I am sorry about those who loss their lives. I live in NYC now, but I used to work in the Seattle area from 1998-2003 with Maria Redin. I am very glad she was unharmed. As a retired Army vet, I know the real injuries are not visible. I hope everyone involved is doing well.

John Moore was my brother. He never recovered emotionally from what happened and passed away much too early.

I was one of the security guards on duty at Island Hospital that night. It truly was one of the most horrifying and heartbreaking things I've ever witnessed.

Just moved to Anacortes a few months ago and just found out about this. Very sad story. Unfortunate that so many large companies and organizations seem to foster unhealthy and unsafe attitudes and practices. Even more sad is the fact that the CEOs view injuries and lives lost in dollars rather than the people that are affected.

Nothing new just corporates putting profits first and human lives last. The heat exchanger was 40 years old.

Tesoro stock has trippled since April 10 2010. Seems like the "Disaster" made Tesoro more popular than it once was. Seems like it's doing the same thing now.

This is an amazing piece of investigative journalism, but I hope that 911 dispatcher at the beginning got fired. Laughing when someone calls you in fear because there was an explosion in a fuel refinery is far from acceptable. That, and dismissing reports rudely by going "yeah, we already got reports on it. Bye" and then hanging up is extremely rude and inconsiderate. As someone who worked dispatch for some years, the proper answer would be more along the lines of "We have recieved multiple reports on it and are currently investigating the incident. Thank you for your report"

Good journalism here. Tesoro as a company was short-lived. The refining assets were bought by Marathon Oil but the impact on the lives of the families affected will remain for the rest of their lives.

Does anyone ever wonder why Al Jazerra never honestly reports on oil refinery tragedies within the Arab Peninsula? Anyone?

It's actually ridiculous, would never happen in Australia where unions are still strong. OH&S is written into law, employer is always responsible.

Perhaps if it was required by law that the coprations CEO, corporate head of health and safety officer and the corporate lawyers are required by law to have their offices in the center of "Their Safe Facilities", these facilities might just be a little safer

Look what happened in Houston TX tons of chemical fires for days and days Go such Companies Tesoro location, great for Puget Sound oil spills and smells. Anacortes should buy it out and with State of Washington and put a state of art airport there instead and commuter locations
