    Imagine being a passenger on board this flight. You’d obviously know that something seriously wrong has happened mid-flight. The eery silence and visibly seeing both engines not working.

      Specially when you're a former crew. I took a short regional flight on a Cessna Caravan and all I could think about was how to operate the door or how to evacuate in case of emergency (airline operated by very junior pilots)

      Came looking for this! Palermo ATC sounded incompetent and made the situation worse every time he attempted to speak. Jumbled words, stuttering, no clear confirmations, didn't acknowledge the request for rescue...

      @@hyperkoala1758 Agree. He was making the matter worse, since the crew needed to repeat themselves several times to be understood, losing valuable time. Worst case scenario, if the situation was slightly different, he could in fact be responsible for losing souls on that plane. Luckily, they had to ditch anyway (it seemed too late to glide when ATC was contacted), and ended up doing a good job.

    I don’t believe the pilots should’ve been charged. True, they could’ve glided to land, but not knowing about the incorrect FQI and what was going on due to the emergency, there is no way they could’ve realized that.

      It doesn't matter if knowledge of the FQI was necessary at that point; the constant focus on restarting the engines while they were aware that they were gliding should've have made them triage the emergency better and made them prioritize gliding/flying the plane, period. To me, it also sounded like the captain was panicking more than the FO

      They should've followed the correct procedures, no matter what. The F/O was busy with that, but the captain freaked out and kept on requesting to restart the engines while praying. That stuff doesn't keep an aircraft airborne. windmilling causes tons of drag.

      ​@@quantumdecoherence1289 Two sets of experienced ATR-72 pilots were put in a simulator. One managed to glide to Palermo and the other one ditched just short of the runway. Keep in mind that they knew what situation they were in. It's always easy to talk in hindsight. They made mistakes but blaming everything on the pilots just seems wrong to me.

      @@quantumdecoherence1289 I disagree. There were only three possible outcomes: getting an engine restart, gliding to Palermo, or ditching. Attempting to glide to Palermo would end the chance of restarting an engine and could have led to a less survivable ditching in anonymous ocean rather than in sight of ships capable of improving chance of rescue. They did the right thing, and 23 people survived. That would have been a risky bet with either alternative.

      In today's world it's quite common to study second language from early age at school, particularly English, which is one of the easiest tongues to learn according to many foreigners (me included). Compared to the actual piloting skills, it's figurative nothing, as even an average foreign teenager would be able to speak at that level.

    That's why an airplane has a captain--he is in charge of calling the shots, especially in the most pressing emergency situation where a millisecond could save lives, or kill everyone on board.
    In Sully's case, there were a few simulators which showed that had he decided to turn back immediately to Laguardia after the bird strike, they could've landed safely. But it was not a hundred percent guaranteed--it is a gamble for there is a very low chance of not crash landing, especially on a busy city block. But at that spur of the moment, Sully decided to ditch at the Hudson--again, no guarantee of surviving, but Sully and his crew did their best and they survived.
    In this case, the captain's firm decision was to restart the engines, considering that the faulty meters showed they have a good amount of fuel left. But when this failed, he decided to land near ships, try a water ditching, and hope that the ships would come to the rescue. And indeed, 23 lives were spared. That was still impressive given their dire predicament. In an alternate universe, the flight crew would've been awarded and not incarcerated.

      It looks to me the Italians put as much blame as possible on the pilots so they could divert attention from their totally incompetent air controller, who was totally useless from the beginning till the very end. I mean, if you're so unable to control your stress, you have no business in being in a control tower, even in a minor airport like Palermo.

      Landing in the ocean and landing in a river are completely different things. Ocean waves can disintegrate an airplane if they are strong enough and hit at a certain angle. You don't have to worry about ocean waves when landing in a river.

      In the case of Sully, the only way some of the simulations were successful was because the pilots in those simulators knew what was about to happen and upon losing both engines instantaneously began turning toward LGA. As Sully pointed out, you can’t conduct a simulation without factoring in at least a few seconds for human response to an unplanned event. When they agreed to add a few seconds after losing the engines before beginning the turn to LGA none of them were successful. Not a single one. In every case they crashed short of the runway and in populated areas. Sully made the only call he could, and it was the right one.

      They wished good luck. Not sure what else could have been done. What happened after impact with water? How did some people survive while some unfortunately lost their lives? Was it due to impact? @theflightchannel . Was an evacuation performed with help of boats?

      Sucks doesn't it but if they hadn't ditched near those boats likely everyone would have perished. There's also cold to account for - depending on the water temp; people would have trouble staying conscious more than a few minutes in most places. It's not like being near a shoreline where the water is heated by sunlight and still shallow. I had exposure once while swimming in cold water (about 8-10C). I'd been in there a lot long and I was exerting myself - and despite that I and being in peak physical condition, I almost got into trouble just a few dozen yards from shore! Now imagine being in your street clothes and then dropped into freezing cold water...

      Even typing this like you are allowing some sort of exception for them, like a judge over the situation, is the reddit that is infested in your mind spewing itself out. God could have ensured everyone lived or died in that spot if he felt like it. Your ability to read this and make sense of it and breathe is in his palm right now. Had one of the pilots prayed to the one true and living God with sincerity, maybe no souls would have been required on that flight.

      Hmmmmm.......is that really so? The crew did the best they could do ? Please read the report, and the consequential court verdicts. When two airline pilots are handed a prison sentence of 10 years each, for criminal negligent homicide, do we really believe they even tried to do things right ?
      They were simply walking around with their eyes wide shut. This accident should never have happened. Basically, the captain flew 4 legs the previous day in that aircraft. He did report the fuel indications were faulty, and the fault was repaired overnight. The guy then next morning sits in his seat, noticing "It seems as if we have much more fuel on board now than we had left last night!". And does not act on that. Commercial fuel indicators are quite precise. Why did he not take time to just calculate what they were supposed to have on board ? And when seeing the actual discrepancy, having the wing dipsticks checked ?
      They weren't too clever during the incident. It really should be know by heart that the overwhelming reason for losing all engines is "Our tanks must be dry!". There really isn't any reason for repeatedly trying to restart.
      Finally, there are reasons why prayers are not a part of the emergency procedures....

      What makes it so awful is that they were sentenced to 10 whole years in prison for "praying instead of flying", even though they did all they could to save the plane. They intended to ditch near boats (which was the right thing to do as the passengers had the highest chance of survival) and they succeeded in doing so. I imagine that most of the fatalities were caused by people inflating their life jackets inside the sinking aircraft. The pilots were even hailed heroes because they saved the majority of people on board but then they were put in jail simply because the judge is the very definition of an "internet atheist"

    I credit these pilots for fighting right down to the water.
    According to the info in this video, there were several conditions that led to this disaster. First, the wrong FQI was installed. Second, the crew did not check the fuel quantity loaded with the gauges before departure. Additionally, there didn't seem to be any enroute fuel quantity utilization calculation (may not have been possible due to the misleading nature of the FQI). Finally, their attempt to continually restart caused a lack of optimal glide rate (distance). Of course, a restart would have solved their problems and they weren't aware of fuel starvation as the cause, obviously. Hindsight is 20/20, so I can't fault the crew.
    RIP to the 16.

    I feel for the Captain and F/O, only half of the simulated attempts made it to the airport and that’s with the pilots having full knowledge of what’s going to happen. Being in a life or death situation with incorrect information I think it’s incredibly harsh to put them in prison for 10/8 years.

      You wouldn’t be charged for ditching the plane (the 2nd made it 1NM away). Sully didn’t after ditching in the Hudson. What they were charged for is not following correct procedures, maximising their chances of survival by flying at the best glide ratio, feathering the props and deciding to pray instead

      You definitely should be too, yet they charged the pilot with manslaughter for not feathering and for 'panicking' and praying, yet the cockpit recordings were omitted from the investigation?!! *What we just heard sounded to me like a CALM and COMPETENT crew* and the simulations were EXACTLY the same bullshit they claimed about Sully - "Two crews flew a simulator at ATR's facility in France from the same starting conditions. By feathering the propellers and reducing the speed to the optimal gliding speed, one made a landing at Palermo and *one ditched* one mile short of the runway. The fundamental difference was that *the simulator pilots knew what was happening* and responded accordingly ". *All this crew knew for sure was that it behaved like it was out of fuel but it COULDN'T be out of fuel* - unless the judges etc were in that cockpit they AREN'T in a position to judge. Disgusting.

I know it’s unreasonable but I cannot unlearn of this atrocity. I know Italy doesn’t give a crap about me nor will they lose anything but it’s shameful. Absolutely shameful. These poor men will have to live with this horrible tragedy every waking day of their lives. What if they’re so traumatized they are unable to fly ever again whether as passengers or pilots? I mean, the reality of what occurred is already punishment enough. I bet if they weren’t Muslim they would’ve been given much more sympathy. Sad.

      This was amazing pilot work! God bless them - ditching in water is a nightmare. Only a handfull few can do this. Also the lives lost was not because of landing but drowning! If the pilot wasn`t well trained they would all have perished.

      @@GeoffInfield What bother me most about those "judges", is who in Italy at the same year, the Linate airport and controlers were truly negligent and caused another disaster between a nordic and an american plane, yet, no one was charged, but here, they charged people who had already paid with blood.

    Hey, the pilots saved the majority of the passengers, I give them respect! They were calm and did (mostly) everything they could giving the circumstances.

      The flight crew were sentenced to prison the captain was sentenced to 10 years and the first officers to 8 years the court said because they panicked and focused more on praying and trying to start the engines than setting the aircraft up for optimal gliding distance by feathering the propellers which would have given them more than enough distance to land. They had 8 ATR72 pilots replay the same scenario in a flight simulator and all 8 pilots reached both of the airport runways without a problem. Unfortunately the flight engineer survived but he drown while trying to help people escape the aircraft, most of the survivors were seated in the back.

      @@thefreedomguyuk it wasnt the pilots' fault entirely, they just didnt know why the engines shut down in the first place. it was the engineer's and the person changing the fuel tank (this isnt the correct term im sorry i dont really know much about these things). maybe they did overlook something but it wasnt their fault in the first place and they never wanted this to happen.

      Pilots never once Stalled the plane nor enter a Spin with nose straight down that would’ve killed 100% of people.
      Pilot realized at that altitude to Glide plane slowly downward to closest Ships that can rescue survivors.
      If one Never actually flew a plane, especially a Twin Turbo Prop complex plane you haven’t a clue- your only guessing & repeating what others might of said.
      Anytime one survivor can walk away is a success.
      Many plane disasters end with 100% dead.

      They should have attempted a restart once or twice. If the engine won’t restart by that point, there is something wrong with it and it very likely will not restart. Nothing’s going to happen by doing it over and over. In fact, to have both engines flame out at approximately the same time pushes that likelihood to a practical zero. You’d know it isn’t a temporary issue that randomly happened but a complete loss of a flight-essential service, like fuel or maybe engine control systems.
      At that point, they needed to feather the props. So many accidents have involved pilots who could have made a safe emergency landing but either panicking and forgetting to feather or doing this exact thing.
      Feather your props, people.

      @@Mikinct watch the video again; it states CLEARLY that if pilots took the plane on the right glide path they could have reached land with the strong tail wind; pilots look unprofessional, many times you can hear the captain praying as his "effort" in an emergency situation? come on; he only focused on the ships and in restarting engines when CLEARLY that wasnt going to work; FIRST THING HE SHOULD HAVE DONE WAS FEATHER THE PROPS, avoiding stall and maximize lift on a gliding slope to reach the island

    Some additional information I found: The two pilots survived but the engineer was among those who died. Most of those who died drowned as they were unable to get out of the aircraft as a result of injuries from the initial impact. The captain was sentenced to 10 years in prison for manslaughter for praying instead of following the emergency procedures correctly. The first officer was given a sentence of 8-10 years.

      Was there any evidence the prayer disrupted the landing? Ditchings are inherently dangerous, even a perfectly executed one can end in fatalities, sea state can cause an otherwise soft ditch to become violent, panicked passengers can cause delays in evacuation. Unless they have some proof his prayer contributed to a mistake that greatly increased the force of ditching I think the authorities are just scape-goating. If anyone should be in jail it's the maintenance supervisor who allowed the wrong part to go into the aircraft, there's reams of paperwork involved in maintenance that had to be ignored for an incompatible part to end up being signed off.
      I will fault the pilots for not checking their fueling paperwork though, knowing the weight the fuelers claim to have put in is important for this reason. Many planes have crashed because of pilots not double-checking what ground crews have put in the plane, whether it be fuel or cargo weight.

      I think the main problem was that the ATR 42 & 72 FQI's had the same shape/dimensions, making it possible to install it on the wrong airplane. A small mistake with huge consequences.

      Maintanace and the fuel crew should not get all the blaime,,, are not pilots supposed to fill out a fuel log ? Or at least notice the aircraft seemed light ( 1800kg) on take off .

      @@ericwsmith7722 the pilot calculates what fuel they need, its the ground crews job to check it correctly and not rely just on gauges (can dip test it) and its the engineers job to make sure the part was correct.

      it's always a chain of errors; have the fuel-loading employees noted they didnt have loaded the required weight into that model plane (they have of course a listing of fuel capacity according to plane model) that should have raised the alarm and notified the crew/airline; but hey, they just went thru without acknowledging for the difference (or even worst, they noted it but did nothing about it !) and then you have the pilot incompetence, only "praying" and watching for boats, instead of following the protocol in cases of total power loss

      Heres the information and break down regarding passengers who survived and perished.
      Occupants. 39
      Passengers 35
      Crew. 4
      Fatalities. 16
      Injuries. 11
      Survivors. 23
      One of the four crew members died-a flight attendant-and 15 out of the 35 passengers died. The engineer who died was not a part of the flight crew, but had been called to the flight deck by the pilot and copilot after both engines failed; because he was not officially part of the crew, his death was accounted for as a passenger death. The flight's other flight attendant survived. All of the paying passengers were Italian, while the crew and the engineer were Tunisian. Autopsies indicated that many of the dead succumbed to the impact. Autopsies established that 8 passengers who received injuries during the impact were unable to escape from the aircraft due to their injuries and ultimately drowned. Most of the survivors were seated in the rear of the ATR 72, while most of the passengers who died were in the front. Three dead passengers, including the engineer who tried to help the plane's crew, were found on the seabed. The ANSV stated that the cause of death of these passengers was difficult to determine.

The International Federation of AirLine Pilots Association protested the flight crews' criminal sentences, calling the investigation "injustice" and the sentences "flawed."
      The criminal investigation and subsequent sentencing caused considerable controversy in Tunisia and, to a lesser extent, in the civil aviation world. The official investigation was accused of being one-sided and of ignoring mistakes made by Italian air traffic controllers. Unedited cockpit recordings leaked to the public demonstrated the Palermo air traffic controller as having a poor grasp of English, failing to assign the distressed flight its own radio frequency on which to communicate, and giving the pilots incomplete and/or useless information about their position. These cockpit recordings were omitted from the official investigation report.

Mistakes in well motivated, competent and committed people still happen at a rate that would be unacceptable for aviation safety if the result was a crash. That's where all the different layers of redundant safety come into play. That's why the aviation accidents are almost always so complex. One individual failure (human or mechanical) in general is not enough to bring a plane down. Otherwise, with hundreds of thousands of flights every day, it would be raining planes. To put it in perspective, "99.99% good" would not be good enough, it would mean about 15 crashes per day every day.

      1- Mistake-proof design: Make the 2 different but identically-looking fuel indicators incompatible wo it is impossible to install the incorrect one.
      2- Mistake-proof maintenance procedures: Scan a label in the part that you are going to install, if the part number is not approved for the airplane serial number that you are working on, you don't get the maintenance card that tells you how to do the work and that you use to record the work done.
      3- Better maintenance quality control checks: Include an actual test of the system, for example where add 50 gallons of fuel and then check that the indicated quantity increased by 50 gallons.
      4- Operational procedures: In the first flight of each day make a "stick check" of the actual fuel quantity and before EACH flight cross check the indicated fuel quantity with the calculation of "initial fuel + fuel up quantity - consumed fuel".

    The "Engineer" that installed the wrong part was the fault for loss of these people and A/C. Obviously he didn't read part numbers and effectivity.

      When you work in that kind of business, where you are responsible for people's lives, you must practice repetitious counter-checks. I am a nurse, and every time I prepared an injection, I checked the drug and dose 3 times before I gave it. I recall once on the third check I found an error, and thankfully corrected it before giving it. But it just goes to show, you simply cannot be too careful when lives depend on your actions. If the engineer had practiced this type of checking the disaster would not have happened.

    Another great video! I tend to agree that the pilots had no way of knowing their tanks were actually empty. I think that they thought by trying again and again, maybe they'd restart. Just a case of a simple error leading to chain of events that ended tragically.

      If both engines on a twin fail within 100 seconds of each other, you already know that you are flat out of fuel. Period. Anybody that cannot immediately conclude that should not be flying. Period. Remember that the engines are completely independent of each other. One engine might quit due to a mechanical failure; for both to quit concurrently, you are out of fuel. Now, for those folks who think that landing on water is preferable to a light (no fuel) landing on soil, they are wrong. Water hit at speed might as well be concrete; water is incompressible. If you pancake in on land without fuel fire risk your chances of 100% survivability is way up there. I would have shot for Palermo; if you don't make it, you can pancake it in.
      One other thought: flying over water without enough altitude to glide to land is not something I do. Altitude is your friend. These guys did not have much of that; it cost them.
      The pilots do not "stick the tank" because that is a high-wing aircraft and there is no ladder to get up over the wing. But you can look at the fuel delivery ticket and see what got loaded. If you assume dry tanks and the ticket does not show enough pounds fuel loaded to make the destination, then you do not take off. Tell the fuellers to go load some more. Nothing worse than fuel left behind in the truck.

    What attracts me the most about airplane crashes is the investigation process. It is so sophisticated. All nations join in. You think other countries don't have the same knowledge, but man so every country has their flight specialists, it's awesome. It is the same all around the world. You can pull up a wreck that happened and compare it to another to see where the issue was. The fact that they go and retrieve the airplane to see what exactly happened, is so awesome. You know, looking for one of those wrecks in the ocean is worse than looking for a needle in a haystack. Great channel.

    I believe the FQI design was changed so this was no longer possible and they couldn’t be installed onto the wrong aircraft by mistake

      Can't agree more. In many crashes I seen on this channel, along with the crew, bad or flawed design of aircraft's components too was equally responsible. But surprisingly investigators never have hold manufacturer too equally responsible (along with crew) for crash. This crash also no exception. Manufacturer of this aircraft is ATR responsible for crash! In other cases it's Boing or Douglas etc.

    Not only the Chicken Captain didnt keep the Vglide speed that could make them reach Palermo. He also stalled the airplane high and made it split the fuselage in half, when hit stalled too hard, which killed most on board. Some guys freeze on controls on emergencies..

    The pilots did an excellent job of saving lives in this situation. I would be curious to know if aiming for the two ships before ditching meant that more people made it out alive or not. Also, if they had been able to glide all the way to Palermo, there is still no guarantee that they would have made the runway, and crashing on land would have probably resulted in more fatalities than ditching in the water. That crew handled a bad situation just about as well as they could have in my opinion.

      @@amari2509 yes and no. Yes the insurance underwriters are employed by money-hungry cockroaches eager to defer blame onto flight crew on order to wash their hands of liability. Hence simulator re-creation after the fact.
      However in this case heavy fault falls upon flight crew. Consider:
      1 Flight crew were aware of faulty fuel gauges, could have visually and manually checked fuel level and cross-referenced with anticipated travel distance. They didn't.
      2 Flight crew had THREE aces in their hands when double engine failure occurred:
      A Excellent altitude cushion of 37000 feet.
      B High-wing aircraft capable of successfully ditching on water.
      C Aircraft with low stall speed.
      Flight crew PISSED AWAY all three aces.
      Bottom line: flight crew ROYALLY f**ked up.

      I had hope for them, and they did well considering the props weren't feathered, but once I saw the descent rate and the way the plane struck the water, I knew the outcome wasn't ideal.

      Bad job 16 died because the ground crew failed to do their jobs ! No standards in place for checking any maintenance work ? Putting in the wrong part should have been found BEFORE the plane went back into service. Good thing Tuninter is gone now.

    Not really sure how I feel about this one. If one engine had failed, sure, suggesting that they should have feathered the prop immediately might be a fair call. Immediately feathering both props, which, as far as I know, can't be unfeathered inflight and instantaneously committing to a glide approach is a big call to make, especially considering that both pilots had no reason to suspect that they had run out of fuel. As far as requesting to descend goes, I'm assuming it has something to do with the single engine operating ceiling. Without knowing glide ratio for this aircraft, I doubt they would have made it to Palermo after deciding to ditch either.
    The real culprits here are the ground crew.

      ...I suppose but dual engine failure at 23,000 is likely a fuel/fuel delivery issue, at over 4 miles up his first priority should have been to set a glide speed to the safest landing strip available, once established, then calculate what altitude they could/need to lose to hit the landing then try spinning the engines with the props. He looses about 800ft/minute in glide, that's plenty over 20 minutes and at 135/140 IAS with a tail wind helping a ground of 2 to 3 miles a minute, so over 40 miles. He should have made it to Palermo, and then try to restart once he knew he could make the landing. His decision to quickly descend seemed to make his situation impossible.

      @@t8283287 the crew landed the airplane onto the water in one piece, so i would say that the pilots did a good job in flying the plane, especially considering how many other pilots had fucked up water ditchings in the past EVEN THOUGH they were 'flying as best as they could for as long as they could'. the crew only started praying once the plane was about to crashland into the water. what else could they have done at that altitude after exhausting all of their altitude and speed?

    I'm skipping the whole part pertaining to the installation of the wrong FQI. It's obvious that was the first link in the chain that led to the disaster.
    What's more important in determining the pilots' fault is whether there was a requirement for the pilots to cross-check the amount of fuel in the tanks (using a dipstick or another means) with the fuel gauge in the cockpit?
    If there was no such a requirement, you can't blame the pilots (or the ground crew) for not doing so. If there was a requirement to do a cross-check, the fault for this is with the people who were required to do it.
    The crew's decision to try and restart the engines (more than once) is perfectly reasonable, given that the information they had was:
    - their fuel tanks had ~2 metric tons of fuel and
    - there were no warnings indicating any operational issues immediately prior the flameout of the engines
    The flight crew's belief that they may not be able to reach terrafirma and perform a deadstick landing might have been further strengthened by the fact that they (with both engines flamed out) now had 2 tons of dead weight in the form of fuel, at least based on what the fuel gauge was telling them.

    It's all very well to say "if everything goes perfect and they immediately go into full gliding configuration, they could made it without ditching." Yeah but that's not realistic, is it? I seem to remember hearing some people say thing about Sullenberger. The reality is they are going to work on restarting the engines if they think they have fuel. Only way they would quickly feather the props and go full glide quickly is if they realized they were out of fuel and that the engines never would restart. I don't fault the pilot for this ditching.
