The Tophat ghosts look and act very different than him, implying they were created by Sunny’s imagination just to be ghosts and weren’t the afterlifes of pre-existing headspace characters. But this is the only headspace character to my knowledge to actually die and become a ghost, thus being the only “actual ghost”.

The name “Actual Ghost” implies that the other ghosts seen in Headspace aren’t real ghosts
