bench and all of s sudden i felt dizzy like i was going to fall which lasted for a minute but was quite terrifying.

      PT here, No you can ease into the next position, it’s more important to allow ample time (30-60 seconds) in each position to help those crystals get in the right place

    Hello doc.Im suffering vertigo almost a month..everytime i lay down or sleep i feel like im in a boat floating..is that bppv? Do i need to do this treatment? Thank you

    What does it mean if you have this extreme it's not spinning but can't move my head at all without this severe motion feeling. Its like the crystals are stuck. I can't determine which canal because I have it in multiple canals. I can't find anyone that specializes in treating my difficult case because of my not so great health insurance

      determine which canal the crystal is in by the way your eyes move when lying down. if they twitch side to side you most likely have horizontal bppv. if they move up and down its posterior bppv

    Is there any benefit or harm if I do this daily after I no longer have symptoms ? My spinning sensations are gone but after 2 weeks I still feel a bit wobbly sometimes. Should I keep doing this exercise on a daily basis and if it will help ? Or it may actually cause bppv for a normal person if doing this exercise daily ?
