Hi, it seems that your iPhone has never synced with the laptop before it's locked. The problem isn't about your USB cable. You just need to use iPhone Unlocker tool 1:20 to unlock your device. Or 3:39 use your laptop to put your iPhone into recovery mode and erase everything including the passcode on the device.😄

Don't worry for that, and you just repeat the process to enter recovery mode again. There's no harm.

No, you have to use iPhone Unlocker to retore your phone to factory settings and set a new passcode. There's no way to enter your original passcode when it's disabled connect to iTunes.

Hi, did you use iPhone Unlocker? Make sure you install iTunes (or Finder on macOS 10.15 ) on your compuer so iPhone Unlocker recognizes your iPhone.

There's no way to recover your passcode if you can't remember it. The only way to fix your disabled iPhone 12 Pro Max is restore it back to factory defaults. The article explains this.
