Your hanger is bent.
Turns out that the cassette was too small so the tension pulley was too far from the cog. Using 11-34 instead of 11-28 fixed the issue even through the RD was supposed to work with a 11-28 cassette
@@nphilou there is a Mode converter, you have to remove it for 11-28
You can also adjust the angle of your derailuer so the pulley is closer to the cassette.

@@JohnSmith-xi9hm It was the closest I could get to cassette unfortunately. I suspect a slight design flaw to this frame
