I think Stan Edgar set something up with Payback fucking up Grace's operation in Nicaragua. Whether he knew Payback would fuck-up so royally because of sheer incompetence, or to some extent (it could've just been one or two members of Payback that knew the real plan) intentionally. SB aiming a rocket-launcher towards a munitions stockpile (whether intentional or not; like this guy's been in multiple wars, I feel like it should be common sense not to aim anywhere near the general direction where they keep explosives), Swatto flying up in the air drawing enemy attention, CC killing Grace's own men (also whether intentional or not; she doesn't even apologize or look sorry), CC's very sketchy confirmation of SB getting killed (she says she saw him get killed, when asked by what, she makes it sound like she wasn't sure, she says "a gun or a weapon"; like no shit it was a weapon, could you like narrow it down? A 9mm pistol is a gun, so is a .50-caliber armor-piercing anti-material rifle. A sword is a weapon, so is a bomb. Also, nobody else saw SB get clapped? Just her? She can turn men into piles of hamburger meat and she just like stood there and did nothing?), and the sheer convenience that the enemies just have a weapon that could maybe kill supes (like they just have a supe-killing gun lying around in their standard arsenal and decide to break it out now of all times?
