It's illegal to pose as a medical professional in the same way as it's illegal to pose as a police officer etc. As this was not fiction and he's actually performing a medical examination and claiming to be a real doctor, that means legally he has to be or he could go to prison and the production would get in deep trouble.
When you mentioned Florida gators as pets, I wasn't thinking professional trainers with enclosures and proper equipment more than the local crackhead with one tied to the end of a string like a dog outside the 7/11 or Wawa as I saw in Tampa when I went there.
