• Hair loss regiment ( 3 layers)

1. anti androgen (dutasteride)


2. growth agonist (minoxidil)

• Big 3: hair loss, prostate growth (and cancer), and low T. Related?


• People with insulin resistance, higher IGF-1, more pronounced metabolic syndrome (pre-diabetes, type 2 diabetes) are more prone to issues


• Metabolic syndrome and sleep apnea are the two leading causes of low total testosterone (but not necessarily causing low free testosterone)


• Free T and DHT matters more for athletic performance and body composition


• Total T matters more for how you feel, and for delivery of androgens to the prostate (growth)


• Several different types of 5 alpha reductase enzyme; finasteride and dutasteride have different effects on each which is why they can have different effects.


• It mostly about the ratio of testosterone plus dht vs estrogen. If the ratio changes you are more likely to have symptoms of high estrogen


• Free T = calculated with total T and SHBG (tightly binding) and albumin (loosely binding)


• Free T should be 2-3x lower than estrogen (in normal, not same units). If 4-5 lower estrogen too high.


• free t of 15 ng/dl; estrogen pg/mil of 45 (45/15=3)


• Free T usually listed as ng/dl (0-50). Can be pg/mil.


• Estrogen usually in pg/mil


• When on dutasteride or finasteride, have to watch the ratio more closely because the lower dht makes you more sensitive to higher estrogen.


• 1-2/x week dose of dutasteride will decrease DHT but increase testosterone and estrogen.


• SHBG. Why does it reduce free t? SHBG is a good thing unless you have a low free T


• As long as free t is 12-15 ng/dl, then the higher the SHBG the better. Helps with stability, metabolism, and delivery to tissues…c...

      Thanks for the suggestion. Why “navigate”? The episodes are prepared for listening from start to finish. You don’t need to listen all in one go. Perhaps I could add clips of particularly interesting segments, when there are good sound bites.

    I sought to see how legit some of the guests recommendations were. Did a google search and first thing I saw was Dr Goldberg in a "Plant Power" t-shirt holding a bunch of rhubarb. I couldn't find any good science to support some of his dietary claims. Makes me wonder if ideology is guiding his protocol rather than science.

    This is great. I have methylene blue and haven't been taking it regularly, but I know I have a pretty acute mitochondria problem and my fatigue has gotten worse. I havent been sure how to use it properly but I am motivated again to do the digging.

    Question for Dr. Day. Have you encountered an onset of AFib after covid vaccine shots. An athletic person, not excessively, was healthy with no heart issues of any kind. Right after the shot at the pharmacy , that person became very ill. During the following 4 month was getting worse to the point she was not able to walk. The vaccine had attacked her immune system and her heart.

      Glad you liked it. This investigation has led me to switch from going for Vitamin D via direct sunshine to spending more time outdoors in the shade or under a hat to get a ton of Near Infrared light. Plus I’ve upgraded my indoor lighting to get NIR inside as well.

      @ it reminded me that years ago I used to have NIR socks and gloves to help keep warm in the winter. I’m not sure if there’s any science behind NIR clothing but they did work and had many anecdotal reports of benefits. I might revisit wearing those again in addition to improving my home lighting. It also now makes sense why I’ve found forest bathing so beneficial. Forest bathing for 20-30 minutes actually feels better than I remember an anti anxiety drug feeling when I last took one 25 years ago! I’m looking forward to more episodes.

      I agree that getting outside is the solution. Stay in the shade (forest bathing!) to avoid excessive UV and get maximum NIR. Or wear a hat (NIR goes right through clothing). I’m also using a wide spectrum lamp to get NIR all day everyday. And I still use my red/NIR lamp in the AM. I always feel better when I get outside (fresh air, full spectrum light with extra NIR, moving my body, distance from electronics)!

      Mineral deficiencies should be addressed. I get regular blood tests to identify deficiencies rather than take a lot of supplements just in case

    this was very helpful. I had afib, had an successful ablation. have been a competitive athlete my whole life....I believe we can really overdue it when it comes to exercise. there has to be a happy medium.

    Sleep apnea leads to physical changes to the heart. The increased number of red blood cells generated by the lack of oxygen require higher pressures to maintain flow and this results in enlarged heart valves.

    Not ALL surgeries come out great & over time alien metal screws & braces cause mis-alignment resulting in dis comfort. I’ve done my best to disassociate from the neuro-pathways yet listen to the message to pull back the demands by slowing or ending activity then allow for many forms of recovery when necessary.. My fear is to have to have a knee replacement down the road. ( left ski edge locked onto a hidden rock -09 fracture tibial plateau lateral)

    Thanks, I enjoyed this interview very much. Sometimes I had to laugh, but some excellent advise for mature athletes. I'm 72 now and very happily, discovered swimming about 10 years ago and pilates more recently as my major sport activities. Diet and stress management are key, too, of course. My competition is with myself and keeping track of other swimmers in my age category. Kudos for a very informative chat :)

    Nice to hear from Seiler from slightly different angle. Thanks for the interview. I disagree with dangers of strength training: based on statistics they are one of the most injury-free training forms. Even high-level strength athletes tend to have just minor strains and overuse injuries, while we speak of hundreds of kilograms of weight on one's shoulders or hands. Sure at times something goes horribly wrong, but these are elite levels lifter truly knocking on their limits. Sure without understanding how to deadlift or squat there is more danger, but these movement patterns are easy to learn (not including snatch or clean) and with little practice one can keep oneself safe. If rep range is something like 5-10 then staying away from failure injury is very unlikely. Running sprints, jumping etc are far more injurious.

      Thanks for the comment. I agree that even older people can slowly adapt to heavier weights as long as the skills are developed as well. It’s the older athlete who tries to do too much too soon (me!) that I worry about. But there is no arguing away the risk of lifting heavy. It’s a risk reward thing.
