Even if you put your friends into absolute death situations such as falling from 10,000 feet from the sky, burned to death, drowned deep underwater, shot to death, or run over by a train, they somehow survive and end up in the hospital with no limbs or organs lost, and somehow know you were behind it and have them charge you for the hospital fees.
But they don't survive in the final mission that's strange
Trevor is not happy about him in the hospital, but he is about one of the protagonist paying

I think they'd still message you saying they got out of the hospital and you don't pay their charges, when I switched over to michael I ran amanda over with my car and after she got out of the hospital, I didn't pay them cuz I didn't have enough, you'd still get the same messages, just without paying

I assume, if you have no money, then no consequences will be there, but if you have less money than 5000, than I think some percentage is deducted from your balance. Don't know exact figure though.
