There's a reason the Lady of Pain only shows up in cutscenes - if she actually appeared in-gameplay, someone would find a way to kill her, just like they did with Elminster in BG1, and Lord British in every Ultima game ever.

nah, there's another character in torment that you can't kill, lothar. they could just make it so anytime you try to attack the lady you die, same as when you try to attack lothar.

@Terra1331 - I think she is scripted to appear if you kill too many entities that aren't already hostile to you. An ultimate "police presence" that many games struggle with when player characters or parties become too powerful.
The whole point of the game is not to die it was for nameless to figure out why he was immortal in the first place then own up to his fate. At least that's my interpretation
@jmbrosendo - No. It's probably that the movie was stretched to fit your screen res. This game was done before the time when using in-game assets for cutscenes became the standard, so they basically played a pre-fab movie clip, stretched to fit the screen.
Considering that nothing short of Ao and the Master is scared of her, I think you might of erased you from existence or something. I think she even trumps Greater Gods when they're in Sigil.

She and sigil are the true definition of neutral, and the surrounding planes in the multiverse align with the the alignments of the game. Gods normally get there status and powers from worshippers, but also are flawed as the have an alignment. The Lady of Pain has no alignment, and does not discriminate. People are scared to even look at her, and if you have her attention it is game over 100% of the time.
She doesn't ACTUALLY kill you, it's impossible to kill the nameless one, lore-wise But she might just destroy you so bad that your next incarnation is in a worse shape than when you start.

The death text literally says "You tested your immortality against the wrong creature"...You're dead. There's actually a number of ways to truly die and gameover in the game, not just here. The Lady of Pain has killed other immortal deities before too.
Lorewise, the Lady of Pain is one of the exceptions for nothing being able to kill something, Ao being another. The Lady kills you in such a way that there is no return, she has even done this to literal gods, such as the god of portals. And that is if she's feeling *merciful*. If she isn't, you get mazed.

First time you get mazed and you have possibility of escaping (Ravel was and could have escaped if I recall and I think paranoid incarnation was and escaped, game nameless one can be thrown into maze and escape). Second time if you piss her off she outright kills you and it's game over.

Lady of Pain and sigil represent neutrality, so she cannot discriminate or she wouldn't be neutral.
