  • Getting working length radiographs during an endodontic procedure can be one of the most frustrating procedures on earth (well, that's what I think). It's practice and also tips. I review some of the tips I've been shown over the years from my mentors that have helped me get working length radiographs almost everytime.

    Generally mandibular teeth is easy to take, what frustrating me every time is upper 6 and 7, I've tried all the techniques and i still find it hard to catch the apexes pf the roots 😩

    How would you make sure that you get the pt to bite on the PA holder without them biting the endo files deeper into the canal as well? If you bend the files out of the way how do you make sure that you dont inadvertently slightly pull them out as well

    So would you say that you would normally do a distal shift for mesial roots and mesial shift for distal roots? Or it doesn't really matter? Thanks.

      Such a great question. In my experience, to see the mesial root of maxillary molars (ie - to help see MB2 - which can be difficult even in a radiograph), a distal shot is helpful to "throw" the mesial root forward. In mandibular molars, I have found that it's not as important to be honest. I should make a video to explain!

    Taking a good PA with that clamp and gutta percha can be a real pain in the tooth sometimes.. 😤

    Could you share the details on the paralleling device so that I could buy it online. I’m using an RVG sensor. So I need it in my clinic when I take x rays with file in place.

    Do you do rootcanal treatments with or without microscope or loupes? I am having a hard time doing rootcanal treatments without them. What filling technique do you use?

      This is such a great question. A microscope is honestly, in my opinion, the gold standard for root canals. However, you could argue that a CBCT of a tooth and high powered loupes could totally be an alternative.
      But - I hear exactly what you're saying. What if I don't have either and $$ is an issue.
      I have worked on operations overseas with my high powered loupes (3.5x) with a super bright LED light and that, I believe, served me well for that situation. I super prefer a microscope, but, that's not a possibility for everyone. I get it. i purchased my microscope used from a retiring dentist. They are not cheap. (I'm not sure why not though).
      Accessing a tooth can be the most scary thing to do - especially if you're new at the game (I'm still nervous at times with a calcified tooth) - or - you don't have magnification.
      My super humble advice - without magnification, I would try to do teeth that don't induce a lot of stress - max incisors, canines, premolars - that aren't calcified - and - are vital. Vital is critical b/c their success rates are higher and you want to start to build confidence. There is nothing more damaging to your endo confidence than getting kicked in the teeth by a non successful root canal or perforation.

    Mandibular x rays are always easier you don't even need a holder.. could you please make a video with some tricks to take maxillary PAs without xcp?

      diamelys morales peña great idea. I can do that. I use phosphor plates in another practice (essentially the same as conventional xrays) and will show that.

    At my office they make us free hand it is there a way to take a X ray with the files in without having to freehand it

    How a dentist could take x- ray for a tooth with root canal and file in place (within the canal ) and in the same time the patient have to bite using these things
