And I wish I'd known all this before mine last September at 56. But I knew none of it. I didn't really know what carbs were. Now, I watch youtube vids a few hours a day or more. I now know more about nutrition than most nutritionists (ran into one the other day who confused ketosis with ketoacidosis) Hoping I can use that to stave off the further progression of the coronary artery disease. This is the most comprehensive complete information I have found yet on heart disease. I lost mynyounger brother 😢 to a massive heart attack. He was only 42 a good man, not over weight. He had 80 percent blockage in one ventricle and 50 percent in the other. We have all mourned very deeply over his loss. I cannot express how grateful I am for this information 🙌 🙏.
Ivor, my husband and I have watched many of your videos. This was first time seeing this video. I did not know about your dad. It reminds me of my dad. My dad practiced as a family physician in rural America for 38 years. He was an early adopter of the Dr. Atkins Diet in the 1970s and the fasting approaches but his patients did not want to hear about these options, they just wanted a pill to fix the problem. And there was a lot of negative press about risk of kidney issues with low carb approach and slowing metabolism with fasting approach. After being worn down, and trying to follow evidence-based medicine approach, he abandoned low carb and fasting and recommended calorie restriction and exercise with low fat dietary approach. My dad developed Type 2 Diabetes in his 60s and dementia in his 70s. He died this past year after 8 years with a poor quality of life. He would have LOVED the science you and other low carb community speakers present. Like you, I wish my dad would have been around to benefit from the growing evidence that hyperinsulinemia is the primary culprit. P.S. Thanks to your CAC awareness campaign, my husband got the scan done with very high score of 1514 leading to next day cath lab session, clearing of 4 blocked arteries and placement of 2 stents. He had reversed Type 2 Diabetes in previous year with keto/IF and reassured he has dodged the bullet and has taken steps to ensure low risk moving forward.

Ivor you have out-done yourself yet again. What a fantastic talk! Although I’ve been learning from many others the past couple of years I really believe I owe you at least 75% of the credit for my own turnaround. A few years ago-after 15-20 years with a Tg:HDL ratio of >5 (once as high as 9.5!!!)-I suddenly brought it to a more reasonable level of ˜3 simply by eliminating sugar. While feeling pretty good about myself, I came across your videos, then your book, and came to realize how much more I had to do. Yesterday morning I received my latest lipid profile, showing a Tg:HDL ratio of 1.35...a level I used to think was impossible for me. And what did the doctor say, do you suppose? That I have borderline high LDL (which, by the way, has also dropped some) and therefore I should eat less fat, more whole grains, and consider medications. Wonderful advice...the exact opposite of what I’ve done to make this much progress!! Sorry doc, I’m driving this train now and my next stop is to get this ratio below 1.

You’ve brought so much power and tools by your commission and dedication. Thank you! Unfortunately my cardiologist refuses to have anything to do with CAC scan and neither does my Medicare here in the USA. Don’t know if they will ever wake up. In July I had oral surgery but needed my cardiologist to give the “OK” but only after a nuclear stress test which cost the insurance company a pretty sum and left me with a $200 copay. A local scan clinic said my out of pocket cost for a CAC would be $75. No insurance needed. This is how stupid medicare in the USA is. They’re always complaining about money yet their administrators like to certainly spend it making it difficult for those of us on social security to come up with the high copay.

Certainly explains my experience with canned fish... I have advanced atherosclerosis and I noticed that eating canned tuna would make my heart throw a fit later in the day. Turns out there's a non-trivial amount of oxidized cholesterol in canned fish (I think they heat it to a high temp before canning to prevent botulism)-- and it was enough to cause immediate problems for my situation. I can eat salmon fried with butter (low temp) with no issues so I'm eating fish that way now instead, but I do miss the cheapness and convenience of canned fish.

I looked up the study he mentioned saying low carb/keto increases risk of atrial fibrillation and it was completely disingenuous and intellectually dishonest. They studied three groups of people "carbohydrates comprised less than 44.8 percent of daily calories, 44.8 to 52.4 percent of calories, and more than 52.4 percent of calories, respectively." So their "low carb / keto" group were eating 44.8% or less of their daily calories from carbohydrate when an actual ketogenic diet is typically only 5% from carbohydrate and not consuming any sugar, starch, or grain. To eat "less than 44.8%" of calories from carbs you would have to be eating some sort of grain at the very least if not also starch and or sugar. For all intents and purposes the study basically made observations of three groups of people all eating a high carb diet and made statistically insignificant observations about their incidences of atrial fibrillation. As far as I can tell, only the Daily Mail article mentions "keto." The actual quotes from the researcher and the paper don't mention the word "keto," only "low carbohydrate," which is an expression only relevant in the context of describing their lowest carbohydrate group, not an actual low carbohydrate diet. Completely useless propaganda.
Can you explain the process leading to Hypertension in someone who is slim, active, follows a Keto diet and takes supplements. If Hypertension is there, despite the lifestyle choices being good, then what should one do?

Question about your "Soft Plaque" comment. In discussing this with a Heart Researcher, they, too, commented that the CAC Test ONLY reports Calcified Plaque and not Soft Plaque. Your remarks were not clear on this concern. The Heart Researcher confirmed that many heart attacks do occur from just "Soft Plaque" that has not calcified. I agree, this test is still valuable but shouldn't further research be done on identifying "Soft Plaque"?
It's relying on corelation. Bill Clinton had a score of over 1000, whereas Tim Russert a sore of around 200 but Russert died of an MI despite passing tests. Calcified plaques will eventually restrict circulation ( if collaterals don't develop).
I would love you to compare your recommendations to those of Caldwell and Rip Esselstyn. I'm well-versed with both of you and I have no idea which approach is superior. I would love to get your take on this and the Esselstyns' as well. Right now I eat in concordance with your recommendations, but I'm not sure. Frankly, I eat Mediterranean keto - no red meat and mostly olive oil (for fat) along with some milk fat from Bulgarian yoghurt and buffalo cheese.

most doctors seem ‘content’ with whatever they’re taught so that they can minimize time per patient. Most engineers worth anything question ‘everything’ and will naw on a problem until the cows come home. Every little minutia has to be scrutinized.
I do have a question regarding Oxidized LDL. From what I have heard earlier Statins ONLY allow for the removal/recycling of Non oxidized LDL throught he liver, the oxidized LDL not 'fitting' the liver LDL receptors. THUS if you take statins you would be actually increasing the percentage of Oxidized LDL to normal LDL in the blood. THUS if you are say a diabetic and so more likely to have blood sugar spikes- therefore more glycated LDL (oxidized LDL), then Statins are an even greater risk for you. Diabetics blood sugars high spikes prior to the effect of an insulin shot, or metformin. Even though Diabetic insulin shots reduces blood glucose to normal levels, you still get the spikes each time, as the shots/efffect come after the spike. I would be researching this, as Statins may Increase risk for Diabetics.
It's not the ratio of healthy/oxidized LDL that matters, but rather the absolute amount of oxidized LDL, since most of it will end up in your arterial walls. The more healthy LDL you have, the more can be oxidized if you don't fix the root causes (mostly insulin resistance). So if statins lower the healthy LDL circulating in your blood, you have less LDL that will get damaged/oxidized as well. It's definitely not the optimal way to go about resolving the situation, but it does work on reducing the amount of oxidized LDL and as such it will reduce heart disease risk. The issue is though, LDL is necessary and your liver creates it for a reason, and lowering it will have side effects.
@@Kurouzzz Yeh, that is the problem removing good LDL should be creating other issues since it is there for a reason. Removing may exacerbate some other issues. Wonder if any studies on this with regard to LDL and oxidized ldl. Also if you continually removing good LDL via statins then you will likely be increasing the build up of the bad LDL, i.e. the concentration increases if glycation continues. so my original question still remains. I can imagine a heavy sugar eater on statins would be doing more harm than good with the statins, exacerbating the issue.
The worst thing about these presentations is the lack of time spent on the remedy. If you blink you will miss it.. meanwhile your brain will be bombarded with technical jargon you won't understand. Otherwise very good!
A CAC is not cheap here. Anyway, if you have followed the standard food pyramid diet and middle aged, I guess you can assume you are at higher risk. So even without a CAC, act as if you are at higher risk. Moderate exercise and a Keto diet. supplement with things like VC, MSM, K2mk7 all the things an aging body needs. If you are outside Australia, add Melatonin.
Cheap? Is life/health cheap? I'd argue that you're better off to get the CAC score than stay in the dark. The CAC number may provide the scare and dose of reality needed to make one deadly serious about doing the right things and sticking with it 100% and for the long haul.

Pretty much on low carb for life , reason being I just can't control my sweet tooth, if I eat carbs I eat crap and have no control. I just have to pretend I eat the standard diet to my parents who are in their 70s and are on low fat, low meat etc. They would be horrified with the amount of butter and eggs I eat.

Big fan of Ivor, plan to ask for a CAC scan because of Ivor. Been on LCHF/KETO for about a year now and also doing intermittent fasting during the work week. All that being said I notice Ivor has a fair amount of visceral fat. Now he knows about LCHF/KETO and presumably he's doing it. Why isn't he lean yet? I think vitamin c has helped me, but after 3 years on Keto, the amount needed to reach bowel tolerance had gone way down, now 20 percent or less of what it was when I started vitamin c.

Do you really believe that it is God's will for you to suffer in a sickbed, diabetic, with an amputated leg, stuffed full of pharmaceuticals, just because you have not controlled your cravings for food and eat things that God did not intend for man to eat? There is nothing wrong with having high LDL if you have the large fluffy kind and if your trigs are low and your trig/HDL ratio is low and you are not insulin resistant. If your CAC Scan is low, then you have very low risk. So don’t even bring up LDL scores or raw cholesterol scores for that matter; they are largely irrelevant.

Thanks to your CAC awareness campaign, my husband got the scan done with very high score of 1514 leading to next day cath lab session, clearing of 4 blocked arteries and placement of 2 stents. He had reversed Type 2 Diabetes in previous year with keto/IF and reassured he has dodged the bullet and has taken steps to ensure low risk moving forward.
I'm going to listen to this again to catch what I missed the first time.

Although I’ve been learning from many others the past couple of years I really believe I owe you at least 75% of the credit for my own turnaround.
A few years ago-after 15-20 years with a Tg:HDL ratio of >5 (once as high as 9.5!!!)-I suddenly brought it to a more reasonable level of ˜3 simply by eliminating sugar.
While feeling pretty good about myself, I came across your videos, then your book, and came to realize how much more I had to do.
Yesterday morning I received my latest lipid profile, showing a Tg:HDL ratio of 1.35...a level I used to think was impossible for me.
And what did the doctor say, do you suppose? That I have borderline high LDL (which, by the way, has also dropped some) and therefore I should eat less fat, more whole grains, and consider medications. Wonderful advice...the exact opposite of what I’ve done to make this much progress!!
Sorry doc, I’m driving this train now and my next stop is to get this ratio below 1.

Unfortunately my cardiologist refuses to have anything to do with CAC scan and neither does my Medicare here in the USA. Don’t know if they will ever wake up. In July I had oral surgery but needed my cardiologist to give the “OK” but only after a nuclear stress test which cost the insurance company a pretty sum and left me with a $200 copay. A local scan clinic said my out of pocket cost for a CAC would be $75. No insurance needed.
This is how stupid medicare in the USA is. They’re always complaining about money yet their administrators like to certainly spend it making it difficult for those of us on social security to come up with the high copay.

Certainly explains my experience with canned fish... I have advanced atherosclerosis and I noticed that eating canned tuna would make my heart throw a fit later in the day. Turns out there's a non-trivial amount of oxidized cholesterol in canned fish (I think they heat it to a high temp before canning to prevent botulism)-- and it was enough to cause immediate problems for my situation. I can eat salmon fried with butter (low temp) with no issues so I'm eating fish that way now instead, but I do miss the cheapness and convenience of canned fish.

Ditto my Dad drank himself to death.....but he ate bacon, bone broth, ham shanks, tripe etc and the doctor who examined him before colon surgery said that at 71 he had a heart age years younger....it was as clear as a bell.

I looked up the study he mentioned saying low carb/keto increases risk of atrial fibrillation and it was completely disingenuous and intellectually dishonest. They studied three groups of people "carbohydrates comprised less than 44.8 percent of daily calories, 44.8 to 52.4 percent of calories, and more than 52.4 percent of calories, respectively." So their "low carb / keto" group were eating 44.8% or less of their daily calories from carbohydrate when an actual ketogenic diet is typically only 5% from carbohydrate and not consuming any sugar, starch, or grain. To eat "less than 44.8%" of calories from carbs you would have to be eating some sort of grain at the very least if not also starch and or sugar. For all intents and purposes the study basically made observations of three groups of people all eating a high carb diet and made statistically insignificant observations about their incidences of atrial fibrillation. As far as I can tell, only the Daily Mail article mentions "keto." The actual quotes from the researcher and the paper don't mention the word "keto," only "low carbohydrate," which is an expression only relevant in the context of describing their lowest carbohydrate group, not an actual low carbohydrate diet. Completely useless propaganda.

Question about your "Soft Plaque" comment. In discussing this with a Heart Researcher, they, too, commented that the CAC Test ONLY reports Calcified Plaque and not Soft Plaque. Your remarks were not clear on this concern. The Heart Researcher confirmed that many heart attacks do occur from just "Soft Plaque" that has not calcified. I agree, this test is still valuable but shouldn't further research be done on identifying "Soft Plaque"?
CAC reflects ALL plaque - hard, mixed morphology, soft. That is the point. It represents all. Hence is the ultimate risk estimator.
It's relying on corelation. Bill Clinton had a score of over 1000, whereas Tim Russert a sore of around 200 but Russert died of an MI despite passing tests. Calcified plaques will eventually restrict circulation ( if collaterals don't develop).

Right now I eat in concordance with your recommendations, but I'm not sure. Frankly, I eat Mediterranean keto - no red meat and mostly olive oil (for fat) along with some milk fat from Bulgarian yoghurt and buffalo cheese.

most doctors seem ‘content’ with whatever they’re taught so that they can minimize time per patient. Most engineers worth anything question ‘everything’ and will naw on a problem until the cows come home. Every little minutia has to be scrutinized.
I do have a question regarding Oxidized LDL. From what I have heard earlier Statins ONLY allow for the removal/recycling of Non oxidized LDL throught he liver, the oxidized LDL not 'fitting' the liver LDL receptors. THUS if you take statins you would be actually increasing the percentage of Oxidized LDL to normal LDL in the blood. THUS if you are say a diabetic and so more likely to have blood sugar spikes- therefore more glycated LDL (oxidized LDL), then Statins are an even greater risk for you. Diabetics blood sugars high spikes prior to the effect of an insulin shot, or metformin. Even though Diabetic insulin shots reduces blood glucose to normal levels, you still get the spikes each time, as the shots/efffect come after the spike. I would be researching this, as Statins may Increase risk for Diabetics.
It's not the ratio of healthy/oxidized LDL that matters, but rather the absolute amount of oxidized LDL, since most of it will end up in your arterial walls. The more healthy LDL you have, the more can be oxidized if you don't fix the root causes (mostly insulin resistance). So if statins lower the healthy LDL circulating in your blood, you have less LDL that will get damaged/oxidized as well. It's definitely not the optimal way to go about resolving the situation, but it does work on reducing the amount of oxidized LDL and as such it will reduce heart disease risk. The issue is though, LDL is necessary and your liver creates it for a reason, and lowering it will have side effects.
@@Kurouzzz Yeh, that is the problem removing good LDL should be creating other issues since it is there for a reason. Removing may exacerbate some other issues. Wonder if any studies on this with regard to LDL and oxidized ldl. Also if you continually removing good LDL via statins then you will likely be increasing the build up of the bad LDL, i.e. the concentration increases if glycation continues. so my original question still remains. I can imagine a heavy sugar eater on statins would be doing more harm than good with the statins, exacerbating the issue.
The worst thing about these presentations is the lack of time spent on the remedy. If you blink you will miss it.. meanwhile your brain will be bombarded with technical jargon you won't understand. Otherwise very good!

A CAC is not cheap here. Anyway, if you have followed the standard food pyramid diet and middle aged, I guess you can assume you are at higher risk. So even without a CAC, act as if you are at higher risk. Moderate exercise and a Keto diet. supplement with things like VC, MSM, K2mk7 all the things an aging body needs. If you are outside Australia, add Melatonin.

Cheap? Is life/health cheap? I'd argue that you're better off to get the CAC score than stay in the dark. The CAC number may provide the scare and dose of reality needed to make one deadly serious about doing the right things and sticking with it 100% and for the long haul.
@@sleepy580 yes Vitamin C. I prefer lyposomal, though do make my own ascorbic acid capsules as well. Since you body cant make C, and on a Keto diet I want to make sure I get enough, plus it does have a number of critical functions.
Pretty much on low carb for life , reason being I just can't control my sweet tooth, if I eat carbs I eat crap and have no control.
I just have to pretend I eat the standard diet to my parents who are in their 70s and are on low fat, low meat etc. They would be horrified with the amount of butter and eggs I eat.

Big fan of Ivor, plan to ask for a CAC scan because of Ivor. Been on LCHF/KETO for about a year now and also doing intermittent fasting during the work week. All that being said I notice Ivor has a fair amount of visceral fat. Now he knows about LCHF/KETO and presumably he's doing it. Why isn't he lean yet? Would he be much worse off if he wasn't doing KETO? Is it less effective for some people?

Low carb reduces the body's requirement for Vit C and there is vitamin c in all animal foods provided they are not overcooked
I think vitamin c has helped me, but after 3 years on Keto, the amount needed to reach bowel tolerance had gone way down, now 20 percent or less of what it was when I started vitamin c.

There is nothing wrong with having high LDL if you have the large fluffy kind and if your trigs are low and your trig/HDL ratio is low and you are not insulin resistant. If your CAC Scan is low, then you have very low risk. So don’t even bring up LDL scores or raw cholesterol scores for that matter; they are largely irrelevant.
