And leave it there when he’s exhausted. That doesn’t make any logical sense but Oda did go out of his way to clearly show the heart and clearly show the shard not in the heart. What likely happened is since Caesar was exhausted after luffy whipped his ass he didint have much strength and that’s where the squishy/slippery properties of laws power comes in to play and it slid off or slid the heart forward only grazing the heart instead of going all the way through and Caesar fell unconscious. The anime only added an overhead view of the stab but that was not done by Oda. And even if the heart was stabbed law and chopper were there and may have healed it, especially chopper since we know he is naive and even healed Caesar’s wounds. Someone had to have seen the heart because it was clearly seen next to unconscious Caesar. And Oda does things off screen and reveals them much much later like sabo being saved by dragon. A simple internet search will prove in the real world people live through heart stabbings so of course it’s possible in the fictional world of one piece and it would hardly be an ass pull compared to people surviving a city sized nuke or ussop tanking a 4 ton bat with no armament haki.
