Your track should be flat all the way around if not your engine will most likely derail and make sure your rails are clean of dust and ghost terds too.

Well, for one thing you should have a straight track on both sides of the girder then that should work.

I had the same issue i just swap the lead car truck with a microtrains truck with coupler and it works great

I had this issue as well. Your radius on your turns is too tight. If the coupler on the loco is on the shorter side it will derail the car closest to it due to it not having enough space to travel. I actually just recently made my layout slightly larger to fix this exact issue. You can also fix it by installing a long shank coupler on your locomotive, but I'm not the biggest fan of how that looked so I decided to bight the bullet and make the curves on my layout a wider radius.
