Seems like straight edges don’t do well. Try rounding all sides!

You have 2 curved edges and a 3rd being straight. As the curved sides bulge out it's putting stress on the straight edge that folds in like the second attempt. If you made it curved it should inflate like a samosa :)

That shape of bulged out triangle is perfect. Make four of them, weld them as a tetrahaedron and try again. You will get a perfect sphere.
