It's not because of junkies 😂 And Petare is one of the biggest slums but not the biggest. That goes to Neza-Chalco-Itiza in Mexico, which houses roughly 4 million people. I think Rocinha and Venezuela dispute the second place. Also, there are a bit less than 400,000 people in Petare and, in sum, you should not find more than 1,5M slum dwellers in Caracas Greater Area, which has a total population of 7.8M people (which is not to say there's no poverty outside the marginal city belts) To give some context, it is estimated that 30% of the people in Buenos Aires and Lima live in slums, so it's a common phenomenon across Latin America. Likewise, a great portion of the Caracas slums are populated by migrants from neighbouring countries that arrived from the 60's till the early 2000's.
