  • The relatively new Vraylar falls into the family of atypical antipsychotics traditionally prescribed for schizophrenia. An advertising campaign seeks to boost its sales for mania associated with bipolar disorder. Whether this drug actually provides benefits commensurate with the $1400 monthly cash price or roughly $45 a pill remains to be determined. Studies leading to FDA approval failed to demonstrate significant difference between it and Abilify or Risperdal both of which are available as relatively inexpensive generics. Lithium offers an inexpensive option for mania and provides the lowest incidence of re-hospitalization for manic attacks. Atypical antipsychotic drugs often cause severe side effects and should be restricted to those truly in need.

      @@duke5867I was just prescribed Vraylar. Haven’t taken it long enough to tell a difference. But the price makes this cost prohibitive. After my insurance paid $1303 my copay was still $400. So, no matter the results I can not afford to take it.

      Man that sucks . Haven't even begun to watch the video and saw your comment 😅. I just wanted something to regulate the mania with bipolar cause I'm on lamotragine but usually on the depression side for bipolar it's more of a universal med with limited side effects .

    I was taking Lithium for the longest time but it made me completely emotionless and gave me a constant "blah" feeling! So my doc switched me to Vraylar. Thank goodness so far I'm loving it! I've had no side effects so far! I'm no longer cycling an I'm actually motivated and starting to feel like my old self before I started having Mania and was diagnosed with Bipolar1 & PTSD!! ☮️💖

      Yeah, lithium works great, it's cheap and effective. Though you feel really lethargic plus that weight gain. I was 3 450 mg. Tabs a day. They even mixed it with thorazine. (400). Almost tried this one. Geodon is really helping with little side effects and it's only like 160 monthly.

      I tell every person who was diagnosed with bipolar to look into autism. Almost everyone i know diagnosed as bipolar or borderline ended up being misdiagnosed and were actually just autistic people failing at maintaining their masks

      Update: just got on vraylar and i cant feel any emotiins , no joy or inspiration. 😢i want off of it. Spacey, sleepy. I thought latuda was bad but this really is not fun. Maybe caplyta next

    Works good for my mixed manic episodes and lifts my mood during depressive phases. I take it with Zyprexa. Vraylar unfortunately causes me insomnia.

    which one is best abilify or vraylar ? In sweden vraylar and abilify can be given for free to the patient. To be honest I had enough with these medicines. Just let me live with my failures and die when it's time. The worst drug was Haldol, thats my worst living experince.

    You've covered quite a few atypicals. I'd really like to see a video on Geodon. Very few side effects after months of use. Though I've read it's hard on the heart like chlorpromazine was.

    Thank you so much for your perspective. You're helping me make my decision to stick to my lithium, although I am having hypothyroidism and I recently had a slight blood calcium elevation detected in a blood test. I want to start taking better care of myself so I can stay on lithium forever. Sounds like the vraylar would kill me before the lithium ever would. I'm already 59, why would my psychiatrist prescribe this drug to me to transition off of Lithium if I can't take it when I'm elderly? I'm almost offended that he didn't acknowledge that. I've been taking Lithium since 2004.

      Ended up with anhedonia bad to where i couldnt hardly respond to conversations. No emotions. Thought id be like that forever. Slowly thawing out after not taking a pill for 3 nights. The nurse p. refused to switch me or try one of the 2 i have left despite this huge side effect. My fear of being hospitalized again left me w no choice. But im setting up new care in case she contibues to disregard this. Also anhedonia seems to be able to be mistaken for depression.

    I’d read that this was meant to have less weight gain associated with it than the other atypicals.

    Keep in mind you are not comparing this medication against another partial D2 agonist directly but how each rank against placebo. And how could you? Those studies do not occur. Everyone is an individual and some people can be statistical outliers especially in a larger clinical population. Certainly try the Ability first because it is more cost effective but it might be a second or third line option. Many other options you cite assume the absence of polypharmacy which is desirable but not always possible. Side effects you cite are very common across antipsychotics. It sadly comes all down to cost not patient well being. Very sad.

    Why it’s Special or needed could be due to allergies to Brexpiprazole or Aripiprazole. I can appreciate the long half life in some individuals ie adhd. Aripiprazole commonly causes akathesia esp at low doses. I love Rexulti can’t say anything bad about it….well cost. Thanks doc!

    I'm about to start taking vraylar and come off of seroquel. I couldn't take lithium because I had severe hand tremors. I couldn't write because of it

    Vraylar got its approval for bipolar depression in 2019. The studies show that it is better for mania than depression, but it was good enough to get FDA approved. Currently trials are ongoing for unipolar depression.
    Vraylar is unique in that it has a very high affinity for the D3 receptor which is unusual. Other atypicals work on D3 but nowhere near as strongly as Vraylar. This is the mechanism of action believed to be so effective for the negative symptoms of schizophrenia. (There have been some remarkable case reports showing the efficacy of vraylar in schizophrenia).
    So I guess I disagree that it has no place among the other atypicals. I've tried them and none really worked for me except Olanzapine, but that drug will give you metabolic syndrome. With Vraylar I have not gained an ounce, have zero EPS symptoms and zero akathisia (which I've had with several other atypicals).

      Woow ! My case is just so similar to you! I have Schizoaffective Disorder ( kinda Bipolar + Schizophrenia), the only two atypical -Quetiapine and Olanzapine which helped me in improving low mood and psychotic symptoms. But it got me huge weight gain & left my diabetes uncontrolled. So my doctor choose to switch to VRAYLAR ! Though I never had EPS with atypicals except AMISULPRIDE, so I'm worried if Vraylar gives any EPS or not ?

    I had serious problems with abilify, nausea weight gain, shaky hands, worsening depression, zombie like state of disinterest and lethargy. Vraylar gives me none of those things. And if you read the reviews on abilify, even after you stop taking it you can get severe tremors and ticks (including while youre trying to sleep which wake you up/which are also permenent even after you stop taking it) abilify for me was a thousand times worse.

      The companies always hide and obfuscate the side effects of these drugs, and a big reason new ones keep being made is that the alarming long-term effects of newer drugs simply haven’t been independently confirmed yet. More than likely, this drug will have very similar long-term side effects as other atypical antipsychotics. Most psychiatric drugs should only be used for short or moderate-length time periods before being tapered off of them. I suggest reading “Anatomy of an Epidemic” by Robert Whittaker.

      Thats what im wondering. It made me get anhodenia and i stopped 3 nights ago. No withdrawls but wondering if there will b a sudden crash soon? At least i kno the med is causing the feelings not depression. I knew that my np cont on this med for 4 weeks. If she dont switch me im seeking other care

    What kind of alternatives? I am diagnosed schizoaffective major depressive type and no matter what antipsychotic I take if I don't take my adderall I can't function normally. Vraylar seems to be the first drug I've tried that doesn't have side effects and that is normalizing my mood and thoughts. The adderall is helping my adhd and energy and focus. I've tried to stop taking the adderall but based on my thoughts and experiences none of these antipsychotics completely get rid of your symptoms they just help manage them, so if vraylar helps do that and then the adderall further helps me live how I want why is that a bad thing? I keep hearing adderall makes your psychosis worse but I haven't noticed that, it seems to be unaffected by the adderall, in fact it seems to make it better because I am able to function normally so the voices are more positive because I know I'm doing something I want to be doing that I couldn't do otherwise.

      jim hdz. It works very well for depression as well. Also while in a severe depressive when people become suicidal it also helps to prevent it in most cases. They even use it for MDD Major depressive disorder because of its good track on helping depression. In other words it’s good for depression as well. Good luck. Just get your levels checked periodically. Quite important. Good luck.

    Okay I've had brain damage since birth it's permanent it's to my frontal lobe now what can treat my mental illness they're accusing me of all sorts and it just very through the year which makes no sense to me I've been labeled bipolar schizophrenia sociopath what's goes on cuz there's actually a lot to remember here but when I read about my brain damage it causes bipolar like symptoms and mine's never going to heal I also have seizures and Fallout just pulling down out of nowhere and every Med they have give me and you mentioned a lot in this video that I've taken especially respidol that one zombified me when someone touched me I went and say my grandma had to explain that to me

    I took Vraylar for a little over a year for bipolar 1. it got me out of the worst depression I had ever been in. But I went from 120 lb to 197 lb. I went off of it in May and it is now October. I am back down to 140. Don't take this medication. There are others that work just as good. This will cause you severe self-esteem issues because it causes you to gain so much weight.

      @@doctr7147 EPS= Extrapyramidal Symptoms = Tremors,shaking, involuntary muscle movements. This is highly common in First Gen Antipsychotics like Haloperidol,Trifluoperazine which ONLY blocks the Dopamine receptors creating an abrupt increase in Acetylcholine leading to all these involuntary movements. But this issue is less common for Second Gen. Antipsychotics like Quetiapine, Olanzapine, Risperidone,Clozapine etc (but still happens in some people/ in high dose or some Second Gen like AMISULPRIDE) becasue they blocks both Dopamine & Serotonin receptors controlling the Acetylcholine increase, instead they have heavy metabolic side effects (weight gain,high blood sugar & lipids). I'm asking if VRAYLAR causes any more tremors or shaking symptoms (EPS) than Latuda ?

      Not a good idea whatsoever. Unipolar depression or bipolar depression? If its biploar depression yes. Though only for type 1 not type 2. Even then, it's mainly only for mania. People with type 1 are mainly manic though we still have depressive episodes, just not as frequent like type 2. Your doctor is completely incompetent for doing that. Best wishes and good luck.

      I noticed improvement in a couple days. I'm now 12 days into it and I'm looking forward to how much better it can help me.

    So it's a third generation anti-psychotic. Other than the first generation which were known as a "dirty drug's". Abilify and Lunesta didn't work for my type one Manic depression. Risperdal is watered down Haldol. Hopefully Vraylar will work. Do you know how much this will cost for a months supply? Lunesta was 3 grand a month when it first came out. Only worked for my extreme lows. Not the highs. Lithium worked great but too many side effects. 1,350 Mgs. Of lithium carbonate did the trick. Though gained way too much weight.

      It costs around 4 grand without insurance. With insurance it's probably 2 grand lol

    You look like you probably were a ladies man 👨 back in the day 🤔. And I’m currently in vraylar and the drug has been a disaster with my body chemistry!. Hiccups, projectile vomit 🤮, my body has been overheating 🥵 like crazy... any recommendations for an alternative to this garbage

      I recommend getting a genetics test done to test against medications & see which medication reacts best with your body & mind.

      I just started vraylar yesterday! wooo! man my 1st dose l ever took had me....like...ldk...like high actually....like ALL day! l take it with escitalopram and oxcarbazepine. 🤦🏾‍♂️🤷🏾‍♂️😎

      i had it happen to me over the wkend that shit had me shaking.....i had to go to hospital....awful.....i just asked him about something for depression.....what a disaster

    My lack of drive is from put downs. It pretty simple. A body can only take so much of that. You people should be talking to families as well.
