ACLS Megacode Scenario 3: Bradycardia to Ventricular Fibrillation to ROSC

  • This is the third ACLS megacode scenario in the "Megacode Series" by Health Ed Solutions and it will cover Bradycardia to Vfib to ROSC. The megacode scenarios are intended to test your ability to put your algorithm knowledge to work. For each megacode simulator, there will be a scenario and you will have to identify rhythms, prescribe treatments, and recommend the next steps to properly provide optimal care for the patient. Rationales are included with the correct answers to support each clinical decision.

    Well done. It covers everything.
    Just an update: new guideline 2020
    Atropine is given at 1 mg instead of 0.5 mg q3-q5 min.
    Also, dopamine dose now is 5-20 mcg instead of 2-10 previously.
    Epi remains the same.

    The 2015 AHA guidelines recommend Epinephrine to be given after the second shock (defibrillation), not after the first shock.

      @@HealthEdSolutions ok, So in this incidence...after the initial defib, and 2 min cpr.. since it had not changed yet, continue with Dfib at 200j correct? Then if you get the PEA, you give the Epi, and continue high quality CPR...yes?

    At 5.40, first need to give second dose of defibrillation, then only give epinephrine, as patient is still in VF after first dose of defibrillation.

    Pulseless Electrical Activity was present the first time you couldn't feel a pulse (3:04) not VF, and immediate epi is indicated, not defibrillation.

      PEA: "any organized rhythm without a pulse is defined as PEA, including sinus rhythm, atrial fibrillation or flutter, bundle branch blocks, and idioventricular or ventricular escape rhythms" - 2020 AHA ACLS Manual
      so technically VF and pVT are not defined as PEA since the definition of these two rhythms is technically "no pulse, shockable rhythm"

    The BP is 80 which immediately make this scenario an unstable PT. You would never dick around with adenosine when you have a monitor available and can pace. At least thats the current guideline.

      This is good information. However, the order is not exact for in field. You can't defib if pads aren't on the body. If you see Vfib.. chest compression immediately as pads are being put in place and charge aed.Never just wait. If heart is not pumping .. you do it. Remember, you can give all the drugs you want, but if you don't circulate it doesn't get to the areas in need.
      With that said this video is very good
