I used a glitch to freeze the game but I kept moving so I made it on the ball and since the physics sucks if you do it correctly the ball will kill you from fall damage

The letter is counted as an item because for a split second when you pick it up it appears on your item list, and also there are some bugs that basically let you restart the game in old saves (so it looks like you are starting act 3 but you have already picked up the letter) and in order to progress you have to place the letter down and pick it up again
