Arthritis is understood producing stiffness BUT THE PROBLEM is when it pinches the nerve and you loose STRENGTH loose ability to walk . It is debilitating but hard to find a Doctor sensitive to this problem

I had acdf a year ago in c3 c4...now having lumbar issues at l5 with radiculapthy down left hip and leg. Started with sciatica in February. Seeing neurosurgeon Friday.
I have had 5 spinal surgeries with fusion. I am now having difficulty walking unless I use the " shopping cart stance". I have been diagnosed with OA /stenosis. I use a walker.when I stand up straight I have excruciating pain in my T spine area my right leg and foot, in particular the toes. T2 to T10 have not been operated on but C3 to S1 have all had fusions. I am struggling to walk but the pain is chronic and extreme unless I am sitting or lying down. It it not the pain I had before surgeries. This is tightness, burning and inability to move my arms, legs and drop foot without pain. I have been told a pain stimulator may help with pain between shoulders.

I can barely walk across the street without having to sit quickly. Spinal stenosis is very Debilitating for a person still trying to work, walking and lifting all day.
@@DonnallySpineConsult yes I have had an MRI, no explanation from a Dr. and I can't understand it. I've been told I have spinal stenosis and do not do the cobra because I've been trying to self heal. I walk but can only walk a bit before having to sit. What you mention I was reading about but only claudication now I will look up the other part. My back has hurt on the right side for over 25 years, it travels to buttocks until I sit. As long as I do not try to stand straight up I'm okay. Thanks for the reply

I saw your earlier comment and wrote a lot. But there are many fields where there are 2-3 paths to get there. Same w spine. Some NSG programs do a ton of brain things, little spine surgery. Some ortho places do a TON of spine. Fortunately for ortho we get the ability to go do 1 additional year elsewhere of spine surgery. We again have to interview and apply, only the best get to continue
I have age related scoliosis and I’m 76. About 2 years ago the L5 slipped out of place and pushed on the disc pinching the nerve. I had decompression to help relieve my pain. I was told because of the scoliosis on the top of my spine and the slipped L5, that there was no fix for my back. Can you tell me if I’m a candidate to fix my L5 or if I’m fresh out of luck. I did have shots but they didn’t fix a thing.
I suffered spinal stenosis for almost 3 years ago. L3, 4,5 and S 1 was not undergone surgery just took med and some exercises. I can walk now but theres a time if i move or bent wrong it hurts the only solution to stop the pain is i lay flat and 1 hour or more i felt good. Heavy lifting too hurts.
Thank you for the information. I just completed physical therapy and am able to postpone surgery at this time. You made a comment about Aetna being "the devil". I have an Aetna plan and wonder if I should consider a better carrier going forward.

You previously commented on my LSS and arthritis of the spine. One doctor said coflex. You said NO coflex. I went to two other spine surgeons who agreed no coflex but said PT for now, but fusion of L4 L5 down the road if PT doesn't help. How long should I give PT to work? Months, years?

I've been recommended for a four level ACDF by my neurosurgeon in Pennsylvania. I have radiculopathy in both arms, right side is worse. I've been doing research on some of the endoscopic procedures that don't do fusion, only decompression by removing the material that has escaped through the annular tear. What are your thoughts about this approach? I know it's more popular in other countries as is disc replacement approach. Are spine surgeons beginning to accept this approach? Is there enough data to show efficacy? I've watched presentations from the Seattle Spine Surgery group about both endoscopic and replacement approaches that seem to show they are becoming more accepted. It seems like a big hurdle is the expense for hospitals to purchase the equipment. I'm a bit frustrated and feel like I'm between the old and new right now. You did say you were very high on the ACDF procedure. Glad to hear that. I'm a bit worried about adjacent segment disease and trachea/esophagus issues because of mine be four levels. Could you talk about spinal cord arthritis as I just had this operation 8 weeks ago it was c6 c7 I was explained that my spinal cord was being squashed no white lines on either of the cord I was loosing the use of my both my arms as well I had nerve spray down my left arm to my hand I had no feeling in my middle finger I am now fine n happy with surgery

Hi hope your keeping well! I missed your live video because I live in Ireland. I wanted to ask you about the pain I'm in after having a 2 level ACDF surgery from c5 to c7 fusion in May of last year. I've still have the pain coming down from right side of my neck down into shoulder blade h down my right arm. Sometimes down my left arm with numbness & tingling in my pinky finger. I am due back to see my spine surgeon in November 7th 2024. I had an MRI which showed degenerative disease on the foraminal side in c5 to c7. There is also degenerative disease at c3 to c5 im wondering if it is adjacent segment disease. Can I ask you for some advice as to what you as a surgeon would do in my case. I have also had a right hip replacement in November 2024 & have degenerative disease in my left knee. I had a one level ACDF and I couldn't figure out why after surgery I was so hoarse and kept clearing my throat that it was because of the esophagus being moved and held out of the way. My surgeon never mentioned that it might occur. It's not bad it's just very annoying. I felt like I had a little chicken bone caught in my throat but it eventually got better thank goodness.
do you ever go over what are things people should ask you if the patient requires lamectomy and fusion in the neck. Do think it is good to have a second opinion?

Maybe you have something called neurogenic claudication. Have you had an MRI

I suffered spinal stenosis for almost 3 years ago. L3, 4,5 and S 1 was not undergone surgery just took med and some exercises. I can walk now but theres a time if i move or bent wrong it hurts the only solution to stop the pain is i lay flat and 1 hour or more i felt good. Heavy lifting too hurts.

Thank you for the information. I just completed physical therapy and am able to postpone surgery at this time. You made a comment about Aetna being "the devil". I have an Aetna plan and wonder if I should consider a better carrier going forward.

I've been recommended for a four level ACDF by my neurosurgeon in Pennsylvania. I have radiculopathy in both arms, right side is worse. I've been doing research on some of the endoscopic procedures that don't do fusion, only decompression by removing the material that has escaped through the annular tear. What are your thoughts about this approach? I know it's more popular in other countries as is disc replacement approach. Are spine surgeons beginning to accept this approach? Is there enough data to show efficacy? I've watched presentations from the Seattle Spine Surgery group about both endoscopic and replacement approaches that seem to show they are becoming more accepted. It seems like a big hurdle is the expense for hospitals to purchase the equipment. I'm a bit frustrated and feel like I'm between the old and new right now. You did say you were very high on the ACDF procedure. Glad to hear that. I'm a bit worried about adjacent segment disease and trachea/esophagus issues because of mine be four levels. Could you talk about spinal cord arthritis as I just had this operation 8 weeks ago it was c6 c7 I was explained that my spinal cord was being squashed no white lines on either of the cord I was loosing the use of my both my arms as well I had nerve spray down my left arm to my hand I had no feeling in my middle finger I am now fine n happy with surgery

I’m 72 with spinal stenosis in left lower area. Most pain is either with standing, overhead stretching and lying in bed. I still play pickleball daily and can walk without pain. How long do I wait to have surgery? I’ve had two injections this year.

Hi hope your keeping well! I missed your live video because I live in Ireland. I wanted to ask you about the pain I'm in after having a 2 level ACDF surgery from c5 to c7 fusion in May of last year. I've still have the pain coming down from right side of my neck down into shoulder blade h down my right arm. Sometimes down my left arm with numbness & tingling in my pinky finger. I am due back to see my spine surgeon in November 7th 2024. I had an MRI which showed degenerative disease on the foraminal side in c5 to c7. There is also degenerative disease at c3 to c5 im wondering if it is adjacent segment disease. Can I ask you for some advice as to what you as a surgeon would do in my case. I have also had a right hip replacement in November 2024 & have degenerative disease in my left knee. Can I ask your advice for what might be next for me please I would appreciate any information/ advice you can give. Love your videos theyre very informative so thank you for that. Love from Ireland

Crustal I had ACDF . Not bad but get the right surgeon . Buy a lot of pudding and jello first first week .
I had a one level ACDF and I couldn't figure out why after surgery I was so hoarse and kept clearing my throat that it was because of the esophagus being moved and held out of the way. My surgeon never mentioned that it might occur. It's not bad it's just very annoying. I felt like I had a little chicken bone caught in my throat but it eventually got better thank goodness.
