They are supposed to knock three times before asking "Are you there, Hanako-san?" Then they will hear a voice answer, "I'm here." If the person chooses to enter the stall, there will be a small girl with bobbed hair in a red skirt.
For those who don't know, yata no kagami is a sacred mirror that represents wisdom and honesty based off of the source. It is a part of the Imperial Regalia of Japan. It is not accessable to may people, so they don't know if it's located in Ise Grand Shrine in Mie Prefecture, Japan or not.

Toilet Bound Hanako-Kun AU idea: Hanako is the ghost of a girl who suffered from severe gender dysphoria and ended her life to escape the abuse she suffered after coming out about her dysphoria. After becoming the 7th Wonder, Hanako gained the ability to change his appearance and thus transformed into the boy that he had always been on the inside.
This is why they made that anime-

the ending we saw shows your player character taking hanako's place and basically becoming her. im assuming the other endings are probably:
a) eri becomes hanako (it states "im glad i didnt give eri this power")
b) hanako is defeated for good
