      Have you tried a SI belt? see my comment , they worked for me and I didn't do any Exercises! good luck

      Do you have to use the belt all the time a d permanently. I've had pain here for years and on the side my leg is shorter that the other and just desperate for a permanent solution

      @@nnb02 I used the belt for most of the time to begin with, now only on long walks . Good luck and don't give up. I thought I had to put up with increasing pain and I didn't! I'm sure you will find a solution for you. I would suggest that, if you feel the belts are working at all, you invest in a good fitting one, I found the Podo expensive but very good, there maybe others!

      Thank you Nick. Somewhere in the early 90's I started to do whole body evaluation of all the major joints looking for blocked accessory motions. I would document and then start by treating by treating the lowest denominator. Arbitrary example: blocked transverse plane mobility of the calcaneus. Restore it and found that for many the atlas would no longer be counter rotated. learned that lower body typically has compensation in atlantoaxial but interestingly, the sacrum when rotated (or whole pelvis and soft tissue giving that impression) the counter-rotation is at T3-4 and 4th ribs. I blame the righting reflex.

      @@JerryHeschInstituteI’m 18, I’ve been having some problems with my hip for about 3 years now and it wasn’t until recently where I concluded it’s actually my sacroiliac that was hurting. Your videos has really made me understand why it’s been hurting. And when you mentioned hyper-mobility it does make a lot of sense in my case, particularly on my left side I’m especially flexible with my joints and that is the side my Sacroiliac has been hurting. I just don’t know what exactly is wrong that’s my next endeavor.

      @@anshulgupta7158 It still hurts for me, I still haven’t gone to a specialist yet because I had track season and sports but, at least what I found helped a bit was just stretching that area and keeping flexibility but it’s still a problem

    Beginning to realise only recently that my painful lower back was probably More to do with the SI ligaments rather than a muscle problem. I now appreciate much better how complex this area of the body is and that it may be possible to actually heal ligament problems. Your discussion of sometimes treating the opposite side of a problem involving the nerves struck me as something we at times vaguely sense without knowing consciously what is happening. Your professional openness to experimentation is a breath of fresh air in an atmosphere of protocol driven medicine that tends to operate on the basis that the cure is known in advance.

      Well stated everyone well nearly everyone who walks in the door have weakness in spite of lots of spinal stabilization exercises why? Because they have neurologic inhibition not “weakness”. But clinicians with short attention spans cannot invest the two minutes it takes to resolve it

    Wow, I had a injury when atending a bodyguard course. Thought it was my lower back. But slowly realised it was my SI joint. For many many years I have been able to reach farther behind with my right hand. Its my right SI joint that hurts. I just tried your stretch, and now I have no pain and noticed that I can reach just as far back now with my left hand. Thank you very much for your information.

    Thank you! Thank you! Thank you! While wearing my SI belt I would still feel shifting and pain at times. Both my sacrum and SI joint are very unstable, painful and clicks alot. I finally found relief by moving my belt lower as you suggested. Wow! What a difference it has made. I plan on doing your 5 minute exercise and pray to have complete healing as you’ve experienced.

    This was amazing i have chronic pelvic pain in addition to a left leg that is weak i had a motorcycle accident and i have done pelvic PT physical therapy and seen 11 specialists and im a medical science liason and i thought this was an amazing presentation i had never thought that stretching or using the appropriate amount of pressure to help for longer than even 1 minute would help let alone 5 minutes thank you for this excellent presentation

    Hmm I hate to be too optimistic but that 5 minute movement gave me instant relief. Not sure if I am doing it correctly but seems to have helped.

      I had pain for just about the same amount of time you have, 3 month ago I discovered there was such a thing as an SI belt [ see my comment]. For me it has solved my problems and I can now get on with my life, I don't know if it would help you but I wish I had found this option earlier. good luck

    Super helpful. Recently got a belt to use for decades-old SI pain, it was already helping but using it lower is SO much better. Vastly reduces the feeling my leg's about to fall off, haha. Hopefully it helps in the long run :)

    I have it off and on for a few days at a time, usually months apart (started earlier this year). I had the pubic joint instability with it this last time.
    I did the foam roller and some other things one night after watching another channel, and the next day and since then, no problem. Amazing.
    I added you to my favorites. It helps a lot that you have personal experience with this. Thank you for sharing!

    Your insights into inhibited muscles and how that can affect hyper mobility which would apparently be a ligament issue otherwise was profound for me.
    I tried this out on my left thumb injured 15 years ago blocking a volleyball that was being not just hit, but pushed through 2x same person within 10 minutes. This thumb had to be taped to play thereafter, and because of the increased posterior laxity it was prone to mild sprains.
    So it’s been a little better with self deep tissue massage, but not quite normal or as strong as the right.
    Just tried the technique working the right thumb in posterior deflection then pushing back 15x. Seemed better on the left- less movement posterior.
    Then tried a similar rearward deflection with the thumb midway from an outstretch to the index where it still felt excess mobility. Did this on the right 15x 2x. Checked the left and another improvement!
    Then did a kind of thumb radial rom from index pressing back to outstretched then flexing back in Right 15x2x, and the left feels so much more normal.
    So maybe the injury inhibited muscles that create stability vs what I imagined may have been a tear.
    Wow! Amazing!
    I’ll repeat this more!
    Doing MT for years, I’ve seen sacral/hip issues. I’ve got a couple people with some chronic /reoccurring SIJ locking.
    I’d be curious to know more about helping to solve these patterns to not just help temporarily but encourage longer term function.

    Thank you for sharing your story, and the very important, valuable aspects of pelvic correction. I have worked with many wonderful people since a pelvic injury 1998, but no one has known how to help me correct the chronic instability. You mentioned your own ‘hyper sympathetic’ issues after your injury. Yes, I am diagnosed with dysautonomia , and have always thought this is related to the pelvic instability.
    There is a very important book, Chiropractic : The Anatomy and Physiology of SacroOccipital Technique by Jonathan Howat, DC, DICS. They understand the interrelationships of the function of the musculoskeletal and central nervous systems, but sadly do not know how to correct the pelvis.
    What a journey. Next stop for me : Aurora, Colorado. God Bless.

      Thank you for the book recommendation. Most cases of pelvic instability can be healed with appropriate care. I start at the pubic joint because if it is shifted then so is the sij. So few people understand the pelvis. ❤️ I am wondering if those of us who can not seem to realign and heal have an off midline fusion of the filum to the dura at S2? Hansasuta, Tubbs, Oakes ( neurosurgical team at Univ. Of Alabama) published years ago - 3 out of 27 normal cadavers had off midline fusion. Wonder if this puts a torque in the whole neural tube - up to lower brain ? Wonder if off midline fusion is related to an incomplete rotation/ malattachment of the gut in early fetal development ? Hmmm.

    Thank you for taking the time and effort to share this information with us. I came across your work today after performing the same maneouver, except I was lying face down. It offered immediate relief so I thought I would investigate online and discovered your fantastic work. I performed the maneouver on the side of my dropped hip (identified by observing the height of each iliac crest whilst standing) I have had long term lumbar problems on that side. I have also noticed asymmetry in the sensation of my sit bones whilst sitting, I feel the sit bone on the hip hiked side and not the hip dropped side but this can be rectified with motion akin to the maneouver you demonstate. This all makes me think I am heading in the right direction and will try your 5 minute prescription daily for a couple of weeks. Thanks again

    I had same oblique downslip doc that you have for decades. From past 8 months am very frustrated. Tomorrow i came across your video. After all types of pt and si joint injection there is not even 1 percent improvement but after doing the stretch that helps you for 5 min for just 1 time my pelvic is back to place no more pain it's totally gone.     Wow like you sir, I had a jetski accident at age 22. Similar to a motorcycle accident, sudden stop, wrists broke, pelvis hit the front of the vehicle and I was furthur thrown 30 feet/9 meters into the water, luckily. I had several fractures to the sacrum and near the wrists, and internal bleeding. Family history of hypermobility on top of this.
    Im 29, now and have sacroilliac pain only on the right side every month or two, that persists for 4-7 days, with occasionally a sciatic shoot down the right leg.
    I don't know why I'm saying this but it's nice to share because it sucks and no one in my life can relate to this torture... Thank you for your knowledge.
    The part that I found most interesting was treating the opposite ligaments to engage the nervous system... this is amazing. Your knowledge needs to be heard by more doctors! Again, thank you.

    Thank you Jerry, I have had right chronic sacroiliac joint displacement. I did the exercise movement and felt a click and the amazing thing was my right leg had been "longer" for all the time of the displacement. Following doing the exercise, I now have can stand and my legs are of equal length...I have continued to do the exercise twice a day, for the first 5 days it needed and felt the click. It has not been displaced since, but still do the exercise once a day.

      Hi Jennifer, which exercise are you talking about? The one that he leans over a piece of furniture and holds the leg up for five minutes how were you able to do that? I cannot hold my leg up for five minutes.

    Are you aware of the phenomenon of laxed ligaments due to adrenal insufficiency/exhaustion? As a Nutritional Therapist and having suffered for over 40 years from hyper mobility and laxed ligaments, I’ve found treatment using specific Nutritional supplements along with physical therapy assists greatly in healing! Finding an exceptional Dr or Therapist with YOUR KNOWLEDGE, is the most important and sadly, the most difficult to find! Your methods should be “standard” of education in training Physical Therapists everywhere!

    I broke my collarbone over almost a year ago and I went from a very active lifestyle to very sedentary everyday for a couple of months. On the first day of the injury back pain started developing, I could feel something was off balance. It got to the point months later sharp pain ran down my leg. I've tried nearly everything and it's only helped a little to improve myself. But it's not going away, I want my old me to return. I will try the exercise, whatever it takes to fix myself back to 100%.
    I also had posture scoliosis beforehand but I was fixing it before the bone fracture. There was no back pain/hip issues whatsoever before.

      I have put in a comment suggesting an SI belt might be an answer to some. This sort of belt stops movement of the SI joint and for me at least , has solved my pain and mobility without specific exercises and within 3 months! I ski a lot and have a long list of damage I have done to myself, apparently the SI joint is often aggravated by this sort of thing. Good luck

    Cant help but see a deeper, evolutionary framework to your work. Before endoskeletons, there was hydroskeletons. I suspect hydroskeletons are still relevant in us but are now more subtle regarding their current influence on function

    I very much appreciate this information, it is not the standard sacroiliac joint information given in most the TH-cam videos. My right hip is hypermobile and for the last two years I have been experiencing piriformis syndrome, trochanteric bursitis, and pain in the upper right calve and right toes. In addition, a PT examination found my left quad was tight (pulling my left hip anteriorly) and my right hamstring was tight (pulling my right hip posteriorly). After two years of trying various exercises and stretches (which seemed to only make things worse), I tried an SI belt and IMMEDIATELY felt relief. The belt also immediately made walking and bending feel much more stable and with less pain. I plan on wearing the belt for about 2 weeks to a month to help my ligaments rebuild, while gradually addressing the muscle length/strength imbalances. With that said I have two questions....1) Can you explain exactly what movement/exercise specifically I would do on the left hip ( the less mobile hip) to help the tissues on my right hip? 2). Would the movement (which you demonstrated) where take the left leg into abduction/external rotation be appropriate for me as it looks like that movement helps lengthen the right hamstring while at the same time help lengthen the left quad? Once again...excellent job on the video!

    Dr. Hesch, very informative video. In your experience, have you ever heard of SI joint pain causing radiating/shooting pain down the right thigh? Almost every time my wife has excruciating shooting pain down her right thigh, one of her SI joints has been stuck (most commonly one side is stuck anteriorly and requires a Chicago Roll posterior adjustment which produces an audible clunk and her shooting pain doesn't occur again. She's been dealing with this pain for almost 2 years and as of yesterday received her first right SI joint steroid injection to see if this helps. She also looks to have a misaligned pubic bone with her right side being more prominent but unsure if this is due to her hip replacement. We've seen 16 doctors over the last 2 years and still no solid answer to what happened to start this issue and what is actually causing it. The pubic bone misalignment is very intriguing as that is a similarity with her but unsure if it has always been the case or happened 2 years ago through weight lifting...
    She had her total hip replacement at age 15, after normal recovery, she never had these pain patterns, was able to run, jump, bike, etc. Hip replacement has been evaluated by 3 orthos and they all agree it looks great...actually better than they expected 10 years later. My wife can go days with walking and have no pain but suddenly one of her SI joints will get stuck and cause the pain pattern. She can do the same leg workout 2-3 times and have no issues, but then on the 4th time, she has pain. It is such an interesting, frustrating case that has stumped so many physicians. Hoping you could help!!

    Very good information on SI pain,i've checked most of your videos and you show the most complete aproach to this problem i've ever seen.I've dealed with bilateral SI pain in the past, got better after a year but i'm dealing again with the same problem after 7 years with no pain. I'm being treated by a physical therapist but i'm not getting better.
    I wish i could travel to have an appointment (i live in spain) but it's not possible for me at this moment, do you know anyone in spain or near that could help me?

    I’ve have had bilateral sacroiliitis for about 20 years. Not much referred nerve pain but a lot of inhibitory motor function. Eg cannot touch my knees and cannot grab my ankle and pull into extension like you do in this video. I guess a lot of stiffness in S1 to L3 secondary to the injury is not helping and has built up over the years

Where your muscles of your posterior glutes and other muscles go into lock up/ almost tetany? The only relief I have found is extreme force to the muscles to release the tension!       @@ambert2965 yes I've recently discovered transdermal magnesium patches! They help me with anxiety and to relax, possibly minorly helping with pain. Chiro and massage helps the most, but I am trying physical therapy with a therapist who specializes in hypermobility in all joints so well see how much that helps as well!

      @@ambert2965 I also take omega 3, zinc, vit C, 1500 mg CBD, vitamin b12, chlorella, collagen, supplements daily! And noticed the keto diet as well as a gluten-free, low sugar, (mostly) dairy free, vegetarian diet helps to manage pain! I see a chiro every 3 weeks and aim to get a massage at least 1-3x a month also and PT 1-3x a week, as well as inversion therapy daily for 10-20 minutes. Cryotherapy 1-3x a month and lay in a Deprivation Salt Pod 1x a month for 90 mins. I take turmeric and ibuprofen occasionally when the pain is pretty bad and a muscle relaxer here and there when it is unmanageable. I used to take the muscle relaxers every day 3x a day and ibuprofen daily as well but I'm trying to stay away from pharmaceuticals as much as I can to get my liver a break and get to the root of the problem which is sacrum instability and hip instability along with of course the other joints instabilities but training all together is more difficult when your nerves have been compressed and are not functioning properly to activate the muscles that need to be activating!

    If you've found yourself here due to low back, hip pain, SI joint, or even partly thoracic pain (regardless of pelvic tilt, etc), you may want to look into femoroacetabular impingement/labral tear combination.. or even better, see a well-versed orthopedic doctor. I was in chronic pain for about two years and found myself looking at all sorts of possible causes, including these videos. I was later found to have a cam/pincer combination impingement on my right hip. I had surgery 2 months ago and I'm not 100% yet, but a lot better so far. Recovery takes 4-6 months, roughly. Hip impingement is something fairly common in the human population, and something that many PTs/ non-specialist doctors may not consider. It also is associated with arthritis. Good luck!

    I'd like to thank you for this video, and I am going to try your move. I have been doing research over 4 months, and I am pretty sure I have SI-joint instability on left side, fell on my butt while skateboarding ramp. Pains are insane and lasting over 1 year now. I have tried everything.. My plan is: I will continue to wear SI-joint belt most of the day when I feel some stability in my SI-joints, and doing this bending forward move and continue to walk atleast 2-3hours per week. I tried bending forward move once and I was already shaking in 1 minute, so I have to work for 5 mins. The thing is if you are scared to bend forward, you will never have courage to strengthen your SI-joint.

      @@JerryHeschInstitute yes I was guided by doctors who treated the pain only and physiotherapist, but their plan didn’t fix my problem, so I gave a try with your suggestion. I can tell, After one week your move feels like my left side has become more stronger than ever. I am already going for 5 mins. I am standing with only one leg my hurting one and bending slighty forward while holding another leg, and I am feeling improvement immediately example walking and running. it just feel stronger, and more confident. I am not scared to bend forward anymore example pick my groceries or tie my shoes.
      I also use stretch bands, and I like to do glute bridges, claw shells and some core-strengthening exercises. I am pretty sure I am slowly improving my situation better.

    Thank you, very thought provoking! I am searching for answers to my ischial tuberosity pain. Don't know what is causing it but (sacrotuberous ligament tightness, bursitis, high hamstring tendonopathy, or muscle imbalance/weakness). My left side of my body is weaker overall than the right side and the L hamstring is extremely tight at the proximal end due to this pain. How would I do this?

    Maybe nobody will see this but I don't know how to stop muscle guarding around my SI joint. It aches when I sit on one side and when I sleep on my left side, but most of the discomfort and pain I feel is because the muscles get very tight when I'm standing leading me to have to do these painful stretches. It resolves spontaneously after 4 years and now it's back on the other side

      Yes, I am familiar I very rarely recommend sit fusion, but would have to do hands-on testing to discern.

    I have a plate with 9 screws holding my ishium together. It was broken so bad that the plate cannot be removed. My femur was completely jammed up into my pelvis and sacrum. After 3 years I got a new left hip, there was absolutely no fluid (bone on bone) It is 10 years later and my right hip is starting to act like my left. My SI joints are a mess!! If you touch that area in my back I jump..Did I say it was fractured too along with L 3,4,5 S1. I tell the therapists the exercises they are going to have me do. I do quite a few at home on my own. I believe I am looking at 2 surgeries. The state took my pain meds because I take anxiety meds for PTSD. Can you say suffer?

      that is terrible re pain meds. Sorry to hear all you have been through. You are wise to take charge w the therapists.

    Hello Jerry, It was in 2014, My bike passed through a deep big pit on road during one night. It really gave a shock to body (especially left side) and i had bad stomach and back pain due to this. Since then i have continous pain in left side of back and leg. Also uncomfortable kind of stretching pain in lower left side in front i walk or streach my body.
    I cannot put pressure on my left leg. While i pull of push something from my left hand i feel something got disturbed inside and it gives pain in front and lower back. While brisk walking, i feel same pain... Like something inside pulled or got disturbed at the left side of back / in front. Also while walking passes over uneven surface, it gives pain in lower back and in front side as it feels. My MRI shows disc buldge in L4-L5-SI. Over the period of time my pain has reduced somewhat. In winters left leg becomes stiff when you compare it to summers. I am 37 plus. Can you plz reply what is happening? Is it related to SI joint?

    What is best to diagnose SI joint dis function? MRI OR XRAYS? I’ve been having this pain lower back left side soon after a hysterectomy in 2018 . I start PT soon after doing three rounds of cortisol injections that did not help much. They only xrayed my spine not hip area.

      Hands-on evaluation by Hesch trained physical therapist should be part of the equation. MRI or x-ray may not show source of mechanical pain best to pose that question to your physician. depends on your overall presentation, oftentimes shows nothing.

    Hi do you know any self treatment for a movement pattern of the ilium where it goes out perpendiculair to your back, making like a bump on my back around the si joint that hurts. It seems to be the only pattern left and i just can't get rid of it. Thank you in advance

    Jerry, do you have any thoughts on prolotherapy/prp as treatment for long standing (15 years, half my life) SI joint issues?

      I believe in using thorough biomechanical models as hesch method first. I have people w hyper in one direction but hypo in the opposite. The secret is to treat the hypo. Both improve

    If you have SI ligamentopathy and tendinopathy in the groin, glutes, hip, and even hamstring and thigh on one side, would you still recommend your findings?
    What about shockwave or cold laser therapy to target the SI, the disc, and the various tendinopathy points while going to PT for SI dysfunction?
    Do you have anyone in New York, Connecticut, New Jersey, or anywhere in the northeast area you would recommend?

    Great info for SI joint pain… how often should one do the 5min hold for? For how many days? And :-) do you do both sides or just the painful side?

      Depends on hands on findings. Typically 1x day for a week once weekly thereafter.

    Do you have thoughts on whether Prolotherapy and PRP are also good treatments for ligament healing? Or perhaps when might prolo or PRP be indicated as an added treatment?

      PRP will not increase tensile strength, nor will it bring together damaged tissue for repair. I see value in PRP for some (minimal) uses, but unfortunately not for much of what it's being used for.

    How can we find someone in our own area that knows your technique or your knowledge. I have been searching and searching and cannot find anyone. There is no way I could fly to Colorado to be seen and I don't have that kind of money. I am suffering so much. The pain has been getting worse that I can barely walk or function. I live in Pennsylvania and have been seen by many doctors and physical therapists that don't have a clue about this problem. Is there any hope for me? I would appreciate any response.
