    Thank you so much for all of this info!!! I'm just starting and curious if I take my pill in the morning with 2-4T Olive Oil, is that sufficient for absorbtion or do you need solid food with it as well?

      I can't speak to your specific situation, and no one has specifically looked at olive oil, but 2-4T of olive oil is about 25-50g of fat, which one would expect is likely sufficient.

      Isotretinoin is metabolized (cleared from the body) by the liver, so it should be used carefully in those who have liver disease.

    hi Dr, just started very low dose accutane and was wondering, if i take it at 5:00am one day, can i take it at 6:30am the nest day or does it have to be exactly at 5:00am everyday? I appreciate all of your content!

      In general (not speaking to your specific situation), the most important thing in terms of timing of when to take isotretinoin is to make sure it is taken with a meal that has fat in it (to help with absorption; for Lidose/micronized isotretinoin this is less of an issue).

      Isotretinoin can rarely cause a telogen effluvium like hair loss that often improves after stopping the medication. Some studies suggest that it is more common with higher dose regimens.

    I have been taking Accutane ( 10mg per day ) from last November to this year's August. My acne is cleared but should I continue to consume it for precaution?

In general, with isotretinoin, the goal is to achieve a long-term clearance, so typically we discontinue it once someone has had clear skin for >2-3 months and reached >120mg/kg of cumulative dose. About 60-80% of individuals with have no meaningful recurrence of acne after discontinuing isotretinoin.

    What are your thoughts on different brand. I’m on 64 mg of Absorica LD with no progress

      I don't think the brand matters that much. For some who have trouble with absorption, using a Lidose isotretinoin (Absorica) or micronized isotretinoin (Absorica-LD) can be helpful and can make dosing more consistent. But I think most can do well with any formulation of isotretinoin (just might need a higher dose with standard isotretinoin).

    If I was to need to switch to Accutane from doxycycline how long do you typically suggest you wait between medications? does it have to be 7 days? I am worried about breaking out in between ?

      Yes, assuming it was otherwise the same formulation (i.e. need to consider that micronized isotretinoin has different dosing due to the improved absorption).

      Although there is some uncertainty regarding whether isotretinoin is associated with inflammatory bowel disease or irritable bowel syndrome, high quality studies have not found an association between isotretinoin exposure and either inflammatory bowel disease or irritable bowel syndrome. Of note, oral antibiotics are an established risk factor for development of inflammatory bowel disease, so this must be kept in mind when considering treatment options (as well as when designing studies as it can be a confounding factor).

    Hey... Today my skin doctor prescribed me micronized isotretinoin capsules 20mg .. Is this also called accutane??

      Accutane is the original brand (now left the market). Micronized isotretinoin is same drug, but better absorbed. It can be expensive, and unclear that it is meaningfully better than Lidose-isotretinoin.

      It typically takes 5-8 months to complete a course of isotretinoin. Depending on acne severity and dosing regimen, it is not unusual for some acne to still be present until month 4-5 or later.

      I cannot comment on what is best for you (you should discuss with your doctor). In general, a treatment course of isotretinoin is at least 5-8 months.

    I'm currently on 20 mg isotretinoin per day I have mild to moderate acne ihave been taking it from 30 days i have no side effect only side effect i see some dryness over lips Nothing else But i sae on internet the low dose of isotretinoin is.5mg/kg Im 75 kg should i take 40 mg now Please doc my dermatologist is not good i have changed so many they all consult me just for 2-3min

      No. In general, higher cumulative doses are associated with greater likelihood of long-term clearance, but some people need higher doses and some need lower doses. Achieving clear skin x2-3 months before stopping is a good predictor of long-term clearance.

    Sir my demetrologist put me first 20 mg daily for 1 month after one month i get rare side effects pyogenic granuloma (PG). He say to stop taking isotretinoin for 2 weeks than i stopped. Again after consultation he recommended me 20 mg every other day it take 20 mg alternative day for 2 months. i had grade 3 acne after one month only my acne are stopped to come . After 20 mg alternative day he prescribed me 10 mg alternative day now . I have too much oily face my acne are too much recurrance before isotretinoin. After lowing my dose to 10 mg alternative day i see change on my face my acne hasn't come back but all oilness has come back acne also come 2,3 in a week . I don't know what to do and what my demetrologist is doing. Please advise me doctor??

Demetrologist say to continue isotretinoin for 2 months 10mg alternative day than we should discontinued to used he say. Sir should I have to change my demetrologist or what

      For adult acne, it will inevitably return. No permanent cure for adult acne
