Why isint answer b to the first example 1/4? isn't Vp/Vs = Np/Ns?
That is right, but the question asks for the ratio of secondary to primary, Ns/Np.

Mr. Doner, in the exam, should I put the negative (minus) sign in? This question is in general for any question not just related to transformers.
You should know what the negative sign means. I can not think of any numerical instances where it would matter. See the Lenz's law video if you do not know why there is a negative slgn..
