No it is not required, but they say to hold onto if you can.
Be sure keep them in a safe place, make a photocopy on both sides. Start fill up your info at the back of the ticket. If not whoever finds your ticket will become theirs. 😂

What if the winner has a spouse? Does the husband and wife have to sign the ticket? And do they both have to complete 2 separate forms?
Only 1 name is allowed on the ticket. Typically it's the one who the cctv shows purchased it, but it's easy enough for them to simply go along with their spouse, provide a video clip or a signed note that states that their spouse is to be the one claiming. What you could then do is open a joint bank account at a private bank & have the funds deposited there.

Why does it take 7 to 9 weeks to pay out to a jackpot winner? with all the wireless banking that is done today it is hard to believe. or does the stste hold the cash for an extra 6 weeks so as to collect the interest even at 1 or 2 percent that is a lot of money?
It’s because if it’s power or mega they have to collect from every state for the jackpot payout, super lotto is California only so you’d get the money faster maybe even 3 weeks or less

According to the form, once they receive it, it takes up to 18 weeks to process. I’m sending my ticket via USPS Priority mail.
In person is just to go in and talk to someone, like to answer any questions or they can make sure you filled everything out properly. You don’t get your money any faster.

Yes, you can still claim prize without ticket. When you scan the ticket it is recorded in your Second Chance account, and you just fill out and mail claim form.
