Thank you Dr Ryan for all your efforts. I found during community service that I used these materials over the years and you make me stop and think about the importance of the development of these materials. For example I used Ketac silver or Cermet- ionomer as restortaive material and I found it was part of an effort to improve the filler of Glass Ionomer restortaion by adding metal alloy to improve the filler part and reinforce( first approach). Then the second approach was to improve the matrix not the filler by adding Di-methyacrylate monomer which you described in other series to have covalent crosslinking of the matrix through free radical by creating carbon- carbon bond in addition to primary ionic bond in GIC which is excellent in order to reduce the secondary bonds that is causing poor mechanical properties such as weakness, law modulus of elasticity, and law fracture toughness. I found also that the ionic bonds through acid -base reaction leads to release of calcium, aluminium,sodium,fluoride and silicate ions which is favorable for caries control.Thank you very much. Dr K. I have a video on metal alloys in my Prosthodontics series, but not much on the technical aspects of gold restorations since they are not tested routinely on the board exam. That is a great idea for a future video though!

i have a question regarding composite types and your categorization in the operative INDBE video #6
to summaries what i understood is that larger filler particles causes the matrix of the composite to wear off faster making the composite rougher and higher filler content causes less water absorption and shrinkage
however you ended the slide by saying that in case of flowable composite the good thing is that it has lower wear resistance, wouldnt that be a disadvantage ?, and since it has lower wear resistance wouldnt it then have large particles and high content of filler since we stated that too little matrix is what causes the wear and exposes rough composite
i assumed that low filler content and size would mean weaker composite that wears off more and high filler conc. is what would prevent it wearing for example the macrofill would be strongest with the highest wear resistance and flowable the opposite
a paper i read as well suggests that the higher the filler content of a composite the more wear resistance it has.

I'm a little confused by the C factor of Class V lesions. Wouldn't a classic preparation yield a C-factor of 5?

Hello dr please I HAVE A QUESTION in regards to the C factor for the cl v , is it should be 5/1 just like class one ? as we have all the walls, I LOOKED IN TO sources and some says its this ratio and some says its the reverse
this is important as its gonna be bad prognosis for it just like class 1 for the shrinkage, meanwhile if its the reverse its gonna have excellent prognosis with minimal shrinkage
Hello, there are several other comments that have already asked this question... not sure if you saw them. It really comes down to how exactly the tooth was prepared for a Class V restoration. Some resources will consider the entire dish-shaped prep as one surface only, which explains having only one bound surface. Other resources, consider each wall separately for a 5:1 ratio. I would stick with the 1:5 ratio, personally.
Is the resin tag composed of only primer? Or is it a mix of the primer and adhesive?
Hard to say exactly, because it depends on the bonding system you use. It’ll be mostly primer if you’re using a separate primer, but otherwise it’ll be a mix of the two.
do you mean lower the C-Factor, more the microleakage since Class V has 0.2 CF and it has greater microleakage?
Theoretically, according to the overall trend, the more surfaces that are bonded to the higher risk composite will “peel” away from one of those surfaces as it undergoes polymerization shrinkage. Class V has the lowest amount of walls bonded to (theoretically, but not always practically).
Can glass ionomer cement be used when there's a deep carious lesion and you left behind affected dentine.
Affect dentine even through it's hard doesnt bond well with resin composite, can you place glass ionomer filling and then place composite after a few months?
You are absolutely correct that caries-affected dentin negatively influences bond strength. You can definitely do what you are suggesting, although sometimes the area of affected dentin will not be large enough to impact bond strength and subsequent leakage at the periphery of the restoration where the bond is perhaps most important.
How would a Class V restoration have such a low C-factor? Wouldn't a traditional class V have the same C-factor as a Class I restoration?

Hello to my favourite TH-camr!! I am a little confused Dr. Ryan, Class V has 5 bound surfaces, therefore the ratio of the bound to unbound surfaces remains the same as the Class I, 5:1. I imagined a box, Class I is a box with a missing roof and so is a Class V but a differently shaped box with not so high walls (but there is walls). Please correct if I'm wrong!
Could it be the surface area? or/and the amount of composite used?
