    As a patient of an AVAP machine thanks for this! I struggled for almost 10 years to get used to using one of these machines but in the last year since I've gotten my AVAP I've never had better sleep. As someone with very little lung capacity this machine has become my lifeline.

    You left out that you also set a "Rate". I love the AVAPS mode and use it a lot in my daily practice. It's my go to mode for post-op surgery patients on pain meds and overdose patients. I defiantly recommend it over Bipap any day.

    In the Resvent Ibreeze 30STA models, AVAPS is built in to the machine if a breathing rate is specified. These machines are for treating sleep apnea only.

    Excellent Joe. Unless I’ve missed one, can you please make a video showing similarities and differences between iVAPS (ResMed) and AVAPS (Phillips), in the context of obstrtive and restrictive disorders.

    Thank you so much for your sharing! this really helps me a lot! After watch few videos about NIV from your channel, It helped me to understand much better in concepts of NIV.
    There is two questions I would like to ask about. May I know is there a leakage alarm or cutoff (e.g. no more than 20L/min Tot. Leak or Pt. Leak so as to keep the effectiveness of set pressure) in the Philips V60? Is mobilization of patient on BiPAP possible? (will that cause leakage and ineffectiveness?) Thank you so much and would much appreciate if you could answer my questions. 🙏🏻

    • @RespiratoryCoach
      @RespiratoryCoach  ปีที่แล้ว

      There is a leak compensation factor, but I'm not sure of the details of that. Yes, you can mobilize a patient on NIV, so long you are using a mobile vent. Hope that helps!

    Great video! We just received our V60s and I'm creating a training workshop for the RT staff. This was very helpful! Question: is there a consensus on where to start the MAX P and MIN P?

  • @jennaslattery2548
    @jennaslattery2548 4 ปีที่แล้ว +2

    Trouble shooting question. If the alarm says targeted tidal volume exceeded decrease min pressure does this mean the patient is air hungry ? Would you increase tidal volume and then decrease the min and max ?

    • @RespiratoryCoach
      @RespiratoryCoach  4 ปีที่แล้ว

      Hi Jenna! It certainly could indicate air hunger. We see this in PRVC too. I would assess my patient to discern if it's related to distress/air hunger or if they just don't need that much support. If it's air hunger, then your solution is appropriate.

    How does it differ with Hi flow like optiflow or the NIV mode in the ventilator?

    • @RespiratoryCoach
      @RespiratoryCoach  2 ปีที่แล้ว +1

      Very, very similar, in how they adjust PS to achieve volume, but not exactly the same.

    •  2 ปีที่แล้ว +1

      @@RespiratoryCoach so, what is the different?

    • @RespiratoryCoach
      @RespiratoryCoach  2 ปีที่แล้ว +1

      @ AVAPS targets a desired tidal volume, while ASV targets a desired minute volume. ASV will also provide mandatory PC breaths if indicated.

      PS as in the difference between EPAP and IPAP (the actual IPAP, not min or max since these are just setting boundaries)

    Hey there! Just discovering your channel based on a random suggestion by TH-cam and I'm amazed. Would you please help me with the setting of target TV for a neuromuscular/SCI patient on chronic phase? How many ml/kg IBW should I program for those patients? are they the same 6-8 ml/Kg IBW?? I'm asking this based on those cases of patients who end up using non-invasive ventilators connected to their tracheostomy with AVAPS or BiPAP mode.
    In relation with that question, how much can I increase the IPAP, and reached or targeted TV on BiPAP/AVAPS in order to rise the PP without risking lung trauma?
    Thank you so much for your help!
    Cheers from Chile!!!

    • @RespiratoryCoach
      @RespiratoryCoach  4 ปีที่แล้ว +1

      Hello JM! Glad you found the channel. In regards to tidal volume and SCIs, this gets interesting. You can find many interesting research articles on the web that support the use of larger tidal volumes in this population of patients. One in particular, although very small, used tidal volumes as large as 20ml/kg. I'm not suggesting or encouraging that, rather just illustrating the vast variance in management of this patient population. The following link supports the use of 10-12 ml/kg. www.ncbi.nlm.nih.gov/pmc/articles/PMC3781830/#:~:text=Our%20standard%20practice%20in%20acute,30%20cm%20H2O. All studies supported plateau pressures to be maintained less than 30. In regards to max IPAP, 20-25 is generally the recommended max IPAP. If you can't get it done with that, a different mode of mechanical ventilation is most likely needed.
      Hope this helps. Reach out anytime and thank you for watching and commenting! Cheers from America!!!

      @@RespiratoryCoach Thanks, one example is the newer Dreamstation bi-pap with what they call Auto Bi-level mode (it also has Fixed Bi-level mode too if you want), with the following settings:
      EPAP 4-IPAP
      IPAP 4-25 cm
      maxIPAP=(minEPAP+3)-25
      minEPAP=4-(maxIPAP-3)
      PSmin 0-8 cm
      PSmax PSmin-8cm
