    I vividly remember that Friday afternoon of the crash. It was a bright warm sunny day, much like September 11, 2001 weather wise. Living in Chicago and in high school there I had a classmate who was killed in that awful crash. It was really the first time I experienced someone my own age dying. Something I’ll never forget.

    strongly disagree with the description of the DC-10 has having an exemplary record: are you kidding? the DC-10 was a donkey. built to sell, not to fly. The actual aircraft that crashed, featured in a publicity photo for the DC-10, when it was new, at the Long Beach factory: it was lined up with nine other DC-10s and they represented the first ten off the production line. Seven of them crashed.

    Hi, I was there. My story and perspective may be significant or not, but here's my two cents. Working at AA since 1973 as a Fleet Service Clerk, I worked in Cabin Service, doing thorough cleaning of the cabins or interiors of the overnight aircraft from 2130 to 0530 hrs. I was a worker and sometimes a Crew Chief or Acting Supervisor. Part of maybe 25 or 30 people to get the planes sparkling clean and ready to originate in the morning. In May of 1979, we were very busy because the flights were all full and as we said then, the bellies were going out 10/10ths full. The main reason for this is that UA (United Airlines) was on strike and mostly AA and Delta had to pick up the slack. Passengers from United were flooding the other carriers. May 25th I was out at Woodfield getting my haircut and shopping for pants at The Gap. there was a radio station playing on the speaker at The Gap and I heard the report that "an American Airlines DC10 has crashed at O'Hare". As soon as I went out to my car in the parking lot, I could see as plain as day, the towering black smoke from the crash site, and that was about 12 miles away. It was difficult getting home in Des Plaines. I went to work that night and it was such an incredible horrible scene. First responders were bringing in human remains the size of a lunch bag. possibly a hand or a head. They devoted the whole north half of the hangar and the north ramp to the forensics team. That shot of the engine sitting next to the runway, I clearly remember staring at that, as it was located directly across from our parking lot, maybe 100 feet away. It stayed there for days. The mood of our workers was very somber and we were all in sort of a state of shock. They worked on identifying remains for about six months. There were seven refrigerated trucks holding the HR. At the end, I recall that 49 bodies could not be identified and those remains were flown out to California for a mass burial. That whole time period, we had to get used to the whole hangar smelling of formaldehyde. My facts might not all be correct but that's how I remember it. My dad was a Chicago Policeman then and he was there then too. He had some experience at that as he was at the scene after another crash at Midway some years earlier. I think that was a Delta 737. Thanks. I don't necessarily believe in the ghost stories but maybe!

      Amazing, thank you very much for your comments!! That was an incredibly difficult for the entire airline industry, as is all aviation tragedies, especially those of you who were in close proximity to the tragedy, as well as the friends, families, and loved ones of all those who perished. A side note... I was just graduating from high school when the crash occurred. I remember this well because my Dad was a captain on the DC-10 for Western Airlines, and my brother was a probationary mechanic for Western at the time. Soon after, the FAA grounded the entire DC-10 fleet. Western suffered financially from the grounding, and the flight crews took a huge financial hit. Long story short, we had to sell our house, and my brother was let go, while our family prepared for the inevitable. Thankfully, out of the blue, Delta bought Western. Many crews were furloughed and eventually were let go, including my brother. The point being, the crash of American 191 reverberated across the entire industry... again, thank you for your very informative comments. I enjoyed reading them, Dan (TTHL) BTW, my brother went on to have a long career at United as a mechanic, and my Dad retired on the DC-10 at Delta.

I have a son-in-law who is a pilot and trainer for United. My nephew and his wife still work at AA. You mentioned the grounding of the entire DC-10 fleet. I remember when they went back into service because I was on one of the first flights. It was February 13th 1980. ORD to LAX. I was flying out to see Pink Floyd perform The Wall. I remember hearing the news as I got on the plane that actor David janssen died. I remember being nervous as hell and keeping my eyes glued on the pylons. The winds were rough over the mountains and the pilot had to keep throttling the engines and it made me even more nervous. It's amazing that DC-10s kept on flying for over 30 years, considering it had a horrible safety record. Things sure have changed in the industry over the years. I got married in 2019 and we honeymooned in Hawaii. The aircraft seem like they're built better these days but OMG I felt like a sardine for 9 hours. Glad to see the upgrades happening to our airports. O'Hare is becoming just beautiful! Honolulu looks the same as it did 40 years ago. Anyway, I'm rambling now so nice to hear from you.     Im so confused. Lena was the oldest right? She was the one of the guests yet she had defense wounds as if she tried to fight back but its confusing me, how would she of done that? Wouldnt the kids in the other room heard her scream? I know the parents were supposedly killed first. This case confuses me it just seems odd.

    The sex offender registry is much more harmful than helpful. Dozens of studies have shown that registering kids who have committed sexual offenses with minors does not reduce sexual, violent, or nonviolent reoffending, or deter first-time sex crimes. Instead, it creates substantial barriers for kids, increases their own risk of harm, and limits their success as adults.

    She should be honored more than she was. Even the moniker of Black Dhalia did her a bit of disservice and the sensational portrayal changed the narrative to Hollywood gossip. Such a sad case. I never realized that this case was what began the sex offender registry. Thanks for sharing.

    Sex offender registration has been expanded so much that even a minor crime can be grounds for registration. Urination in public is one such crime because children may be present. In many states, there is no way to get off the registry, and the restrictions are draconian.

      Also to add a bit to the story. Actress Lindsay Wagner who played the bionic woman on TV was supposed to be on American 191. She said she had an awful feeling and she refused to board the airplane. People around her thought she was crazy but whatever hunch she had saved her life. They asked her over and over why didn't you get on board and she said that she was spooked and would not get on the airplane

    I was booked on Eastern airlines from Albany to O'Hare, then to continue on the afternoon flight American 191 to LAX that day. May 25th, 1979. My parents and brother thought that I had been killed. The crazy thing was that I changed my mind right there in front of the Schenectady Holiday Inn and had the cabbie take me to our GE office instead of the airport. I took on an assignment in Venezuela to leave the very next day. I went to Manhattan to get my entry visa stamp in my passport, got a room at the Lexington Hotel, booked flights from JFK to Miami to Caracas to Maracaibo. After a terribly stressful week of flying Albany to JFK and back 3 times that week, my body shut down. I napped while AA191 crashed and slept until evening. I awoke and immediately called my mom and dad in LA to tell them that I postponed my stay with them until 2 more months. They were absolutely wrecked, crying, sobbing, making no sense. My brother barely blurted out ''You're dead, your plane crashed.'' What???? While they were on the phone, I turned the TV on and every channel had the story of the crash on. They called me 3 more times that night just to make sure their minds weren't playing tricks on them. All 3 of them have passed away now. I was always forbidden to bring up the subject of the crash of AA191 in front of them. Rest in peace All involved in the tragedy. Rest in Peace Captain Walter Lux who filled in for the scheduled pilot who had a home emergency to deal with. Rest in Peace Flight Attendant Nancy Sullivan who gave me a hard time with a poker face on a previous flight from LA to Chicago. She perished on AA191. One Flight Attendant had been terribly delayed by some kind of traffic fiasco and boarded AA191 at the very last minute. Her replacement Flight Attendant was already belted in on her jump seat and asked to continue to go to LA as she told her mom she just got a last minute chance to visit. The assigned Flight Attendant left the plane and went to the AA Employee's Lounge where shortly after she found out that her plane just crashed. This heavily affected her. She stayed on with AA as an EPT trainer ground job to recertify AA Flight crews in Emergency Procedures. She finally started flying again and spent almost 2 more decades with AA as a Flight Attendant before retiring. I attended her retirement party in the early 2000's. I would have died at age 26 had I not changed my mind while awaiting the cab to the airport.

    Too bad that these ghosts didn't haunt the executives at McDonnell Douglas and American Airlines, whose poor decisions led to this needless disaster. I was 14 when this crash happened but here in 2024, almost 45 years later, I'm one of the "silent victims" ; people who were forever traumatized by the incident. My father was a licensed Psychologist who served in the military but this disaster taught me, that people can suffer Post Traumatic Stress Disorder even if they never served in combat. Incidents like this crash, can cause similar emotional torment. There are no words to describe what the residents of that trailer park were feeling. I know that for a lot of years, the victims families fought to get a memorial erected but were met with resistance. Personally, I felt that that open field where the plane crashed, should have been the memorial site but the city of chicago had plans to pave a new roadway over the area. I wonder if strange things will start happening on that portion of the highway. That was an area where people died and it should have been kept "sacred" and undisturbed. I'm interested to see what future hauntings, or strange events happen.

      It wasn't the fault of McDonnel Douglas. It was the fault of American Airlines due to improper maintenence procedures when replacing the engine back on the wing pylon mount.

      ​@@TTHLposted but if you read the other reports from the NTSB, McDonnell Douglas was cited for not having an adequate amount of backup systems, that would have prevented the crash. For starters, there was only one stick shaker warning system which alerts pilots to an impending stall. Only the captain's control panel had one and that was knocked out when the engine separated. A second stall warning could have been installed but that was an OPTIONAL feature and American chose not to have it installed. After the crash, backup warning systems were MANDATED, as well as backup systems to prevent slats from retracting and creating an asymmetric lift situation. If you Google 'American Airlines Flight 191', the reports are available. Improper maintenance was the final contributor, but the rushed, cost cutting design of the airplane was the beginning of the tragedy.

    For years no one could understand why the crew "ignored" the chime announcing that the autopilot disengaged. The chime was clearly on the tape but it might not have been for all the good it did. Until one day it was spotted that the microphone which recorded the chime was located RIGHT NEXT TO THE SPEAKER from which it emerged! In such close proximity to the mic, of course it was picked up by the microphone and of course it was recorded in sufficient volume! It was within an inch of the mic face! The pilots, however, were substantially further away from the speaker and the sound of the chime could not be heard over the cockpit noise due to air movement and the chatter accompanying the misoperating nosewheel light. But the light was only one factor involved. In addition to the low sound level of the chime, the light in the wheel well of the nose gear was inoperational as well, thus the flight engineer could not see the nose wheel target (an set of crosshairs on the gear strut itself which were visible when the landing gear was in the down position) in the dark through the tiny telescope installed for that very purpose. Procedures called for flying past the tower with the landing gear extended but the dark and no light in the wheel well conspired to force them to go to the next solution which was to replace the nose gear bulb with a new one. And here's where it got crazy and our concatenation of errors extends even more: There were no spare landing gear lamps in their designated location! Thus the only thing that could be done was to remove the offending light bulb and replace it with one that they knew worked: one of the lamps for the main gear. So far so good, except that the lamp for the nose wheel would not come out. Weird because they are designed to come out. When several attempts to remove it failed, it appears Captain Bob Loft got frustrated, bypassed the crew and went to do it himself. In the process, he turned toward the centre console and the flight data recorder shows he struck the control yoke per the flight data recorder. Applying pressure to the yoke is how one disengages the autopilot normally so why didn't Loft notice this? One might put that down to negligence but Loft was a veteran of many hours and did not make mistakes like that. The reason was the impact was not enough to cause the autopilot to disengage, YET IT HAD! Unbeknowst to the flight crew, the autopilot multiplier switch was set at 1/2 instead of 1. (This makes an autopilot useful to a large range of airplanes: for small airplanes, it was set at 1/2, for medium aircraft it was set at 1 and for very large aircraft it was set at 2. Once installed and set, this switch is never touched again!) Recall this was a comparatively new plane. Having been set at 1/2 instead of 1, the yoke took only 7.5 lbs of pressure to disengage instead of the requisite 15 lbs. One might ask why earlier crews never noticed. On flights autopilots are usually only ever engaged at altitude well away from other aircraft and never near the ground. Such autopilot behaviour would likely never be noticed while flying straight and level at altitude normally but near the ground such condition was a distinct liability. The copilot could not see the Captain's flight instrument panel and the two pilot's controls were disconnected from one another at the moment thus the copilot, monitoring his own and thinking the autopilot was on (he was monitoring his own instruments, not the Captain's ) only got clues something was wrong when the plane went into a mild descent. It did not help that beneath them and around them there were no lights on a dark night to highlight the ground. Thus it was that design and circumstance let them, Eastern Airlines and the crew and passengers down. It was sad night and the crew and the airline took the blame hard. In hindsight, it would perhaps have been better to have avoided use of the autopilot altogether and for Stockwell to have hand flown 401. There is evidence too that the thrust levers had been pushed forward right before impact upon realizing a mistake had been made but the crew were undoubtedly as mystified as ever at the situation! I encountered a company using this as an example of bad "cockpit resource management" I corrected them but found out months later they were still using it. I shake my head. The answer is a little more complicated than that. I believe a little later that a similar chime was ignored in a Polish LOT airline incident. Same cause and similar result! You can't know your airplane enough. Most flight crew would never know about autopilot installation and installation settings, nor would they be expected to. But if you are going to fly, you can never know enough!

    My dad was an an EAL employee when this crash happened and worked in the building at 10:03 (i was a toddler and, incidentally, i would become a proud, albeit short-term, employee of the EAL family 15 years later a couple of years before their bankruptcy filing). This crash dogged EAL throughout the 70s and into the 80s. It didn't help that it happened when horror novels and horror movies as a genre were beginning to grow and become high demand. I remember as a kid seeing the paperback novel "The Ghost of Flight 401" at pretty much every pharmacy and supermarket reading section for many, many years.

    In 1977 I was heaving a late dinner in the Bahamas. My wife and I started talking to 2 pilots for Chalk airline (I think Malard Duck Seaplanes). That was there weekend job. Normally they were mechanics for Eastern Airlines in the New York area. They told us that one night an eastern L1011 came in for militance and they were instructed to take out the galley and replace it with a new one. The suggested they could fix the old one , but no way, just replace it. Later they heard about the sightings in the old galley which had come from flight 401. As the 401 had a lot of usable parts after it bellied into the glades. In my plane I flew over the crash site a month later (now this is 5 years after the accident). We had the exact DME from MIA VOR. As we reached the spot (of course nothing on the ground anymore) my compass just started spinning for no good reason. After we left the area, no problems with the compass.

    As an avionics tech on a test flight of an L1011, I was trouble shooting a nose gear warning light. On takeoff the flight crew retracted the landing gear. I needed to go down into the electronics bay. There a little window that I could marks on the gear lock mechanism. I was in the cockpit. I lifted the hatch for the electronics bay. As I descended down the stairs, I took one glance at the at the gear lights. All the crew was looking at me!!! The thoughts of flight 401 flashed before my eyes. Fortunately we were climbing and the autopilot was engaged (I think). The bay is incredibly cramped, noisy and blowing cold air. I quickly verified the nose gear was indeed was up and locked. I gave a thumbs up as I ascended from the pit and locked the hatch

      Aviation is a completely different world 52 years later. An airline pilot once told me that, "The Federal Aviation Regulations are written in blood." Unfortunately, that's true. Not all, but a lot of advancements in aviation safety only happen after accidents... this was definitely the case. CRM and changes to autopilot systems and cabin safety procedures were implemented as a result of the tragic accident of flight 401. We really appreciate ur comments!!       All great points... I was just suggesting that there may have been several other factors. I would think that 34 years ago there was no crew resource manage (CRM) like there is today. Also, the cockpit voice recorder (CVR) and flight data recorder (FDR) quality was nowhere near what it is today. So, I was thinking that I didn't want to place blame, or make any kind of judgement of the crew. I would feel like an arm chair quarterback ~ TTHL

      The parts were backordered because of the demand and Eastern needed them to get their fleet of L10-11s operational. Lockheed eventually caught up and it went on to become a great aircraft!

    I remember reading this book fifty years ago. When I found it in the nonfiction section of the public library, I thought, at first, for it to be a mistake, but then I read the book and got goose bumps, several times. When I found the VHS in Blockbuster video (William Shatner version), I found that the movie didn't do the book justice, at all (William Shatner's overacting just killed the story). Several years ago I found the "other" movie and felt that it was a lot closer to the book.

    The first officer should have flown the plane and let the other two occupants work the problem. Pure incompetency at work as the flight crew killed their passengers. You never know what kind of morons are in the cockpit when you strap into the metal tube and put your life in their hands. The rest of the Boogeyman story is a childish bedtime story BS.
