    If you are considering all on 4 treatments, you may have come across 3 on 6 as an alternative treatment to replace a full set of teeth with dental implants.
    Both all on 4 and 3 on 6 promise to replace a full set of teeth and neither of these treatments are new, although, with good marketing, it can make it seem that one is better than the other.
    In this video, I will explain the differences, and when one is better than the other, if at all.
    In general, all on 4 is a single, long arch bridge that connects all of your teeth, and the 3 on 6 is three shorter bridges that fit together to form the same full arch smile.

I wish we had similar docs in US, I wish I could also sleep good at night, and want to kill my self cuz of constant teeth issues after giving birth, and trying to fix them bankrupt me, I swear after all the pain and struggle it would be easier to die, if I didn’t have a child to raise I don’t think I would be greeting up in the mornings.

      @@drgurssehmi Thank you for the excellent video. May you never have insomnia. I had 50 years of good sleep. Been a battle for 12 years now. Wearing my first set of removable dentures (day 5). Never even heard of 3 on 6 before. Will be scheduling a couple of consult appointments in Las Vegas, NV tomorrrow for a better solution. I really have less troubles with the lower than the upper, which is not the usual case from what I understand. I have a hyper gag reflex, so there is just too much acrylic for me with the upper. While my dentures are esthetically fine, they don't function well for chewing. Tried to eat a salad, and even the lettuce gave me trouble.

Life sucks for me since my health declined and my teeth started acting crazy decay even though I brush and floss 3 times a day.😤 I even developed severe anxiety when it comes time to go to dentist cause I become paranoid they going to say I have to have yet another extraction. They have to put me to sleep give me nerve medicine 2 to 3 hours before every appointment. What's really funny is I worked as a dental assistant for 6 years!🤓😩🤪

      @@deenadee4946 sorry to hear that, but hang in there, u know even if they do extract more, n I went thru similar thing of what the content creator went thru, it def was tough, and ppl looking at you the way like you are some sort of a drug addict amd poor really can cause that anxiety and even depression, even tho they don’t realize that sometimes it’s genetics mixed with immune disorder or some other health issue, plus it’s not cheap, I ve spent over 20k in 4-5 years and it didn’t get me anywhere but to spent even more to extract everything( which in my state is $220-$275 per tooth), and than everything else is even more money, but you just keep going, the sooner u take care of it, the better you will feel, the emotional pain, anxiety and all this horror goes away once you even have a temporary solution, trust me, it’s a huge relief, I’m still waiting to do smth permanent, but I feel and look so much better, all my sadness and embarrassment is gone, and the pain, you forget about it, honestly, you just can’t fix the issue without the pain, so just keep going, and trust me all your anxiety will be gone as soon as u get smth that looks ok, even if it’s temporary! Wish you all the best! It will get better! Just don’t give up, and don’t let it others judgment get to you too much, cuz at the end of the day- it is hard, painful, expensive, but it’s fixable, and you will get there, the most important thing is that it’s usually not life threatening, that’s a bright side, try to think positive ( I know it’s hard) but just let yourself think about the future when u will get your teeth fixed and how you will have the best life ahead of you still! 😎💪💪🙏

      So true, We used HebeDoc in Mexico for my dad and did the all-on-4 for $12,000 instead of $30,000. We financed 80% of it through a medical credit card they helped us with and dad is only paying $300 per month…

      Stay away from doctors in the US, the middle east and Europe. Those are all going to be copying the Juice and stealing your money. The best place to get your teeth fixed is in Mexico, where real doctors work. Really!

      He sells all in 4 all the time.Ask about the way food gets stuck under the all on 4 and the fact that it can’t be removed and that a normal dentist can’t help you.

    I wish there were more genuinely concerned doctors like this. I just had all my teeth pull at Affordable Dentures and I am now convinced I could have saved 80percent of them. So depressed.

      Hey. They do it to thousands of people I believe....they did it to me. They lied to my face and then kept telling me it will be ok and your fine ...your gonna be a billboard for us and I'm giving you a deal....I want you to look good......nah they make bonuses off you at the expense of your well being .....it's costed me 12k lost my detail business lost 20 pounds the first 2 weeks and slipped back into depression and anxiety.....after a year I'm still struggling to forgive myself ......cause I knew better but I gave the benefit of the doubt and it costed me dearly .....my life was looking up and I was a recovering alcoholic.......now I slipped back. and been drunk for a year. There's a video of a girl that had her brother die from drugs so she got I to the same thing and after recovering went to get here teeth fixed at a place liked DDS or affordable dentures same place I did and it fucking ruined her. If we could all find each other on the web we could class action law suit the fucks.... guarnetee it. Affordable and DDS are the same company.

      @@thebrightsideofthemoon5829 No I mean I lost my job was unable to work after being back in the bottom of a bottle......no implants ...nothing but bulky plastic and a bone that has shrunk % 40. I learn now that the bone will continue to diminish since there's no teeth to stimulate the bone. Basically your lips and face will cave in soon as you remove your denture's. That may take many years but I'm not going to find out. I got my cdl and going over the road away from my kids and family to pay for this shit. Bone grafts and implants and fixed bridges. Not doing any dentures. Dr Jose moguel in Los algadones has an all on 8 procedure and many other options and just under 20k some a lille higher. Still beats 30 to 60 k in the states and this guy has been doing it for a very long times, supposed to be the best.

    Thank you for this! Super helpful. After dealing with a lifelong history of bad teeth, non stop tooth pain, cavities filled, root canals, bonding done for broken teeth, extractions, etc. I finally took the plunge a couple of years ago and pulled all my back molars on the top and bottom. I have two partials. The top one I paid $500 for which is better quality then the bottoms that keep breaking on me. One of my canines just broke and I’m on day 3 of excruciating pain. U.S. healthcare is a joke and covers bare minimum with dentistry, as they consider it to be cosmetic. Apparently whoever made that decision has never had tooth pain or the inability to eat certain foods. I’ve been looking into travel dentistry and realize if I want to get the work done, that I will no doubt need to have it done in another country. Hopefully I can afford it come the future!

    I wish Dr Sehmi was my dentist, I subscribed because his kindness, honesty and genuiness (not sure this is a real word😂), pours out from the screen. Plus it’s also nice to hear a fellow British person as a lot of Dentists on You Tube are overseas.
    He sounds like a great man and his parents must be so proud ❤️
    Unfortunately I think it’s too late for me and my teeth to have a nice smile again 😢😢 I’m grateful for the masks 😷 we have to wear now 😢

    Thank you so much Dr. for your honest presentation of the option for the denture implants . I am 63, I only have 4 teeth and I am so very miserable and have run into walls in trying to find the right treatment for me. I have not gotten anyway close to finding some that I feel I can have enough comfort was 40,000 to 50,000 and the last one was cheapest at 36,000.....I feel like crying, that is way too expensive for me I could maybe do 20,000 to 25,000 but so far there is nothing in that range. I feel so discouraged. At the first of the year , I was told that I needed to have all my teeth removed except for four and then they would make me a partial which was a lot better option than dentures. The dang partial did not work good for me at all, went to see if they could adjust it and they tried to adjust and see if it was a good fit after three hours they tossed it in my lap and told me to do the best I could with it. I cannot believe what they did not do and how I was left out with no teeth. More to this saga but this is enough just to see all of my frustrations. I am so depressed and I have tried different out of town and state to no avail. I appreciate your time I saw an opportunity to vent because of the way you seem to care a lot about patients and their circumstances. I wish you were here in the US,lol. I wish you well.

    Most Excellent description of the various implants and versions of each. Excellent explanation between 3on6 being a name brand manufacturer and have a set regiment to their implants where also an individual dentist can do the same thing and have more freedom to fit it to a particular mouth depending on gum depth etc. A lot of variables to consider and the specifically 3 on 6 does not allow for full freedom on Dentists. The all on 4 is a very good presentation of what that is. I like a Dentist to consider how minimal the gum brace needs to be in my mouth so they are also be secure and give the right Aesthetics when laughing. Great Job Doctor!!

      @everyonetookmyid just caught your comment. I'm from the US also northern east coast area. Have you been able to find anyone to do this procedure for you? I cannot find anyone who will do extractions and procedure in house. Seems I need to see Oral surgeon then Peridontist which with doing 4 on four snap on's will cost more than a house. Full mouth extractions - bone grafting, etc is $26,000 plus extra for sinus liftand CAT scan; Periodontist - just temporary set $21,000, final set won't give me cost.
      Hope all goes well, wish I lived in India as prices here are outrageous. Plus I hear that after care here in the US is extremely costly.

    Most detailed explanation of implants I have herd so far. I am 65, and have about 7 teeth on my jaw and none left on my upper. I had a childhood disease for which the doctors gave me a medicine which militated against a normal smile ever. What price range could you imagine for my implants of either style?

    Younger sister had the 3-on-6 she got it done here in US. Within 3 months she was regretting. After a year she had them removed and got traditional All on 4.

      @@CC-hv9ei the actual teeth on the 3 on 6 weren’t as strong & chipped & implants never really healed 100% but with All on 4 the teeth were stronger bc it had the additional layer of acrylic gums and not just the teeth with metal rods.

    WOW!! Thank you for such an informative video. I wish more dentists had the desire to educate. I have had an initial consult with dentist and he is recommending 3 on 6 (no gum!) procedure. I have osteoporosis and have sever bone loss so is it reasonable to proceed with dental implants? Is it reasonable for me to expect successful outcome, I have a tremendous fear that the implants will not be stable and fall out!

    I like how you explain yourself concise & precise. I'd go to ya if ya were in the USA. You're straight up. I really like that 🙏🇺🇸 what's wierd is today I went to a dental surgeon that does the whole enchilada. I was asked what I wanted so I said definitely not dentures but idk what. He's staff explained it all for the all in one. Think I'm going that route.

    The only real thing he said is that No Replacement options will ever feel like your real teeth, this is true but some will be closer. Keep in mind that this guy promotes all in 4 non stop. His objections to the 3 on 6 are not serious enough to have your gums cut off and a big chunk of plastic screwed into your gums in their place. To give you the ‘pink triangle they cut over an inch of each of your gums upper and lower. The biggest complaint about the all in 4 is the food getting stuck between the new gums and the fact that they are very difficult to clean. The other issue is the HUGE size of the gum sections, the lack of space between that and your teeth and the way they effect your speech. The other issue is the diagonal implants at the back, if you research prosthodontists they will tell you how the jaws do not like these diagonal implants.
    Do your research and make sure you speak to a prosthodontist who are the specialists in implants not dentists!
    Also take into account that in the US alone there have been over 3million complaints with the health authorities about botched results with implants alone, the vast majority being all on 4

    i just love your demeanor you have such a positive vibe. i have been missing most of my teeth for over a decade and my youngest two children don't even know what I look like with teeth. I just turned 40 and keep a pic on the fridge of my teeth before i lost them. I use my mouth as a cautionary tale for them to take care of of their teeth. Though I lost mine with each pregnancy. I so appreciate these videos with valuable info as I watch all day everyday knowing one day I will possibly be able to see a dentist. You seem like a lovely caring person. Thank you.

    Supposedly the 3-on-6 also has a better/stronger biting force than the all-on-4. I was hoping that would be discussed some. Would that be a good trade-off for the gum issue?

QUESTION ONE: At each of the seams between the three different bridges in an all on 6, I see two implants in close proximity? Is their close proximity an issue given that they are carrying half the load of 6 teeth? QUESTION TWO. I had a bridge (3 unit upper )for 24 years and never had it removed so why would I want to remove a 3 on 6 bridge, especially given that I can track bone loss and condition using serial cone beam and I do a semi annual blood test that sniffs out infection via CBC and an immune panel? QUESTION THREE. Are any of these systems able to identify EVERY material used in their manufacture so that list can be compared to a biocomp or ELISA or Clifford PANEL? QUESTION FOUR. review your video beginning at 6:13 mark and catch yourself doing what you say the 3 on 6 chart is doing: you are skewing your dialog to support All on four while ADMITTING you have never done a 3 on 6 or even seen one! IN HONESTY: your approach to the learning patient in a growth mindset turned from genuine to disingenuous right about here. I ENCOURAGE you to clinically observe a 3 on 6 process or at least a patient before making the conclusions that you have. At the same time I thank you heartily for the clarity this video provided me in helping me to formulate more questions for practitioners. It looks like due to costs I will soon be a dental tourist.

    Excellent and informative. The pursuit of the 'perfect' smile is resulting in patients not listening to dentists like this and having functioning teeth unnecessarily removed which is baffling for those of us who have lost natural teeth though accident or genetics.

    oh my goodness I wish you were closer to me. I am going through so much with my dental experiences, one thing after another. I had read about the three on six prior to seeing your TH-cam video. I was experiencing some dental issues, my two front teeth started to chip off and turn black. When I went to the dentist i was told the best option was to have all the teeth extracted and get dentures, I was limited because of financial situation, because I had to quit work due to some health issues. The temporary dentures never fit and the dentist I went to tried to due some sort of reline, when new impressions should have been done since the didn't fit from day one the top created and overbite and the bottoms never set properly and would always pop out when I was talking, and they were yellow.. This was from a very updated with all the fancy machines. watching your video gave me hope that I will find a dentist that can do nice dentures, I wanted the three on six but not sure I can afford it. Thank you for giving me hope, it just made my night. If I am lucky I will be able to find a dentist as special as you.

    I suggest going to a prosthodontics. I had my regular dentist creat my crown and bridges for a very uncomfortable result. Five years later I saw a prosthodontics doctor and had it all redone. I has to take out a $12,000 loan but it was worth every penny to feel comfortable. I put comfort over appearance every time. I finally got both.

    I believe the implant supported overdenture is the best option. AO4 or 3O6 have issues. It's difficult to keep clean..

      Hi Chris. We are starting to open things in the next week, but we will only be able to see the most urgent emergencies first, and probably only about 5 patients a day. We need to wait a full hour between patients according to the new guidelines. It's a bit of a nightmare, but the guidance is changing regularly, so hopefully things should be near normal in a month or so... But don't quite me on that! I think you have my number right?     Great video . I've talked to you before about angling implants. I'm a firm believer keeping implants as vertical as possible. I used to fabricate orthodontic appliances. Number one rule,put pressure on a tooth ,it doesn't take long for the root to become mobile and the tooth will move. You replied once that angling an implant could contribute to implant failure. Why don't dentists automatically do sinus lifts in order to place implants more vertically to lessen the possibility of failure Thanks Larry ,Stay Safe

      Hi Larry. By splinting the implants as in the ao4 technique, you minimise lateral forces on an implant. It's the lateral forces that cause problems. Sinus lifts add a significant cost and surgical risk and time to the procedure. Often you cannot immediately load with sinus lift treatment. There is no evidence to show that angled implants like this are any more likely to fail, however single angled implants are a bad idea.
      Also, it is almost impossible to have a full set of parallel implants, the maxilla is slightly conical, so even if it looks parallel on an X ray, it does not take into account bucco lingual angulation.

With the over dentures we do if the implants and abutment are not as vertical as possible snapping on the denture gets more challenging but the pressure is distributed over the ridge and implants reducing the pressure on the implants.

    My gums do not show when I smile. I had the three on six done and I absolutely love them. The best part if something breaks you only have to replace that bridge and not the whole thing and also you can still have them taken out for cleaning if needed. All on four means your jaw is actually shaved down taking away good bone to make it work. My new teeth really do feel natural to me and I’m so glad I had this procedure done. One last thing to mention since you brought up prices, it was about $10,000 cheaper compared to all on four.

      @@1RungAtATime Smile Clinic in Utah. They have a few other clinics in other states but I’m not sure where. I do know that people fly from all over the states to come to the one in Utah which happens to be the one where the inventor of the 3 on 6 works. He will also do all on Four for far cheaper than Clear Choice.

      @@lk-cd9fe it was expensive I’m not gonna lie but every candidate will have a different price depending on how many teeth are removed. I was quoted by clear choice at $54,000 and I kept looking around until I found smile clinic and had everything done for $43,000. Yes it is the price of a new car so I’ve kept my beater car going and never looked back. It’s been over a year now and the teeth are paid off and I have no regrets. It’s been amazing to have nice teeth and to eat whatever I want with no worries. It’s an investment in yourself is how I looked at it.

      @@vegetaking1272 the all on 4 has a gap running between your gum and the denture which gets food trapped inside. You have to pay about 3 times a year to have the all on 4s removed and have a deep clean which I guess won’t be cheap. But the all on 6 or 8 are permanently fixed more like your real teeth.

    Dr. Sehmi-my husband had dementia and need full mouth restoration. I think all on four is too hard to keep clean but the three on six is easier. Or are snap on the best and how to get them with the palate cut out? I am unclear on overdenture vs. denture... I am trying hard to find zirchonia implants as well due to biofilm formation and their lowered Immune system. Is there a way to start with snap one, implant placement and go to the best one if he can't manage? Grateful. 🙏

    Just a little detail to add about why not doing sinus lift before to avoid tilted implant on all-on-4, because in the all-on-4 procotol, you can not use implants on the grafted site. So you do implants tilted to enlarge your prosthesis structure. Thanks for this video and explanation.

    So it seems like the all-on-4 is a tad better, but what got me looking into the 3on6 was price. They claimed the 3on6 was nowhere near the cost of the other, wonder how much of a difference they are talking about? If it's a huge difference in price, I would think maybe I could live with the non- perfect gum line issue...thanks in advance for any reply even though I don't expect one as this vid is over a year old...

      @@debrasingz9555 After 4 months, are you satisfied? My wife is going through it now. She is about a month after the initial surgery. Temporaries don't fit yet, but she still has stitches. We both agree the 3 on six seem to be the best primarily because they don't remove all the bone that AO4 or AO6 do.

      It's really expensive to get your teeth done in the UK or US. We went with HebeDoc for my dad and did the all-on-4 for $12,000 instead of $30,000. We financed 80% of it through a medical credit card they helped us with and dad is only paying $300 per month…

      Yes, this should not be a problem... The only down side is that if you wait too long, you might lose a lot of bone, making it harder to have implants (when I say long, I mean 5+ years)

      @@drgurssehmi my dentist said bone loss is immediate and bone grafting would have to be done if implants are not done when teeth are pulled. You are saying I could get dentures and the all on 4 or 3 on 6 could be done if i do not wait to long that would really help me get my finances in order for the implants.

    Great video and very helpful. I just want to say that no false teeth will ever feel like real teeth. Not just because of the ligaments, but also because of the nerves. There is a certain satisfaction to eating, biting, (even if it's just gripping a piece of thread while sewing), and other sensations associated with natural teeth that you will never get from false teeth, no matter if they are dentures or implants, simply because there are nerves in teeth that allow you to feel those sensations that are not in false teeth. People need to be aware of that so they don't expect more than fake teeth can deliver.
    Also, another thing you mention is measuring the patients teeth. Don't you take impressions of a patients original teeth (if they still have some of them) as a guide? I have a different situation. I've always had perfect teeth. They still are perfectly shaped, but in my 60s I was starting to get periodontal disease and developing pockets. I had a new procedure done, laser gum surgery, which exposed my roots. My dentist bonded the whole front line of my teeth to hide that and prevent root issues, but didn't do the back of the teeth. Well now I have decay on the root of one front tooth that got into the back exposed root, and as a result will have to get that one front tooth replaced. But the surgeon that I went too took an impression of my teeth so that I will have an exact copy of my front tooth to replace that. Isn't that the normal procedure instead of just measuring and making a guess?

      @@inasyal6311 Been there. I once had a back tooth that hurt so bad that the dentist could have pulled all my teeth just so I could get rid of the pain of that one tooth. Fortunately, a root canal solved the issue.

    Dr Gurs Shmi, Do you recommend getting your teeth straightened before doing bridges? I need to have my 6 Middle teeth on top (two middle teeth are crooked) and bottom 6 replaced. (I had an accident) One dentist I saw wants to replace all six on top and bottom and the other one wants to do a complete mouth fix. I do grind my teeth badly and started about 4 years ago wearing a guard at night. suggestions please.
