    Could I ask why Triptans are not recommended for hemiplegic migraines?

      People with hemiplegic and brainstem migraine are at a significantly higher risk of stroke. But it's a "talk to your specialist" situation - some neurologists are more open to it.

    Going to give this a ago. I deal with it 24/7 and can no longer walk. It's taken my ability to live my life. Need a wheelchair to get around. I live in a dark room with hardly any sound or light. I hope it helps. I don't want to live like this for much longer.

    I cannot wrap my head around the fact that you need to pay for your medications in US 👀👀👀 In Europe, we get them free of charge 😅 And I think it’s also cheaper in Europe - the current price for AJOVY is 383 EUR.

      Agreed. Am in the U.K and on my second year (Free) of Ajove but we in the U.K have to jump through “Hoops” to meet the strict criteria to be prescribed and continue the prescription. Most have to have an enforced 3 month break after the year before we start the questioning of the criteria again!😡 (Probably down to funding) - luckily I only had to have a 3 week break.

    what about patients who were well before a martial artist, soccer player, swimmer and in the army so impeccable balance and tolerance, but post head injury pain/migrains and sensitity all the time, disabled and can't hardly leave the house.

    I have constant headaches, noise light sensitivity, sensitivity to scents, trouble balancing and all kinds of problems after a head injury. I never had problems or headaches before my head injury, I also have oscillopsia and visual vertigo, extreeme fatigue. But tests show the vestibular system is norma;l my neuro said it's hypersensitivities post head injury and a study from cambridge came out this year and corroborated that saying post concussion hyperconnected pathways can cause the myriad of symptoms we suffer. The question is how on earth do you treat it as I've had vision and vestibular therapy with a slight improvement to how dizzy I get but not massive. It also feels like the dizziness and imbalance can fluctuate which is confusing, I have heard autonomic issues can cause dizziness and I have alot of issues like I don't sweat anymore and my stomach doesn't seem to work properly. Could it be autonomic causing the dizziness and imbalance. It's been 5 years I'm completely disabled, can't work and rarely leave the house because it causes so much more suffering.

    You cover alot of ground there but what about post head injury headaches? I have a constant aching head and almost constant migraine, how do you abort it when it never goes away? A patient who never had headaches in life and post head injury is disabled by them and many other symptoms going on 5 years now.

      My advise. Be patient. Some are Slow Responders and need more than the 3 month trial here in the U.K we are given. I was one of them but once Ajove started to build into my system I started to see improving results. I was in an awful state for many years before the correct diagnoses. I can not begin to tell you the amount of Vomiting / Pain Days I had and missed dinners and outings before Ajove. I had tried 4 oral preventatives before 2 rounds of Botox and then Ajove. Am now on my 2nd year on Ajove. Is NOT a miracle drug as there is no complete cure but I am living my Best Life now with a reassurance I can carry out my daily plans. ❤Good Luck❤️

    Thanks for this video. A rare but possible hereditary cause of hemiplegic migraine (especially FHM) is a genetic cerebral small vessel disease called CADASIL, which is caused by one a Notch3 mutation on chromosome 19. I was diagnosed in 2006 and had regular, severe hemiplegic migraines several times a year for 23 years. From predrome to prodrome, each could last for up to two weeks. On one occasion, parasthesia in my right calf muscle did not reverse for about seven weeks. On a few occasions, I had dual parasthesia within 24 hours. Didn’t always get head pain but, most times I did and it was horrendous.

    The low dose almost killed my sister. Tachycardia, weight gain, hair loss, not to mention the migraines were the same. Oh fatigue and inability to sleep. Plus the long term effects of suppressing calcitonin is a dangerous path

    My doctor won’t prescribe Triptans due to having history of asthma even though it was over 10 years ago. They’ve put me on anti depressants instead for migraines but doing my research I’m convinced the anti depressants will be less beneficial compared to triptans

    My doctor gave me a sample of Emgality. I'm still reading and listening about it. Thank you for sharing your experience. Several months ago I did injection of Ajovy. I like it. I took a break from a migraine more than one month. There were about 6 migraines during this period. Some of it gone itself.

    I'm starting Ajovy tomorrow so fingers crossed. I was on botox and after intitial success it stopped working which was heartbreaking so I am hoping this will give me back some quality of life.

    So I've been dealing with vestibular migraines for a decade. Only I didn't find out they were vestibular migraines until about three months ago. It's gotten so bad to the point that I have blackouts, vision loss, and vertigo. I also survived two massive heart attacks this year and I'm only 33 and so many medications I am denied as I am at major heart risk. I'm at the point where I don't want to live anymore and am so broken and depressed. I can't even leave the house let alone my bed. I don't want to die but at this point, I don't want to feel anymore pain

It might also be time to see a new neurologist. Someone that specialises in migraine and headache disorders. If there are none in your area, travelling for care can often be worthwhile.       I feel the same way, I'm so tired of pain and all the other symtpms after a head injury nothing has helped all the meds have made me worse and some like botox seem outright toxic. I can't take all the meds I had such awful side effects to them as can happen after a head injury as the body doesn't metabolise drugs the same way and you can have a permeable blood brain barrier, but all they wanted to do was trhow awful drugs at me. I was scared of anything they wanted to do afte the drugs made me so much worse.

    I used to get migraines about 15 hours after exposure to diesel fumes. Since I avoid that situation I have not had them for quite a while. We also changed location. I'd stand in Grenfell Street (Adelaide) waiting for a bus and then next morning around 5 or so I had these enormous headaches and vomited. I reckon it must be the dioxins in diesel fumes. I was a smoker but that never made a difference. I never went to a COVID testing site for fear of diesel vehicles running their engines.

    Love love love that he discusses addressing central sensitisation as “changing how sensitive the brain stem is” rather than “how the patient perceives symptoms.” Shows an incredible awareness of the power of language in discussing these things and the importance of minimising stigma and blame on the patient. It’s not a matter of our cognitive interpretation of the pain being wrong - it’s the central nervous system rewiring itself in a misguided attempt to protect itself. Discussing it as such is so important for reducing the tendency we have to question our own perception of our pain.

      Thanks Tammy, Yes, I spend many hours each week with clients who have had to defend the assumption that if there is 'nothing on the scan' then it must be a mental health disorder. It shows a fundamental misunderstanding about how pain (or other symptoms), anxiety and mood interact. You are not meant to feel 'good' when you have pain. You are meant to think about it, and try to avoid it happening again. Its adaptive - for acute symptoms. It's maladaptive for chronic. So much by way of anxiety and depression symptoms are being driven from the 'bottom-up' rather than a true behavioural or mental heath disorder. In many cases it is not a 'comorbidity' - you have migraine AND a mental health disorder, but they are different, and entirely predictable manifestations of the same process. Our medical system struggles with 'functional' or 'primary disorders' - nothing to 'fix', no bugs to kill. So the next 'silo' that fits the symptom list is mental health. The way we view this relationship, and the judgements that are inherently linked to it needs to change. Thank you again for your feedback, Regards Roger@MHC

    So interesting. What's the clinic book and books mentioned? I definitely am hearing new things here re eliminating. I have the dizzy cook and heal your headache books and have done both of those. I did find out I can't have oj or lemon but confused on the rest.
