Why Do Patients with Myasthenia Gravis Experience Choking
Patients with myasthenia gravis (MG) may experience choking or difficulty swallowing (dysphagia) due to weakness in the muscles that control swallowing. MG is an autoimmune disorder that affects the communication between nerves and muscles, particularly at the neuromuscular junction, where nerve signals trigger muscle contractions.
Here's why patients with MG may choke:
1. **Oropharyngeal Muscle Weakness**: MG often affects the muscles involved in the initial stages of swallowing, including the tongue, soft palate, and pharyngeal muscles. Weakness in these muscles can impair the ability to control food and liquids in the mouth, making it difficult to form a cohesive bolus (a mass of chewed food) and push it toward the throat.
2. **Delayed Swallowing Reflex**: The weakness of the pharyngeal muscles can delay the initiation of the swallowing reflex, increasing the risk of food or liquid entering the airway (aspiration) instead of being directed down the esophagus.
3. **Incomplete Airway Protection**: Normally, during swallowing, the larynx (voice box) elevates, and the epiglottis covers the airway to prevent food or liquid from entering the lungs. In MG, the weakness of the muscles that perform these actions can result in incomplete closure, increasing the risk of aspiration and choking.
4. **Fatigue**: Swallowing involves a coordinated and timely sequence of muscle contractions. In MG, muscle weakness and fatigue can worsen with repeated use, leading to progressive difficulty with eating and drinking during a meal. This can increase the risk of choking, especially if the patient becomes fatigued during a meal.
5. **Bulbar Symptoms**: When MG affects the muscles involved in speech and swallowing (referred to as bulbar muscles), patients may experience dysarthria (difficulty speaking) and dysphagia, which contribute to choking hazards.
### Management and Prevention
1. **Dietary Modifications**: Patients may need to switch to softer foods or thickened liquids to reduce the risk of choking. This helps to ensure that the food or liquid moves more slowly and can be better controlled during swallowing.
2. **Swallowing Therapy**: Speech-language pathologists can work with patients to strengthen swallowing muscles and teach techniques to reduce the risk of aspiration, such as chin-tuck swallowing or alternating between sips of liquid and bites of food.
3. **Medication Management**: Adjusting MG medications, such as anticholinesterase agents (like pyridostigmine), can help improve muscle strength and reduce symptoms. It's important to take medications at appropriate times, especially before meals, to maximize muscle function during eating.
4. **Feeding Strategies**: Patients may be advised to eat smaller, more frequent meals to avoid fatigue and use specific postures or swallowing maneuvers to facilitate safer swallowing.
5. **Monitoring and Support**: Regular monitoring by healthcare professionals, including neurologists and speech-language pathologists, can help manage symptoms and adjust treatments as needed.
6. **Emergency Preparedness**: Caregivers and family members should be educated on recognizing signs of choking and how to respond, including performing the Heimlich maneuver if necessary.
Effective management of swallowing difficulties in MG is crucial to prevent choking, aspiration pneumonia, and other complications. It often involves a comprehensive approach tailored to the individual's needs and the severity of their symptoms.

      ​@@jenniferdailey2540 Yes, it is helpful. I have similar symptoms but haven't been diagnosed. They did draw blood but no results yet. I'm frustrated that it takes so long to figure out what is wrong with me, but I've read that delays are common. All the best to you & I hope treatment is helping you!

      Definitely agree I try to stay out the heat and if I’m out I make sure to be well hydrated and I wear a sun hat to shade me from the heat. Thanks for your reply ❤️

      Thanks for your reply. I’m a diabetic and I had to learn how to be a diabetic I had changed my diet by eating smaller portions, and I exercised for 30 minutes every day while having a waist trainer on and I lost 30 pounds I was determined to get that weight off me because I had never been overweight before and it was depressing to see myself like that. It was the side effects of prednisone that caused the weight gain.

    I work. I found something that accommodates my myasthenia gravis. It's not easy, but it's possible. Of course, before I was stable, I could not work. I was too sick. Everyone is different. We know our bodies and do what we can 😊

      @@imgm94 I absolutely agree about everyone being different. I have been on disability for sometime now since being diagnosed with myasthenia Gravis . I used to be a CNA and enjoyed working and actually miss the work I used to do. Unfortunately I suppose it’s impossible for me to go back working as a nursing assistant. It’s been rough throughout these 9 years because I have generalized MG and my symptoms recently returned last December after being in remission for 4 years. I desperately want to do something part time but I’m afraid it will trigger my symptoms.

    Do you have any clue about the mechanism involved in execution of the symptoms? I have general body MG. I understand that the acetylcholine receptors are blocked by antibodies And I understand that the whole process is in microseconds. What I don't understand is how stress or physical activity leads to eye lid droop or flareups. Which mechanism goes And does this. Is it the defence mechanism that has already identified those points and goes And attacks or maybe there is chemical reaction going on in those parts of the body that attract antibodies more.
