Tank dead = healer fault
Healer dead = tank fault
DD dead = DD fault
That is classic rule since WoW exist.
I was main healer for many many years.
Yes, when your tank or even worst situation - main tank and you dead, that usually mean all raid dead.
BUT DONT FORGET that healer need to move too!
But when you moving you can be same efficient as when you static. More tank doing useles movement, to far, extra dislocation, LOS etc. That is bullet to tank.
I personally was feeling when tank have trouble and somethow my tank never dies.

That rule applied when it was OG vanilla, but at this point with people trying to rush content and parse by being the fastest it just doesnt hold the same water it did.
It can be the tanks fault or the healers fault, every incidence is independent and not some universal truth.
these rules existed long before WoW

this why, all need to do is try help person understand what he or she dong wrong not yell or insult or threat to kick out of guild.
