Staff did not wash scope well and its channel with tap water 5 Staff did not change into clean gloves before testing for Acecide MEC and touching AER 6. Staff does not properly position scope in the AER 7. Staff put only gloves when removing scope 8. Staff hang scope in the cabinet with bare hands I am surprised that this facility passed the JCAHO inspection.

FYI, if you see the receipt printed it reads WARNING incomplete process that means something went wrong during the process. This clinic or hospital needs to put on check by Joint commission.

The submerged endoscope cannot be placed in water before turning on the MU1.
