When Mr. Cochrane first presented on 12/08/18, he reported a history of shortness of breath (Dyspnea), chest tightness, a nocturnal cough, and orthopnea. On examination, signs of congestion including raised jugular venous pressure, bilateral ankle edema and bi-basilar crepitation were observed. Additionally, he had shifted apical impulse. On investigation, a chest x-ray revealed signs of infection and had cardiomegaly on electrocardiogram. Considering the above evidence, a provisional diagnosis of left ventricular failure precipitated by pneumonia was made. Mr. Cochrane was commenced on broad spectrum antibiotics, frusemide and digoxin. He was advised on smoking cessation and reduce alcohol consumption and a follow up visit was scheduled after 2 weeks.
53:19 “The patient was prescribed a seven-day course of broad-spectrum antibiotics, frusemide 40 mg per day, and digoxin 0.25 mg per day. He was also advised to stop smoking and reduce alcohol intake. A follow-up appointment was arranged for 14 days later.”

On examination, patient showed dyspnoeic associated with high jugular venous pressure and noticeable bilateral ankle oedema. Upon auscultation, it shows crepitations in the lung base along with apex beat lateral to the mid-clavicular line and in the 6th ICS. On investigations, ECG revealed cardiomegaly, and chest xray detected signs of infection. Moreover, cardiovascular and abdominal were indicatively normal. Overall, the diagnosis is suggestive of left ventricular heart failure.
