How does our child get out of his crib? Mission Impossible

Our 2 yr old keeps getting out of his crib and we figured out how! We also realized why he throws his dock-a-tot and blankets over the crib railing ... for cushining! Even though we put him in a slip sack and zip tied PVC poles to the crib bars, he still finds an opening to get out!
