Can u explain why 100%on the rise time?

To get the most tidal volume, you should keep the most pressure in the lungs for the entire I-time. Once you set up the ventilator, the rise time is then adjusted to to prevent over-pressurization or the little "bump" on the pressure scalar. There may be different schools of thought on this aspect, however I tend to set optional settings to basically off and then adjust as needed by the patient.

Whats the difference with a Bilevel mode?? Thank you

The BiLevel mode is the APRV mode. This mode has the patient spend more time on inhalation and less time at exhalation. An example would be 4.5 seconds for a Ti and 0.5 seconds at Te. This would cause the Mean Airway Pressure to increases (improves oxygenation) and assist with using collateral alveolar conduits to reinflate the alveoli. You know the Pores of Kohn, Canals of Lambert, and Pathways of Martin. The CO2 is controlled by the releases and the difference between the pressures.

NSU BSRT I think bilevel mode is a pressure mode but inverse APRV, that’s why I think it is so so similar

@@alvaroocon3173 You are correct that it uses 2 pressures. As a clarification are you stating BiLevel as in BiPAP or BiLevel as in APRV. The Servo i calls APRV - BiLevel and the PR840 calls APRV - VC+ and other ventilators call it different things.

NSU BSRT I mean Bipap, longer time with low pressure, opposite aprv. I cannot find a big difference with SIMV-PC. Any help? Thanks! Good job!

@@alvaroocon3173 I think I am following your question. The main difference is the BiPAP is a spontaneous mode. The rate that is set is the back up rate (if the patient drops below that rate). Other than that the mode is spontaneous. Respironics (the maker of the V60) stated that the maximum "suggested" set rate is 8. Do not use the rate to try to blow off CO2 but use a higher IPAP.
SIMV has a set rate and then the PS is worked in. The BiPAP is more akin to CPAP with PS than SIMV with PS. The main advantage is the cost of the machine. If someone only needs NIV, then why use an expensive machine when a much less expensive machine (V60) will work just fine.
      Does that help?
