Uncheck the "block untunneled traffic (kill switch).

least that gets the internet moving for my client. I've not tested lan access but it seems to be working fine without that kill switch

Depends, it should automatically open the port Via UPnP, but some ISP routers don’t do this

No, port forwarding isn't required. As long as it's not blocked by your firewall (3 rules, WAN, Wireguard, LAN) then you're okay.
