I think the saddest moment for Douma is him being born a psychopath
I think the saddest is that douma 's childhood was so fucked up. His parents made him a living worship item that end others' suffering because of his eyes even though he was a young child. Many adults and senior citizens would come to ask Douma who is a child to end their sufferings and lead them to paradise. Douma felt bored and had no idea why he had to help them as he doesn't know anything about his own religion but decided to continue the cycle. His father cheated on Douma 's female followers and was killed by his mom . His mom then commit suicide. Douma watched how his parents when he was a kid

The sad moment is when inosukes mother had to run away from douma after finding out he was a demon she dropped inosuke into a river and then moments later douma found her and then muzan came and he said to either eat her or she'll have to become a demon but douma refused as he had a bond with her

The fact that Douma said “she was stupid to the very end. how can dropping a baby off a cliff save the life?” And the fact that Inosuke survived and they met each other again. That’s also sad.
