English transcription (written in US terms):
ch = chain
st = stitch
slst = slip stitch
sc = single crochet
hdc = half-double crochet
dc = double crochet
Ch 12.
Leaf point (0:49 - 2:39):
Sc in 2nd ch from hook. Hdc in next st, ch 1. Sc in the 1st ch from hook (in the little space shown at 1:30 in the video). I suggest putting a stitch marker here for later. Sc to the original ch 12, sc in next, slst in next.
Leaf 1 (2:41 - 2:48):
Ch 4. Join to the stitch marker with a slst, ch 1. Around the ch 4, 2hdc, 3sc. Skip 1 st on original ch 12, slst in next.
Leaf 2 (2:50 - 6:22):
Ch 5. Join to the 3rd st in leaf 1 with a slst, ch 1. Around the ch 5, hdc, 3dc, hdc, 3sc. Skip 1 st on original ch 12, slst in next.
Leaf 3 (6:23 - 8:05):
Ch 6. Join to 5th st in leaf 2 with a slst, ch 2. Around ch 6, 5dc, hdc, 3sc. Skip 1 st on original ch 12, slst in next.
Leaf 4 (8:28 - 9:57):
Ch 6. Join to 6th st in leaf 3 with a slst, ch 1. Around ch 6, hdc, 5dc, hdc, 3sc. Skip 1 st on original ch 12, slst in next.
Working in the other side of the ch 12, skip 1 st, slst back up to the top of the leaf. Repeat Leaf point, as well as leaves 1 and 2.
Last leaf (15:25 - 17:15):
Ch 6. Join to the 5th st in the previous leaf, ch 2. Around ch 6, 5dc, 3hdc. Skip 1 st in original ch 12, slst in next. Finish off and hide tail. All done!

It's very interesting that the artist doesn't turn his work in order to crochet into the chain. I was able to follow the pattern by occasionally rewinding and always following his crochet movements with his hands. A lovely pattern.
