OBGYN Discusses: Ovarian Cysts and Pregnancy
Hysteroscopy is neither cute nor adorable - it is barbaric. Puncturing the cervix with a bullet extractor, filling the uterus with water, dilating the cervix with metal rods, before forcing a metal rod thru into the uterus and moving it around. 1 in 3 women suffer extreme pain (7-10/10 on VAS chart) and some are traumatised and suffer PTSD. Ladies, don’t EVER have an office hysteroscopy unless you are sedated or under general anaesthetic. This is the 21st century and they are performing nineteenth century invasive, intimate procedures on women without analgesia or anaesthesia. Don’t be conned into letting them have a go.
Excellent video. A gynae who cares. If only the ones in the UK were like this, instead of the patronising, gaslighting attitude they usually have
I had a total hysterectomy 4 WPO and notice a huge change in my orgasms so far. I had deep uterine contractions before and really notice that. Can orgasms improve over time? Does pelvic floor therapy help?
I really hope obgyns do more to prevent these.
We certainly do care and do what can be done to prevent them!
We need more OBGYN Drs. In our reproductive health, Like you!! 👨🏽⚕️♥️
Thank you for the kind words!
after surgery is it ok to climb stairs? as bed is upstairs?
You would need to talk to your doctor regarding your specific surgery. But if I am talking to my patients about recovery after hysterectomy, yes of course ☺️
I have noticed this pattern that without changing my eating habits much..every cycle there’s constipation during heacy days..wondering why? and if there’s link to fibroids?
If you are noticing this pattern, I would definitely see a GYN to discuss options!
Hi there! can you address in any of your videos about fibroids and the constipation relationship based on the locarion and size of fibroid..and especially during period.
I have 20 days of bleeding and pain also have fibroid but don't know where i am any more
Please find a gynecologist to help! I’m sure there is an option that will work!
I am 4 weeks into my recovery from a robotic hysterectomy. I woke up with very little pain just uncomfortable pinches. Never took anything more than tylenol. But i do have a large scar 6 inches. Because of a large uterus and fibroid. But the little incisions were no problem. Stayed overnight and discharged in the morning because of age (71) and incisions. Used softners 2 days then all was fine. Surgery was 8am done by 12.I was up and walking by 6pm drinking and crackers. Fully awake by 9pm .took catheterout at midnight and i used toilet no problem. . Using a pillow for stomach if sneezing or coughing. I had no nausea or dizziness. Should of had it 10 yrs ago but gyn at the time said a Dc was better.. But it takes a lot to get back to normal, still tired some days.find long car rides still tire me out.
Dr , i cant take hormones due to hx, but thinking of hys/ovary remove due to heavy continous bleed, and hx of lynch. what to do ?and i wear pessary 3xweekly for medsurg floor work.
Yes, usually if someone has been diagnosed Lynch syndrome they should consider surgical removal. Talk with your provider to see if they can do a prolapse surgery at that time!
Please can you create a video about hormone testing and estrogen dominance (I'm trying to find the root cause of my fibroids and have noticed the ratio is off). Appreciate your work, thank you!
I will certainly get to it! It’s a hot topic right now. I have a whole fibroid series if you haven’t seen. Thanks again for the recommendation!
...So if you have to have your ovaries removed due to you had breast cancer and it was triple negative or whatever they call it and it is a generational that this type of stuff runs in the family and you don't want to take a chance of getting ovaries if they have to do a surgery to take all of that out including your ovaries can you still have sex life please let me know
Of course! Just talk to your doctor to see what you can do to improve the symptoms 😊
I had a total hysterectomy (11/6/24), but ovaries were kept. My cervix was removed.
Hope you are feeling well!
I give birth truth cs last year July and I lost my baby too can I still give birth normal 😮
It’s definitely an option! But talk to your doctor to discuss!
What are your thoughts? I'm planning to have a Vba2c. My first baby was via cs , and my 2nd baby was a vbac. My 3rd baby via cs because of the baby heart rate was dropping.... my last cs was 4 years ago... almost 5 years
It’s a shared decision with your doctor. There is risk but it could be an option. Not all hospitals can manage these higher risk deliveries
How do you determine menopause in someone who is suppressing their periods with continuous birth control?
Good question. Would likely need stop the medication to truly tell. But depending on the reason for the medication, age, symptoms… it would need to be a shared decision if it’s worth stoping to get the diagnosis
I had a hysterectomy and left ovary and fallopian tube removed due to endomeitrosis adenomysis one ovary is left my doctor told me that one ovary functions sufficiently enough like one kidney? And I don't need any hrt? Please can you tell me what would be your view?
There is more information now suggesting there is a change in hormone function. One can expect potentially menopause 1-2 years earlier. If menopause symptoms arise, you should definitely seek attention and potentially treatment with HRT.
23 y.o here, im scheduled to get a supracervical hysterectomy but im trying to asvocate for a total hysterectomy
It may be worth making another appointment with your doctor to voice your concerns! If you haven’t already
I know it is dependent on the patient, but what size fibroids generally requires open surgery (myomectomy/hysterectomy) vs laparoscopic?
Or robotic?
It’s often surgeon dependent. A GYN that has more experience dealing with larger fibroids would likely have a better chance of completing the surgery laparoscopic. Often it’s a combination of why type of surgery patient history and safety. Dr Valieva and I don’t have a number which we say we have to open at. But to try and maybe simplify my answer… mass size volleyball or basketball size
From a patient perspective, I am surprised to hear Dr Valieva refer to a Hysteroscopy as a ‘cute’ procedure. Having just endured this in-clinic procedure, there is nothing cute about it. You are at your most vulnerable on the table, the procedure in my experience is very poorly explained beforehand and the Dr barely explains what they are doing as they are performing the procedure. Whilst the camera is uncomfortable, I wouldn’t describe it as painful (though recognise everyone is different). However the grabbers that do the biopsy are very painful and shocking and a pipelle aspiration if needed is just plain horrid!and it feels like 30 seconds lasts forever. You are full of water, feel like you are wetting yourself, there is a very unglamourous bucket under your bottom collecting saline and blood. And you are expected to leap up and get dressed ASAP when the Dr is done with little to no recognition that this was in fact a very invasive procedure for a woman to experience (I know, not as clinically invasive as surgery…). Definitely not a cute procedure.
I’m sorry to hear about your experience. I agree, it seems like more could be done for setting expectations or present options such as being done under anesthesia. However, I think we can give her the benefit of the doubt and not to try and dissect her words. She cares more about the patients experience and comfort more than I have seen from anyone in our profession. By cute I imagine she meant minimally invasive and quick recovery, speaking from a provider perspective.
14 weeks post total abdominal hystorectomy and so over gyns that just reqd textboom and say there is no side effects. Women suffer sometimes a long time If a woman keeps her ovaries great. But you also experience side effects hot flashes dizzyness fatigue mood awings nausea. Nerve damage is always going to happen with a large incision but drs ignore this.. Im suffering severe fatigue and dizzyness as well as dry mouth and nausea and the terrible nerve damage
Im sorry to hear. We definitely need to keep challenging our prior thoughts to take the best care of everyone. There is now good information since recording this to support what you are saying. It’s worth another visit to reevaluate options with your provider!
Mine ended up being unnecessary. I hope my now-former gyn gets sued one day.
I’m sorry to hear this. I always advocate for people to not feel bad about getting a second opinion. I’m hoping this channel can one day be a place where people realize the options they have
Excellent informative videos! Thank you. Would you consider doing a video on using local estrogen? I'm in my early 60's and recently found out during a CT scan for another issue that I have a 7.5 cm uterine fibroid (no symptoms). Being at the late post-menopause stage and using Vagifem estrogen for 4 yrs now; I was curious if this was safe in continue. I was referred to an ObGYN but wait times are 3 - 6 months.
Yes certainly! Hopefully we get a video out soon about the local vaginal treatments! I’m not sure if you have had imaging of the uterus previously. Generally speaking, hard to know if the fibroid has been there for a long time. To see if there is growth, I would often order an ultrasound in 3-6 months.
@@newschoolobgyn No previous image, it was found during a CT scan for kidney stones. Healthcare is a waiting game in Canada, unless it's an urgent issue. Meanwhile, your videos are a great source of knowledge and information, even though each person's situation is unique to them, the information is one step closer to understanding how the body works. Thank you

How woman can diagnose herself regarding perimenopause stage if she is on combined pill ? Is fsh blood test helpful or there is no point as results will be affected by combined pill ?
Good question! I think I need to make a video on it. There is no real good perimenopause pause blood test, mostly because the FSH can be so varied in a given day or hour. Also yes taking hormones could affect the diagnosis. Often diagnosed via the symptoms and timing
had my 3rd ultrasound today, and I'm 11 weeks and 3 days. My second ultrasound shows that the size of the cyst on my right side is 3x3. Now, it went up to 5cm, and I'm afraid that it could lead to miscarriage (hopefully not). The remarks shows that it's like a simple cyst and can't understand the meaning of it since my OB was not there to explain it to me.
A simple cyst is not associated with miscarriage 😊
I undergo total abdominal hysterectomy last Sept. Now, i’m having an issue with urinary incontenence 😢😢
Go to your doctor to get checked out! There may be another cause or simple solution
Hello. Im 48 year old in peri. I have 4 fibroids, the biggest is almost 8cm, then 4cm、and about 2-3 cm. According to my ob gyne, i have all three kinds of fibroids. Symptoms still manageable with a little discomfort. Should i take lzrtial hysterectomy or follow my docs advice to take gnRH? Thank you.
As always, I cannot give personal medical advice. However treatment of fibroids is often based on symptoms. I am assuming you mean GnRH agonist which is Lupron. I personally rarely use this medication because of the side effects. But I do think you should have good options still whether surgery or not 😊
I’m 47 have small to medium fibroids 3cm don’t want to take surgery,
You definitely have options! If there are no symptoms, you may not need any treatment 😊

Have just been diagnosed with Cervical Cancer, Adenocarinoma In Situ, don't know the stage yet? I am 70, obivously I will be having a hysterectomy; removing the cervix, uterus, fallopian tupes, ovaries, not sure about lymph nodes? Depends on the margins (can you explain margins?) I have an appointment with the Gynelogic Oncologist on Wed. Nov. 6th. Actually I am seeing 2 Gynecologists, they are a team so I am wondering about Radiation afterward? I assume yes? What about the after effects of Hysterectomy, are you incontinent, swelling of the legs if the lymph nodes are taken? Other side affects? I live in Toronto, Ontario Canada.
I’m glad you caught it and It looks like you have a good team taking care of you! I’ll defer the specific questions to them. Treatment is often determined by the stage, which it appears will be figured soon
What is In-situ Adenocarcinoma (AIS)? That is cancer correct? Thx Jane
It’s technically precancerous, but needs further work up and treatment
I'm 60, stopped having periods 20yrs ago, but still having hot flushes & sweats, most annoying. How long is it going to last ?
If you haven’t already, I think you should see a specialist in the area to determine if this is menopause transition related or not. This timeframe is much longer than normal
What are your thoughts on the UFE procedure?
I think it can be good for some fibroids and avoid an abdominal surgery. But it has downsides. Maybe higher pain than surgery, higher re-operation rates, not for people who desire pregnancy. I plan to talk more about it in a video soon!
Do you know when the HPV vaccine was first developed; the year? starting with the 1st one and the latest one? Thx
First Available 2006 in the USA. I don’t have the date of each off hand
What is the success rate of Leep? The purpose of Leep is to remove pre-cancerous cells and to send the samples for testing? Thx
There isn’t a good published success rate as there can be so many variables. It often is very good at removing the abnormal cells but it doesn’t completely remove the HPV. So often we need to continue to watch closely after. But if you are getting all the tests done and follow up there is extremely low chance of it developing further towards cancer.
Did you do a session on LEEP? Very interested. Thx
I did! Check it out and hope it helps!
Thanks I checked it out. Are you in the US?
Gas!! Its been the worst and I wish you talked more about it
Keep moving and get back to a normal diet. It’s definitely a tough part of it. But hopefully better soon!
Hi i have a corpus cyst Im also 7 weeks pregnant i went to the doctor last night for abodminal pain and fever cause i also have an infection, based on my research the ive been doing the cyst can cause bleeding and lower back pain similar to the symtopms of a miscarriage And the pain im feeling is coming from the right side which is where the cyst is at
Corpus luteum cyst is needed for the pregnancy until the placenta forms. Usually they wouldn’t cause pain and shouldn’t develop into a hemorrhagic cyst. Pain can be hard to pinpoint in the pelvis
@newschoolobgyn so I'm assuming it's more leaning to a miscarriage?
@@sarahnorman2196 based on this, I wouldnt think so!
I have dermoid both right and left ..but my left ovary was removed and my right has 6cm dermoid ..I'm 31 no kids yet .How can I save my right ovary?
Choose an ovarian cystectomy!
I have HORRIBLE hot flashes, night sweats, fatigue, brain fog, frozen shoulder pain, and some days where I feel down. I am a 36 years young female. I had to have a hysterectomy in February of 2022 due to uterine cancer. My uterus, fallopian tubes and cervix were removed. Because of my age, my doctor opted to keep my ovaries in for hormone regulation. I feel for anyone going through this sometimes excruciating process. Great video. I was told by my doctor that at the 5 year mark post surgery that I was going to go through full-blown menopause. She also said that until then, I will experience perimenopause. Well I went onto TH-cam today for some more information about it because I’ve got it bad. I just want to encourage anyone who is going through this to talk to holistic doctors and get in the right supplements to combat these symptoms. I’ve also been researching hormone replacement therapy.
There are treatment options for you! I would ask your doctor sooner than later!
Is it better to get a cuff than abdominal laproscopic
I’m sorry, I don’t understand the question
Abdominal laprascopic is two different things Abdominal is a large c section cut Laprascopic is keyhole ie small holes Having a cuff means the cervix is removed and stitched up
Can you give your opinion on the sling mesh?
I think you are referring to the procedure for stress urinary incontinence? I wouldn’t do this unless someone was proven to have urinary incontinence. Hope this helps
Hi there 👋🏻I gave birth via c-section in August of this year, but unfortunately I lost my baby due to a heart problem and now I’m planning on getting pregnant in a year and I really desire a vaginal birth my experience with c-section was really traumatizing especially since my doctor didn’t really prioritize/listen to what I wanted. My c-section was due to a narrow pelvis🤔 now my question is Can it be possible for me to have a VBAC after a year of a c-section?
I’m very sorry for your loss, our hearts go out to you! It’s always a big conversation with who ever your doctor will be. There are serious risks but they are rare. Some people prefer a planned CS because they want to avoid that risk or labor not progressing and going through a tough experience again. Some people really prefer to try for the vaginal birth again. But you should have options. Some doctors really prefer 1.5 years between deliveries if someone had a CS before. Hope this helps
5 weeks 4 days pregnant. Had PV bleed. USG done and haemorrhagic left ovarian cyst with intrauterine gestational sac no yolk sac or fetal pole. Still having minor brown spotting? Could this PV bleed and spotting be from ovarian cyst?
Usually ovarian cysts shouldn’t cause bleeding. Usual at this time it would be implantation/sub chorionic for the source.
Hi doc. I have 6.4 dermoid cyst in my right is there any way my cyst will be remove besides removing my ovary ?
Yes of course! Would be called ovarian cystectomy.
@@newschoolobgyn but why my OBGYN did not tell me about this option instead she told that I might lose my right ovary too because of my dermoid..I lost my left ovary because of my 13 cm cyst .Had surgery 2 weeks ago
@@mmmxmm-p There can be a number of factors that go into the decision. The biggest one would be age and type of cyst.
