    Definitely second half of night. Now, having AS for 15-20 years I am almost always stiff and in pain. Mornings are by far the worst. I do not feel human in the morning. I walk around like The Tinman(with no oil) or Frankenstein! 😂

      Oh no. Extended morning stiffness can be a sign of active disease. Just having osteoarthritis can also give joint stiffness in the morning but if it is hours then it could be active inflammation.. Feeling like tinman is tough ❤️

      weight loading exercises are ok just have to be very careful with form. Better to start with lower weights and go up from there.

    Hi. I'm having these symptoms forever but the past 4 months got worse.. neck issue. Now lower back. Eyes. Chest bone.. my fingers..etc. I started martial arts 3-4 years ago and now I barley can do it. Which is killing me because muay Thai and Brazilian Jiu-jitsu became and judo a love affair to me. I can't stop doing them :(

      If you have AS, I hope you are getting the correct medications for it. There are so many different options but it can take time to find the right combo.

    I’ve enjoyed finding your show. 3 yrs ago I walked did some exercises, now I’m suffering with bursitis in left hip. I’ve had both knees replaced,left ankle, neck fusion done. I think when my nerve was nicked during ankle replacement is causing problem with balance . I have inserts for shoes because my legs aren’t the same height. I know I can’t do your exercises except the bench, but I could use a step. Your show is terrific!,,

      Glad it was helpful! You don’t always need supplements, turmeric and garlic are great added in foods. NAC seems to help some people depending on the issue but there isn’t as much data on it yet..

    I check all the boxes and I have died for 18 minutes of cardiac arrest and everything you are saying I have! I wasn't diagnosed until later in life. I was an athlete for years and although my picture shows a man, that's my husband. I have degenerative disc disease. I pitched an 80 mph softball 🥎 I retired years ago. I have Ulcerative Colitis. I am your poster child. 🤦🏽‍♀️

      If it is very early disease some people hold off on immunosuppressants but you would need an expert in AS to monitor because if the disease advances and you get erosions, those are not generally reversible. Good luck.

    I have a question that is slightly off topic but I have wondered about this for quite a long time. I have AS and a few other dx's. I have been on immune suppression for decades now. I have never really got much passed DMARDS. Like most people during this time I have had my fair share of opportunistic infections. I understand why that happens however even when I have had very serious infections confirmed for example kidney infections. (where the surgeon told me he drained purulent liquid out of my kidney, that was behind a large kidney stone, he was bypassing) Despite having all the other symptoms of fever it didn't register on a thermometer. This is quite a regular experience for me. I always just assumed it was because of the immune suppression. However when I tell all sorts of Dr's that my temps cannot be relied upon I get some pretty strange responses. So as a Rheumatologist is there any explanation you can give me for this? Obviously the lack of fever can be dangerous, especially when health professionals rely on those readings. Thanks so much.

      Yes, immunosuppressants can sometimes prevent or mask a fever response. They can lower the body’s natural immune response, which may also affect how it reacts to infections.

    Lots of the pain meds we are given do it too.... I am a longer term AS, HEDS, OA and Fybro sufferer. Best thing I ever did was get on Medicinal Cannabis. Then slowly ween off as much as the pain medication as I possibly could.......Got rid of a few altogether and dramatically reduced the rest.

    Recently learned that kidney stones are also more common in people diagnosed with AS. Then because of immune suppressive drugs massive infections in the kidney are more common and harder to treat. If I had known this I wouldn/t be going on to my 8th month of being treated for UTI's and kidney infections.

      You are right, kidney stones are more common in AS. Very little data on it though-we need more. It’s so tough when immunosuppressives are needed but infection risks are high. I hope you are better soon

    I love the fact that you note every body is different and we need to trust how our body responds to different kinds of goods. I 100% feel more triggers when I eat refined foods with sugar or drink alcohol. I love you as my rheumatologist & also that you KNOW we need to trust our body. If get reacts to refined sugars, then slow down or remove it. It seems simple, but even better it is individual as most of our bodies are too.       Thank you 😊. Yes each individual is different. If you DONT have an autoimmune disease it’s still important to know triggers. But if you DO have one, it’s especially important to know your triggers. Refined foods, sugar, alcohol can be triggers.. and it may be ok sometimes.. but good to be aware!

    Thank you for this comprehensive video. I am a 66yo female just recently diagnosed. I was diagnosed with Lymphocytic colitis 10 years ago. I always complained that I felt like my whole body was inflamed. Over the years I developed Achilles tendonopathy, tore the meniscus in my knee and had small muscle tears in my shoulders. I had no idea that AS could have these effects. I could have cried when a diagnosis was made from the sheer relief that there was an underlying cause for my seemingly unrelated issues.

      Autoimmune diseases can be extremely frustrating. Lymphocytic colitis is tough! But yes, not as big a relation as the other inflammatory colitis to AS-which makes it so much tougher for diagnosis. I’m glad you finally got one! So many treatments out there, I hope they work amazing for you.
