Great video. Point of clarification, is the end point to inject LA in the plane through the posterior fascia/border of the QL and "zipper" this from the psoas muscle? So, you have to pop through that fascial plane, correct? thanks.

Question...simce i can see the SCM muscle when doing an interscalene block, and these nerves are intermuscular fascial compartmental, cant i administer it as if im performing an interscalene and the drug will spread? Thank you

Hi Melody. This is my go to anesthesia for above knee amputations. Sometimes with a little sedation. And mostly without leaving catheters due to local constraints. Most of our AKAs have a contraindication to spinal and would be better without GA.

Thank you for the nice video! Used the approach (that I first heard from your Twitter btw) recently in combination with fem. and lat cutaneous nerve blocks for analgesia after thigh amputation - worked beautifully, patient was completely painfree postop :) One thing has been bothering me for a while now. Why is it that with the more proximal sciatic nerve approaches one doesn't have to penetrate the paraneural sheath for an effective block? Unlike the popliteal block, that is.
