Giant Gear 5 is kinda a form too. He absorbs enough air for all parts of his body (bones, muscles, organs, etc) and becomes gigantic.
I think boundman was first use when luffy training with rayleigh, but introduced when luffy fight doflamingo
So the first use in the story line is against Do. Even if he did transform before for the training ^^
