I think it's a representation of a Biblical angel. They were described as just eyes with wings attached to them (without human bodies or human-like forms like nowadays). At the beginning of 'Bedtime Story', as Madonna emerges from the ground, this eye with wings is behind her. Also, the letters inside the angel stand for the name of the Divine (YHWH). I guess, it's the name of God in Old Testament, too (Yahwah). It's connected to Judaism, Kabbalah, Christianity, which she's been studying and referencing for like 30 years. I might be wrong, though.
