It's possible that the gameover still has Date get pulled out safely but the investigation flops due to the lack of leads at the precise date and time, but the imagery suggests that despite the attempts to pull the Psyncer out, the mind collapses inward and crushes the eye under the weight, preventing any means of making a clean escape (as the successful Psync shows a clear eye and path back outside).
In the final Psync you see the same animation (I think), but it's followed by some static and a cleaner "light tunnel". So I assumed that these game overs just meant a mind swap took place, and the game cuts out before you see anything.
