How long voriconazole 200 bd to be continued in pts of immunodeficiency due to Rituximab for NH lymphoma at 3 monthly dose, the immuno therapy with bendamustine and Rituximab having been completed a year back. Pt is 66 year male presenting with chronic nonproductive cough for last 2 months. The diagnosis of invasive asperhilosis was made after blood test was positive for Galactomannan.
@@kaydee5639 We cannot recommend anything based on a single question because the case you are sharing needs detailed assesment of multiple factors to decide the dose and duration of Voriconazole .... What we can tell is Usually Voroconazole is given for 6-12 weeks and can be extended upto 24 weeks in certain cases as well depending upon status of Immune Correction/symptoms resolution/serial BaL galactomanan levels. In certain cases the therapy maybe even prolonged

Sir ye pata kaise chalega ki inhaler me abhi Bach Raha h ya khatam ho gya? Kya ye point ek bar inhale karne pe change hota h ya kuchh times inhale lene ke bad??
@@RupamSinha-uv1iq 20 points kam hone pe niche ata hai. Lekin agar dhyaan se dekhege to movement hoti hai with every dose. Khatam ho jaega tab 0 aa jata hai. Sabse best tareeka hai ki apni doses yaad rakhein ki kitna use kiya hai because inhalers me doses fixed hoti hai jaise 120 doses or 200 doses

TB gold test ka koi use nahi hota TB ko diagnose karne me . Jo main test karne hote hai vo shayad abhi tak aapke huye hi nahi hai ..... Fnac or Biopsy from l.node for Gene expert ultra & Afb rapid cultures..
Dear Sir, it is said that in ILD/ Lung Fibrosis lung lost it's elasticity and become hard. Under this circumstances are these exercises may be benifical to increase lung capacity or not?
@@sukhendu1974 yes these are certainly helpful in improving lung capacity because the actual lung capacity is determined by the functionality of lungs as well as the muscular strength of chest wall and surrounding muscles .... muscular strength can be improved significantly with rehabilitation.

@@Supriyameshram97 no. Ulti hone ka reason pata karna chahiye. TB medicines pe ulti sabse common Liver pe swelling ki vajah se aati hai. Liver ka test (LfT) blood test karvana chahiye

@@antimpunya Mantoux test positive hone se na TB diagnose hota hai aur Na hi iske Negative hone se ye kah sakte hai ki TB nahi hai. Isiliye thik tareeke se test karvaiye ki sach me bimari hai kaunsi

Sir if x ray and ct scan suggest TB but sputum report 2 times negative then what should i do? Should i start anti tuberculosis medicine as per clinical and radiological features
@@S.B-o8f ideally not possible or there is some alternate diagnosis. We dont rely on sputum tests only if clinical suspicion of Tb is there. We prefer doing a Bronchoscopy test and send the lavage (sample suctioned during bronchoscopy) for various kinds of TB tests.

Sir mujhe pulmonary tb hua tha 2013 me after delivery mera 6to 8 monthe treatment chala tha uske baad sab normal ho gaya but ab mujhe 6 month se zyada saan lene me problem hone lagi hai to kya mujhe pulmonary fibrosis ki paroblem start ho sakti hai kya
@@KanchanHansHans Chest xray karvaiye. Post TB ke symptoms aise nahi aate 11 saal baad. Aapke najdik kisi Lungs specialist doctor se consult kijiye xray karva ke aur Lungs ki spirometry test bhi karvaiye. Pata lag jaega saans kyu ful raha hai aur ilaaj ho jaega
@@Lungsukh_by_Dr.Ankush sir ji maine x- ray karawaya hai usme doc ne bataya ki tb ke chans to nahi hai but halka sa nushan hai jo fibrosis ka bataya tha is baar maine medicin cheng karwayi but mujhe usse aaram nahi mil raha hai last six month month se dawai le rahi hu mai pahle 1 time leti thi usse aaram nahi mila to 2 time kar di but itna khas aaram nahi hai starting me inhelr or nibulizer bhi kiya tha coz dry cough tha cogh to nahi hai mujhe dawai se thik ho gai thi maine dispencery me dikhaya tha aab to thoda sa bhi heavy activities karti hu kitchen ke kaam me to jaldi thak jati hu aisa lagta hai saans sir rukti hai normal karti hu khud ko thoda

In my pleural fluid cytology Ada level was 36 and lymphocytes was 96%.nd no malignant cells are seen..and after cect moderate tuberculous pleural effusion was detected...nd rest everything was normal.Dr diagnosed me with tb pleural effusion....i was on att from 16-08-2024.....i had only 2 symptoms chest pain and fever sometimes...now they are gone...but fluid absorb nhi ho paa rha h..hrr baar ek rib increase hi hota h...abi waise 1.5 mnth hi hua h......att induced hepatitis b ho gya tha dr ne HZR bndh kr diye the... levofloxaxin start kiya tha fr...ab fr se H chla di h ..but rifampicin nhi start ki h......abi dr ne mera hrct krwaya usmai.... bronchiectasis, pneumonia or pre nd para tracheal,subcranial nd mediastinal lymph nodes without calcification nd necrosis dikha rhe h....sirf report maj pr...ct 2.3 baar dekh li h sir ne....sir ko kuch ni dikha....sir ne ct review b bheja but unhone vaise b likh k wapis kr diya .my chest specialist said he is not satisfied with written report nd mera cbc b krwaya usmae lymphocytes 13 or neutrophils 82 the....muje ye janna h ki kya mai misdiagnose hui ho ...muje or koi problem to nhi h ...sir keh rhe h ki 1 week medicine kha k dekho ..nhi to fr cect krwayenge.....ye lymphnodes kiu aaye muje ye janna h sir..muje kya koi or test krwane chahiye....maine suna h lymphoma se b ye sb kuch hota h ...ap btaye hm bahut preshan h

Copd chronic bronchitis since 9-10 years of age. Taking montelukast and levocetrizine tablets and diethylcarbamazine citrate + chlorpheniramine tablets. It makes me very sleepy. Any alternatives?

Hello sir My father is suffering from lungs tb and the doctor has suggested him medicines that is BEDAQUILINE 100 MG , LVX 1000 MG, CAP CS 750 MG , LND 600 MG , CAP CFR 100 MG . And said to take this for only 4 days for now and my father is feeling suffocation and unconsciousness after taking this for the first time.. so i am asking is these medicines are good for health to continue for 4 days or it can cause any big problem 😢😢😢 plzz reply sir
@@Kaizan203 cannot comment anything. Because the medicines you mentioned are given for Drug resistant case. I dont know why they are prescribed and why only for 4 days .... Kindly consult with your doctor only

@@CookingWithFaridaButt jis organ ki tb vahan ka sample chahiye. Aur koi tareeka nahi hota. Baaki sab tests sirf andaaja batate hai means guess work .... Confirm diagnosis ke liye Balgam, pus, lymph node ki biopsy etc
