    (1) Identify the individual: name, age (or date of birth) and/or relationship to the patient
    (2) Find out where they are (geographically) right now: address
    (3) Find out whether it's a good time to talk (whether they have a few minutes)
    (4) Ask for their telephone number in case the line gets cut off

    If the patient is driving then do we ask to talk to them later or should we request them to stop on the side of the road and talk?

    Are we suppose to pretend that the patient is with us? Or be real that it's a phone call?
    Like for example, for examination should I say " I would like to examine your rash?" or should I say "ideally I would have examine you but now can u send me a picture/email of your rash?"...
