U have to make new one. One advice Go to the hospital where u made medical certificate before i.e in josaa format, ask the doctor to fill the same in nitk format by showing old reports. Request him or give him money. Don't make new one, again you have to take tests.

Bro did you see the notification, madical certificate banne main hardly 10min work, gap certificate I don't think notification main likha hain dekh Lena ak baar jo hain wo bhi la sakta hain, OBC NCL ka dekh lena , or bhai more confirmation ke liye notification main contact diya hain uspe baat karo
I don't think so it's required but please check the list of documents carefully required for admission process while coming if it's written you might need it. In my knowledge NITK don't ask for it.
Near by University ke saath match hote hain if you are in team and your captain is active person. But other Nit compete in only InterNit tournament only.

You better make it in NITK Surathkal format, download it from website.

For the safe side you can bring any scholarships opening or something.
For sc st tuition fees is always free
